摘要
【目的】检测人体肝癌组织及癌旁肝组织中表皮生长因子受体EGFR及其突变体EGFRvⅢ的表达情况,并分析其临床意义。【方法】用免疫组化法检测58例肝细胞癌患者肿瘤及癌旁肝组织中EGFR、EGFRvⅢ的表达情况,与患者的临床资料进行相关分析,同时对EGFR和EGFRvⅢ不同表达的患者无瘤生存期和总生存期进行生存分析。【结果】肝癌组织EGFR、EGFRvⅢ表达的阳性率分别为74.1%、63.8%;癌旁肝组织EGFR、EGFRvⅢ表达阳性率分别为81.0%、77.6%。肝癌组织EGFRvⅢ的表达情况与肿瘤病理分级(P=0.028)及术前AFP值(P=0.035)呈负相关。癌栓及/或卫星灶(P<0.001)和肿瘤病理分级(P=0.023)是肝癌术后无瘤生存期的主要影响因素;肝癌患者术后总生存期的主要影响因素包括患者的肿瘤家族史(P=0.014)、肿瘤最大径(P<0.001)、癌栓及/或卫星灶(P=0.039)以及肿瘤的病理分级(P=0.002)。生存分析显示肝癌组织EGFR及EGFRvⅢ表达阴性的患者与表达阳性的患者无瘤生存率及总生存率均无明显差异。【结论】肝细胞癌患者肿瘤组织及癌旁肝组织中均存在EGFR以及EGFRvⅢ高表达,EGFR以及EGFRvⅢ过度表达可能是肝癌发生过程中的事件。肝癌组织EGFRvⅢ的表达情况与肿瘤病理分级及术前AFP值呈负相关,肝癌组织EGFR及EGFRvⅢ的表达情况不是肝癌预后的独立影响因素。
[Objective] To detect expression of epidermal growth factor receptor (EGFR) and its variant EGFRv Ⅲ in human hepatoeellular carcinoma (HCC) and hver tissue just around the tumor (LAT), and to analyze their ehnieal significance. [Methods] Fifty-eight HCC patients entered this study. Expression of EGFR and EGFRv Ⅲ in tumor and LAT were detected with immunohistoehemieal assay. Then correlated analysis was done within different expression status and ehnieal data. Survival analysis was done for tumor-free survival and overall survival of the patients with different expressions of EGFR and EGFRv Ⅲ. [Results ] Expression rates of EGFR and EGFRv Ⅲ were 74.1% and 63.8% in HCC, respectively. Expression rates of EGFR and EGFRv Ⅲ were 81.0% and 77.6% in LAT, respectively. Expression of EGFRv Ⅲ in HCC had negative correlations with cancer pathologic grading (P=0.028) and preoperative AFP value (P=0.035). Tumor embelus and/or satellite tumor (P〈 0.001) and pathologic grading (P=0.023) were main influential factors for tumor-free survival. The influential factors for overall survival of HCC patients included family history of cancer (P=0.014), tumor size (P〈 0.001), tumor embolus and/or satellite tumor (P=0.039), and pathologic grading (P=0.002). Survival analysis indicated tumor-free survival and overall survival were of no difference between the patients with different expression of EGFR and EGFRv Ⅲ. [Conclusions] EGFR and EGFRv Ⅲ are beth overexpressed in HCC and LAT. Overexpression of EGFR and EGFRv Ⅲ might be an event during hepatoeellular carcinogenesis. Expression of EGFRv Ⅲ in HCC is negatively correlated with the cancer pathologic grading and preoperative AFP value. Expressions of EGFR and EGFRv Ⅲ are not independent prognostic factors of HCC.
