摘要
目的 探讨股骨小粗隆缺损后对股骨上端生物力学的影响 ,分析加压滑动鹅颈钉治疗股骨粗隆间骨折失败的原因 ,探求防止髋内翻的方法。方法 用人体股骨模拟股骨小粗隆骨折造成股骨小粗隆缺损和股骨小粗隆广泛缺损 ,模拟股骨头垂直受力 ,测量小粗隆对侧应变大小。结果 小粗隆缺损后 ,其对侧的张应力增加 6 0 % ;小粗隆广泛缺损后 ,其对侧的张应力增加370 %。结论 股骨小粗隆缺损 (治疗中未做复位固定 ) 。
Objective To investigate the effect of the biomechanics on the proximal femur when the lesser trochanter is defected.Methods The cadaver femur has been made to lesser trochanteric fracture.The opponent strain of the lesser trochanter has been tested on biomechanical experimental machine under the vertical force of compression.Results In case of lesser trochanteric defect the opponent stress increases 60%.In case of widely defect it increases 370%.Conclusion The lesser trochanteric defct is the main reason leading to fails of DHS treatments.
