The expression of HSP47 in renal tissues of chronic renal disease patients and its significance

摘要 目的探讨热休克蛋白47(Heatshockprotein47,HSP47)在慢性肾病患者肾组织中的表达及其与肾组织纤维化的关系。方法选择50例慢性肾脏病患者,根据肾组织纤维化程度积分为3组:轻度损害组20例,中度损害组15例,重度损害组15例。对照组5例,为肾肿瘤患者,取其切除肾脏远离肿瘤部分经病理检查结果正常的肾组织为对照研究对象。应用免疫组织化学方法检测4组受试对象肾组织中HSP47、Ⅳ型胶原(COL-Ⅳ)的表达,并将HSP47分别与肾脏纤维化损害程度、COL-Ⅳ、血清肌酐、内生肌酐清除率等进行相关分析。结果(1)在纤维化病变肾组织中,HSP47主要表达于病变区小管上皮细胞、肾间质细胞及肾小球内系膜细胞、上皮细胞内,且其表达量随肾脏纤维化程度加重而增加(P<0.01);(2)HSP47表达分别与COL-Ⅳ表达、肾组织纤维化积分、血清肌酐成正相关(P<0.01),与内生肌酐清除率成负相关(P<0.01)。结论随着慢性肾脏病患者纤维化病变程度加重,HSP47表达量逐渐增加,它可能通过促进胶原合成增加,参与肾脏纤维化的发生、发展。 Objective To investgate the correlation of heat shock protein 47(HSP47) with fibrotic renal tissue.Methods Fifty patients with chronic renal disease were divided into three groups according to the glomerular sclerosis and renal interstitial fibrosis score. 20 patients were in light fibrosis group(score=0 to 2), 15 in middle fibrosis group(score=3 to 5), and 15 in severe fibrosis group(score=6 to 8). Five specimens of normal renal tissue were obtained from microscopically normal sections of renal cell carcinoma patients and served as control group. Heat shock protein 47(HSP47) and type IV collagen in renal specimens were examined by immunohistochemical staining and computer image analysis system. Clinical data of the 50 patients were collected when they received biopsy. The relationship among the expression of HSP47, type IV collagen (COL-Ⅳ),the score of renal fibrosis, serum creatinie (Scr) and clearance of creatinie (Ccr) was analyzed using SPSS10.0 system for windows.Results (1) In the normal control group, the level of HSP47 expression was very low in the renal tubular epithelial cell. In contrast, there was high HSP47 expression in the tubular epithelial cells, renal interstitial tissue, glomerular epithelial and mesangial cells of trial groups. The intensity of HSP47 expression was positively correlated with the degree of glomerulosclerosis and tubulointerstitial injury(P<0.01). (2) The expression level of HSP47 was positively correlated with COL-Ⅳ(r=0.871, P<0.01), renal fibrosis score (r=0.747,P<0.01) and Scr(r=0.517,P<0.01), and negatively correlated with Ccr (r=-0.667,P<0.01).Conclusion Increased expression of HSP47 correlates with the degree of renal fibrosis. This suggests that HSP47, associated with increased collagen production, may play an important role in the development of renal fibrosis.
