摘要
目的探讨单边外固定器治疗四肢骨折的术后并发症及防治措施。方法对970例四肢骨折患者应用单边外固定器治疗,分析骨折治疗中产生各种并发症的原因并提出防治措施。结果并发症:针道感染21例(2.2%),针孔渗液76例(7.8%),固定针松动18例(1.9%),断针4例(0.4%),膝关节功能活动受限17例(1.8%),骨折延迟愈合7例(0.8%),骨不连3例(0.3%),再骨折2例(0.2%)。结论单边外固定器对胫腓骨、肱骨干、股骨转子间骨折及儿童股骨干骨折疗效满意,并发症较少。对火器伤和多发伤骨折的救治更有其独特优越性。而对成人股骨干骨折并发症较多,适应证要严格掌握。针孔感染、固定针松动及关节功能受限是主要并发症。正确操作,针孔干燥、引流通畅是控制并发症发生的有效措施。
Objective To study the complications and the countermeasures of extremities fractures treated by unilateral external fixator. Methods 970 cases of extremities fractures were treated with unilateral external fixator stabilization. The complications were analyzed and the countermeasures were carried out. Results The complications were： pin tract infection in 21 cases （2. 2% ） , pin hole drainage in 76 （7. 8% ）, pin loosening in 18 （ 1.9% ） , pin breakage in 4 （0.4%） , knee functional motion limitation in 17 （ 1.8% ） , delayed union in 7 （0. 8% ） , nonunion in 3 （0. 3% ） , and refracture in 2 （0. 2% ）. Conclusions Unilateral external fixator is effective for diaphyseal tibiofibular and humeral, intertrochateric, and children diaphyseal femoral fractures, and has special advantages on treating gun shot injuries and multiple fractures. However, it has high morbidity rate for adult femoral fractures and the indication should be controlled strictly. The main complications are pin tract infection and pins loosening and joint function limitation. The effective countermeasures are proper manipulation and keeping pin tract drainage unblocked.
