代谢综合征患者血清瘦素与血脂、肥胖及胰岛素抵抗关系的研究 被引量:8

摘要 目的:探讨代谢综合征(metabolicsyndrome,MS)患者血清瘦素(leptin)水平与血脂、肥胖和胰岛素抵抗的关系。方法:60例MS患者和28名年龄相匹配的正常对照者,按体重指数(BMI)将MS患者分为肥胖组和非肥胖组,采用ELISA方法测定受试者空腹血清瘦素水平,同时检测其身高、体重、腰围、臀围、血压、血糖、血脂、尿酸及血清胰岛素水平,并计算BMI、腰臀比(WHR)及胰岛素抵抗指数(HOMA-IR)。结果:MS患者肥胖组和正常对照组相比其血清瘦素水平明显升高(14.71±5.74与3.14±1.28,P<0.01),非肥胖组和正常对照组相比其血清瘦素水平明显升高(13.52±5.66与3.14±1.28,P<0.01)。肥胖组和非肥胖组相比其血清瘦素水平差异无显著性。相关分析显示血清瘦素与BMI、腰围、臀围、WHR、收缩压、舒张压、甘油三酯、尿酸、空腹胰岛素、HOMA-IR呈显著正相关(分别为r=0.59,r=0.64,r=0.53,r=0.64,r=0.47,r=0.53,r=0.49,r=0.36,r=0.39,r=0.32;p<0.01),与血糖呈显著正相关(r=0.23;P<0.05),与高密度脂蛋白(HDL-C)呈显著负相关(r=-0.40;P<0.01),而和总胆固醇(Chol)及低密度脂蛋白(LDL-C)无相关性。结论:MS患者血清瘦素水平明显升高,并存在一定的瘦素抵抗,且与肥胖、腹型肥胖及胰岛素抵抗有明显相关性,因而瘦素抵抗可能在肥胖、胰岛素抵抗和MS的发生及发展中具有重要的作用。
