BALF中SOD和溶菌酶检测对肺癌的诊断意义 被引量:1

A STUDY ON THE SIGNIFICANCE OF THE DETECTION OF SOD AND LZM IN BALF FOR LUNG CANCER DIAGNOSIS

摘要 对74例肺癌和76例良性肺病患者进行了血清及支气管肺泡灌洗液(BALF)中超氧化物歧化酶(SOD)和溶菌酶(LZM)的测定,结果表明:①肺癌患者血清及BALF中SOD水平明显降低,以癌患侧BALF中SOD降低最明显,阳性率为71.8%。腺癌较鳞癌及小细胞癌低,周围性肺癌较中央型低,Ⅲ、Ⅳ期较Ⅰ、Ⅱ期低。②肺癌患者血清及BALF中LZM水平降低,阳性率为46.3%和59.7%。小细胞癌降低最明显,晚期肺癌较早期更低。③SOD和LZM联合检测诊断肺癌的价值明显优于单项指标检测,其敏感性、特异性分别高达87.8%和89.5%。提示:通过血清及BALF中SOD和LZM联合检测对肺癌早期诊断、病理分型及预后判断有一定意义。 Superoxide dismutase(SOD)and lysozyme (LZM) in both the serum and bronchoalveolar lavage fluid (BALF)were tested in 74patients with lung cancer and 76 patients with benign lung disease. the results indicated:①The level of SOD in the serum and BALF decreased obviously in lung cancer patients and most obviously in BALF on the affected side, the positive rate was 71.8%.The level was lower in adenocarcinoma than that in either squamo-cellular carcinoma or small-cellular carcinoma. In comparing the peripheral type to centric one and stages Ⅲ, Ⅳ to stages Ⅰ,Ⅱ, the level of the first-mentioned was lower.②There was a decrease of LZM in the serum and BALF and the positive rate was 46.3% and 59.7% respectively.The decrease was most evident in small-cellular carcinoma,the level of which was lower in the late stage than in the early stage of lung cancer.③The value of the combined test of SOD and LZM for diagnosing lung cancer was significantly better than that of single test. The sensitivity and specificity of combined test reached 87.8%and 89.5% respectively. The result suggests that the combined detectionof SOD and LZM in both serum and BALF is helpful to the early diagnosis,pathological type and prognosis of lung cancer.
