摘要
目的探讨小儿支气管哮喘与HLA_DRB1的相关性。方法采用序列特异性引物_聚合酶链反应方法(PCR/SSP)对78例支气管哮喘患儿和82例健康儿童进行HLA_DRB1基因分析,患儿的年龄为1~14岁,平均6.5岁。HLA_DRB1有19个等位基因,包括DR1_DRW18、DRW52和DRW53。计算各个位点的基因分布频率、危险性比值比OR值;并进行卡方检验,筛选有意义基因。结果支气管哮喘患儿HLA_DRB19的基因频率为10.8%;HLA_DRB110为11.5%;HLA_DRB11为1.92%。以上3个基因型在疾病组与正常组之间进行卡方检验,χ2值分别为4.39、4.44、6.7,与正常对照组比较差异有显著性(P均<0.05),其余16个位点差异无显著性。观察组与对照组间进行逐个等位基因比较,计算比值比OR结果HLA_DRB11的OR值为0.25;HLA_DRB19的OR值为2.58;HLA_DRB110的OR值为2.43。结论HLA_DRB19和HLA_DRB110可能是汉族小儿哮喘的易感基因;而HLA_DRB11可能为保护性基因。
Objectives To study the correlation between the HLA-DRB1 and asthma in children, and to explore what impact of HLA-DRB1 gene type has on childhood asthma. Methods Eighty-two cases of normal children and 78 cases of asthma children, including 41 boys and 37 girls aged from 1--14 years, with the average of 6.5 years, were studied. Among the 78 cases, 52 had mild asthma and 26 had severe asthma. All children were Han nationality. PCR/ SSP (sequence specific primers PCR) method was used in the study. HLA-DRB1 contains 19 allels, including DR1-DRW18, DRW52 and DRW53. This resolution was achieved after performing 19 PCR reactions for each sample. Each tube had its own specific primers and different products to detect DRB1 gene type. Then we calculated the gene frequency of each type, RR(relative risk) and Chi-square test. Results In asthma children, the gene frequency of HLA-DRB1^*9 was 10.8%, HLA-DRB1^*10 11.5% and HLA-DRB1^*1 1.92% while in control group, HLA-DRB1^*9 was 4.8%, HLA-DRB1^* 10 5.5% and HLA-DRB1^*1 6.7%. All 3 gene types were compared with Chi-square test, their Chi-square values being 4.39, 4.44 and 6.7, respectively (P〈0.05). Significant difference was found in this 3 gene types, but not found in other 16 gene types. Each allele was compared to observing group and control group. The RR of HLA-DRB1^*9 was 2.58, of HLA-DRB1^*102.43, of HLA-DRB1^*10.25. The RR of HLA-DRB1^*9 and HLA-DRB1^*10 increased, but the RR of HLA-DRB1^*1 decreased. Conclusions The result showed that HLA-DRB1^*9 and HLA-DRB1^* 10 were probably the predisposing gene and HLA-DRB1^*1 was probably resistant gene in Han nationality asthma children.
