摘要
目的:分析比较应用外固定器两种手术操作方法对固定针松动、感染的影响,探讨防止固定针松动和感染的外固定器最佳穿针操作技术。方法:共240例患者,A组120例,为传统手术和护理方法;B组120例,为改进手术和护理方法,分析比较两组固定针感染发生率及感染发生时间。感染定义为针道炎性渗出伴疼痛,细菌培养阳性,使用抗生素有效。结果:B组感染发生率低,且第一次出现感染的时间延迟。结论:避免术中热损伤和局部血肿形成,术后应用抗生素,局部采用碘伏纱布加压包扎等方法,能够大大降低针道感染和固定针松动的发生率。
Objective： Two protocols for the operative technique and care of the pin-site with external fixation were compared prospectively. Methods： There were total of 240 patients with 120 in group A and 120 in group B. Infection was defined as an episode of pain or inflammation at a pin site, accompanied by a discharge which was either positive on bacterial culture or responded to a course of antibiotics. Results： Patients in group B had a lower proportion of infected pin sites and the time to the first episode of infection was longer （P〈0.001）. Conclusions： The risk of pin-site infection might be lower if attention is paid to avoid thermal injury and local formation of haematoma during surgery and if after-care includes the use of an alcoholic antiseptic and occlusive pressure dressings.
