摘要 目的研究分析宫颈腺癌(adenocarcinomaoftheuterinecervix,AUC)肿瘤中组织微血管密度(mi-crovesseldensity,MVD)与患者预后的关系。方法采用免疫组织化学方法(SP法)检测23例FIGO临床Ⅰ期和Ⅱ期宫颈腺癌肿瘤组织的MVD及ki-67表达,应用SPSS10.0软件分析肿瘤MVD与宫颈腺癌临床病理参数及预后的关系。结果该组病例中,肿瘤MVD的范围为22.2~51.6,中位值为35.7(36.6±8.4),宫颈腺癌MVD水平与患者年龄、FIGO分期及有无淋巴结转移无关,与组织学分级和Ki-67标记指数显著相关(P<0.05)。微血管计数≥35.7的病例术后生存率低于微血管计数<35.7的病例(P=0.0271)。Cox比例风险模型证实MVD水平可作为宫颈腺癌患者的独立预测指标。结论宫颈腺癌中肿瘤血管新生与肿瘤进展有关,高MVD患者生存率明显低于低MVD患者,MVD水平可独立用于预测宫颈腺癌患者的预后。 [Objective] The purpose of this study was to discuss the relationship between intratumor microvessel density (MVD) and prognosis, as well as the adenocarcinoma of the uterine cervix(AUC). [Methods] Twenty-three cases of FIGO clinical stage Ⅰ and Ⅱ adenocarcinoma of the uterine cervix were examined by immunohistochemical studies (SP method) with anti- FⅧ-Rag, anti-CD34, and anti-ki-67 antibodies. We assayed the intratumor MVD and labeling index of Ki-67 and analyzed the relationship between them and the clinicopathological parameters (including age, Histological grading, FIGO stage and Regional lymph node Metastasis). Meanwhile, we discussed the relationship between them and the survival. All of the analyses were performed with SPSS10.0. [Results] MVD ranged from 22.2-51.6, with a median value of 35.7(36.6±8.4). There was no correlation between the MVD and the age, FIGO stage, the lymph node metastasis. However, MVD significantly correlated with histological grading and the labeling index of Ki-67 (P 〈0.05). The count of microvessel was associated with survival by Kaplan-Meier analysis. Patients with high MVD (〉35.7) showed poorer survival than those with low MVD(≤35.7) (P =0.0271). A Cox multivariate analysis revealed MVD to be an. independent predictor of unfavorable prognosis. [Conclusion] Tumor anglogenesis correlates with tumor progression in adenocarcinoma of the uterine cervix. High MVD is associated with poorer survival rate. Moreover, MVD is an independent prognostic indicator in patients of the adenocarcinoma of the uterine cervix.
