摘要
目的研究89Sr和153Sm-EDTMP治疗前列腺癌、乳癌、肺癌及胃肠道癌骨转移所致骨痛的疗效。方法注射用89SrC l2以35~40μC i/kg计算剂量,静脉注射,每次注射间隔为3个月;153Sm-EDTMP静脉给药,以0.5~1.0 mC i/kg计算剂量,每隔1~2个月注射1次。结果前列腺癌、乳癌、肺癌及胃肠道癌经89Sr与153Sm-EDTMP治疗骨痛的总有效率分别为95.7%、90.0%;93.8%、89.5%;53.8%、53.3%及66.7%、62.5%。且无论89Sr还是153Sm-EDTMP,前列腺癌和乳癌的疗效与肺癌相比均有明显差异(P<0.05),胃肠道癌与肺癌相比无明显差异(P>0.05)。主要不良反应表现为骨髓暂时抑制,且以153Sm-EDTMP为甚。结论放射性核素治疗以成骨为主的骨转移灶,止痛效果明显,方法简便,副作用小,且以89Sr为首选。
Objective To investigate the clinical effect of ^89Sr and 153Sm-EDTMP treatment on pain palliation in the patients with bone metastasis of prostate cancer, breast cancer, lung cancer and gastrointestinal cancer. Methods 58 patients with bone metastasis of cancers were intravenously injected 35 -40 μCi/kg body weight of agsr, once three months. 62 patients with bone metastasis of cancers were intravenously injected 0. 5 - 1.0 mCi/kg body weight of 153Sm-EDTMP, once a month or two months. Results The effective rate of pain palliation of ^89Sr and 153Sm-EDTMP treating bone metastasis of prostate cancer, breast cancer, lung cancer and gastrointestinal cancer were 95. 7%, 90.0% ; 93.8%, 89. 5% ; 53.8%, 53.3% and 66. 7%, 62. 5% respectively. Both ^89Sr and 153Sm-EDTMP, The efficacious rate of pain palliation in prostate and breast cancers was significantly higher than that in lung cancer （P 〈 0.05） . The efficacious rate of pain palliation in gastrointestinal cancer was the same as lung cancer （P 〉 0. 05 ） . The most side effect of ^89Sr and 153Sm-EDTMP treatment is the inhibition of marrow. The side effect of 153Sm-EDTMP is more seriously than ^89Sr . Conclusion Radionuclide ^89Sr and 153Sm-EDTMP treating bone metastases of cancers had the advantages of simplicity, less side effects and definite pain palliation effect and ^89Sr treatment is the first choice.
