摘要
目的:探讨颈椎黄韧带退变和脊髓型颈椎病的相关性。方法:收集10例脊髓型颈椎病和19例颈椎外伤患者的黄韧带标本,对其进行厚度测量及组织学检查;用Woessner法及盐析法测定黄韧带中Ⅰ型与Ⅱ型胶原含量的比值;对黄韧带厚度与Ⅰ、Ⅱ型胶原含量的比值进行相关性研究。结果:颈椎黄韧带退变后弹力纤维含量下降,排列紊乱,胶原纤维含量增加,颈椎病组黄韧带厚度和Ⅰ/Ⅱ型胶原含量的比值与对照组比较有显著性差异(P<0.05);颈椎不稳节段的黄韧带胶原含量较其它节段显著增加。结论:颈椎不稳与黄韧带退变密切相关,黄韧带中Ⅱ型胶原过度增加可能是脊髓型颈椎病发生的重要因素之一。
Objective:To study the relationship between degenerative cervical ligamentum flavum and cervical spondylotic myclopathy.nethod:Ligamentum flavum sample from cervical spondylotic myelopathy and cervical vertebral trauma were acquired and pathologically examined.The ratio of Ⅰ type/Ⅱ type collagen was determined by Woessner's method and salting out.The correlation between thickness of degenerative cervical ligamenturn flavum and the ratio of Ⅰ type/Ⅱ type collagen was explored.Result:In the CSM group,the decrease of elastic fibers and increase of collagen fibers were observed in cervical ligamentum flavum.On the contrary,thickness of ligamentum flavum,the ratio of Ⅰ type/Ⅱ type collagen were markedly increasing compared to that in trauma group.The contents of collagen in cervical instable segment increased significantly.Conclusion:Decrease of the elastic fibers and significant increase of Ⅱ type collagen quantity and disorder of arrangement are present in the ligamcntum flavum of CSM.The degeneration of ligamentum flavum might be the main factor to induce CSM.The cervical instability is associated with the degeneration of ligamentum flavum notable.
