7.62 mm弹颅脑火器伤实验研究——Ⅰ、脑贯通伤、脑切线伤和颅骨切线伤的比较 被引量:16

摘要 本实验用7.6mm弹致伤犬头,制作了脑贯通伤、脑切线伤和颅骨切线伤动物模型。通过创伤弹道学、病理学、血和脑脊液酶学、血脑屏障通透性定量试验以及呼吸和循环系统病理生理效应的综合研究比较,得出如下结论:(1)高速弹所致的颅脑火器伤,无论何种用型均可造成脑的弥漫性损伤,引起远隔部位的颅脑血肿;(2)脑损伤的程度取决于弹丸穿越部位、弹丸动能的大小和暂时空腔是否接近脑室;(3)高速弹颅脑伤能破坏血脑屏障,导致严重的脑水肿;(4)呼吸和循环系统功能障碍是高速弹颅脑伤的显著特点,也是死亡的主要原因;(5)测定血和脑脊液中乳酸脱氢酶含量是判断受伤程度和预后的一种简便而有价值的方法。 The experimental models of cranioccrebral wounds caused by 7.62 mm bullets, i.e.the penetrating craniocerebral injury, the tangential brain injury and the tangential skull injury,were establishsd in dogs.The craniocerebral ballistios,cramocere-bral pathology, ssrum and cerebrospinal fluid total lactate dehydrogenase, blood-brain barrier permeabilities,and the pathophysiology of cardiovascular and respiratory systems were studied.These results suggest that : l.Thess injuries of high-velocity missile can all caus: general brain damage and intracranio-hematomas s 2.The ssver-ity of the wound depends on the site of the injury, the kinetic energy of the missile force and the effect of ths temporary cavity ; 3.The brain injury could seriously damage the blood brain barrier, leading to brain edema ; 4.The dysfunction of respiratory and cardiovascular system is the fatal complication endangering the life of ths subjects ; 5.Estimating srrum and cerebrospinal fluid total lactate dehydrogenas; is a simple and valuable way to judge the severity and prognosis of this injury.
