摘要
目的观察白三烯拮抗剂对激素抵抗型(steroid-resistant,SR)或激素依赖型(steroid-dependent,SD)哮喘患者外周血单个核细胞(peripheral blood mononuclear cells,PBMCs)中转录因子核因子κB(NF-kappaB,NF-κB)、活化蛋白-1(activator protein-1,AP-1)活性和细胞因子IL-4I、L-5I、L-10水平的影响。方法收集15例SR或SD哮喘患者的外周静脉血,常规分离PBMCs,每个标本分成4组,分别加入孟鲁司特(培养12、18、24 h)和空白对照(培养24 h),分别收集上清液和细胞沉淀,酶联免疫吸附法(enzyme-linked immunoadsorbent assay,ELISA)测定上清液中IL-4I、L-5I、L-10的表达,采用电泳迁移率分析法(electrophoretic mobility shift assay,EMSA)测定细胞沉淀中NF-κB、AP-1活性。结果予孟鲁司特培养后NF-κB、AP-1活性和细胞因子IL-4、IL-5水平较对照组均显著降低(P<0.05),IL-10水平较对照组显著升高(P<0.05)。结论孟鲁司特可以影响SR或SD哮喘患者中转录因子的活性和细胞因子的表达,可能成为SR或SD哮喘联合治疗较有前景的策略。
OBJECTIVE To observe the effect of leukotriene antagonist on nuclear factor-kappa B(NF-kB) and activator protein -1 (AP-1) activities and the levels of interleukin-4 ( IL-4), interleukin-5 (IL-5) and interleukin-10 (IL-10) with steroid-resistant(SR) or steroid-dependent (SD)asthma. METHODS Fifteen patients with SR or SD asthma were selected based on their clinical responses to prednisone therapy. The peripheral blood mononuclear cells (PBMCs) were separated and incubated with phytohemagglutinin (PHA) in vitro for 24h in the absence of or for 12, 18 and 24 h in the presence of montelukast, respectively. Supernatants were collected. Expressions of IL-4, IL-5 and IL-10 mRNA were determined by enzyme-linked immunoadsorbent assay(ELISA). The NF-kB and AP-1 activities of PBMCs were measured by electrophoretic mobility shift assay (EMSA). RESULTS In the presence of montelukast, the activities of NF-KB and AP-1 were significantly lower than those of incubation in media alone, meanwhile, the expressions of IL-4 and IL-5 were significantly lower and IL-10 were significantly higher, respectively(P〈0.05). CONCLUSIONS Montelukast could influence the activities of NF-kB and AP-1 and the levels of IL-4, IL-5 and IL-10, SR or SD asthma could be treated more effectively.
