摘要
目的探讨“分离式”微温(30~34℃)氧合血持续逆灌对移植供心保护的临床效果。方法供心在移植开始时即行微温氧合血经冠状静脉窦持续逆灌保护心肌。结果14例同种原位心脏移植术中13例心脏自动复跳,1例电击除颤1次成功复跳,长期心功能良好。结论“分离式”微温氧合血持续逆灌可缩短供心缺血时间;减少术中晶体液用量和避免钾的过度使用;获得理想的心肌保护效果。
OBJECTIVE To study the myocardial protection effect on donor heart with “Separate -type” continuous retrograde perfusion of lukewarm oxygenated blood. METHODS “Separate - type” continuous retrograde perfusion of lukewarm oxygenated blood was initiated immediately from the beginning to the aortic cross - clamping off during the donor hearts transplanted. RESULTS Fourteen transplanted hearts were recovered to sinus rhythm after reperfusion and showed well cardiac function. CONCLUSION “Separate - type” continuous retrograde perfusion of lukewarm oxygenated blood could shorten the time of myocardial ischemia, reduced quantity of crystal solution and potassium used, and acquired better myocardial protection.
