Effect of IL-18 on peripheral blood and lamina propria lymphocyte proliferation and proinflammatory cytokine secretion in patients with inflammatory bowel disease

摘要 目的:分析IL-18对炎症性肠病患者外周血和肠黏膜固有层T淋巴细胞增殖和促炎症细胞因子分泌的影响。方法:收集12例克罗恩病(CD)、25例溃疡性结肠炎(UC)患者和15例结肠镜检查正常者(对照)的肠黏膜组织,分离外周血单个核细胞(PBMC)和肠黏膜固有层单个核细胞(LPMC),体外培养并用IL-18刺激,48h后,利用ELISA检测上清液中干扰素(IFN)-γ、IL-2、IL-4、IL-5和肿瘤坏死因子(TNF)-α水平,采用3H-胸腺嘧啶掺入法检测LPMC的增殖情况。结果:IL-18刺激后,CD患者PBMC和LPMCIFN-γ、IL-2和TNF-α分泌水平高于UC和对照(P<0.05);CD患者的LPMC增殖率亦高于UC和健康对照。结论:IL-18可激活CD患者淋巴细胞的增殖,PBMC和LPMC分泌大量促炎症细胞因子,可能参与了CD的发病过程,体内阻断IL-18可能对CD患者免疫治疗有帮助。 Aim : To investigate the effects of IL-18 on proliferation and proinflammatory cytokine secretion of peripheral blood mononuclear cells (PBMC) and intestinal mucosal lamina propria mononuclear cells (LPMC) from patients with inflammatory bowel disease. Methods:PBMC and LPMC, isolated from 12 patients with Crohn's disease, 25 patients with ulcerative colitis and 15 healthy controls, were cultured and stimulated with anti-CD3 and IL-18, and supernatant was harvested after 48 h culture to determine interferon ( IFN)-γ, IL-2, IL-4, IL-5 and tumor necrosis factor (TNF) -α using ELISA. Proliferative response of LPMC was also determined by 3H-thymidine incorporation after 60 h stimulation with IL- 18. Results: PBMC and LPMC from patients with Crohn's disease, stimulated with anti-CD3 and IL-18, produced marked- ly higher levels of IFN-γ, IL-2 and TNF-α than those from ulcerative colitis patients and the controls ( P 〈 0.05 ). Proliferative response of LPMC from Crohn's disease patients significantly increased than those from ulcerative colitis patients and healthy controls. Conclusion: IL-18 could induce lymphocyte proliferation and proinflammatory cytokine secretion in patients with Crohn's disease. Blocking IL-18 may be helpful in treating Crohn's disease.
