95°DCS治疗股骨粗隆间及粗隆下粉碎性不稳定骨折 被引量:42

Treatment of Comminuted and Unstable Fractures of Intertrochanteric and Subtrochanteric of Femur with the Dynamic Condylar Screw

摘要 目的研究和评价95°DCS(动力髁螺钉)在股骨粗隆间及粗隆下粉碎性不稳定骨折中的应用.方法自1998年8月~2003年1月对23例髋部骨折(股骨粗隆间骨折15例,股骨粗隆下骨折8例)采用闭合复位95°DCS内固定治疗.结果全部病例均得到10个月~3年随访,主要指标为颈干角、髋关节伸屈活动度等.骨折全部愈合,无钢板松动、断裂、骨不连等并发症,1例轻度髋内翻.优16例,良6例,优良率95.7%.结论95°DCS能提供有效的固定,其治疗髋部骨折是DCS的另一种用处,是值得推荐的治疗粉碎性不稳定股骨粗隆间和粗隆下骨折的方法之一. Objective To investigate the treatment of comminuted and unstable intertrochanteric and subtrochanteric fractures by dynamic condylar screw. Methods From August 1998 to January 2003, twenty- three patients with an unstable hip fracture were treated by close reduction and fixation with dynamic condylar screw. There were 15 cases of intertrochanteric fracture (Evan's classification: 13 cases of Type one and 2 cases of Type two) and 8 cases of subtroehanteric fracture (Seinsheimer's classification: 3 cases of Type Ⅱ, 4 cases of TypeⅢ , 1 cases of Type Ⅳ ). Results All patients were followed up for average 18 months (ranged from 10 months to 3 years). The range of motion, the collodiaphyseal angle and Dong' s evaluation standard were adopted as main parameters to evaluate the outcome of the operation. All fractures healed without the three complications of hardware fixation. There was 1 case of lightly coxa vara. According to Dong' s evaluation method, the excellent or good rate was 95.7 %. Conclusion 95"DCS can provide reliable fixation and treatment of hip fractures, which is a new application of DCS, especially in comminuted unstable intertrochanteric and subtrochanteric fractures. It is a recommendable method in practice.
