摘要
目的探讨彩色多普勒超声在鼻咽癌颈部疗效评价中的应用价值。方法选择初治、颈淋巴结阳性的鼻咽癌患者161例,给予常规放疗,治疗结束时进行颈部临床触诊和彩色多普勒超声检查,将两种检查方法所得结果进行对比分析。结果161例中,触诊和彩色多普勒超声检查淋巴结消失率分别为90.1%(145/161)和75.8%(122/161)(χ2=11.5995,P<0.01),以淋巴结残留枚数来比较,触诊和彩色多普勒超声检查残留率分别为10.1%(55/547)和28.7%(157/547)(χ2=60.8714,P<0.01)。彩色多普勒超声检查出较多位于颈部肌肉深面而触诊遗漏的淋巴结。结论彩色多普勒超声检查能减少鼻咽癌颈部疗效评价中淋巴结的漏诊率,而且有助于鉴别残留淋巴结的良恶性。
Objective To explore the value of Color Doppler Ultrasonography in the assessment of treatment effectiveness of neck radiotherapy in the patients with nasopharyngeal carcinoma. Methods Total 161 de novo nasopharyngeal carcinoma patients with neck lymph node invasion were enrolled in the study.After conventional radiotherapy,the results of neck palpation examination and Color Doppler Ultrasonography for lymph nodes were compared and analyzed. Results The disappearance rate of lymph nodes for palpation examination and Color Doppler Ultrasonography was 90.1%(145/161) and 75.8%(122/161),respectively (χ~2=11.5995,P<0.01).The residual rate of lymph nodes for palpation examination and Color Doppler Ultrasonography was 10.1%(55/547) and 28.7%(157/547),respectively (χ~2=60.8714,P<0.01).The Color Doppler Ultrasonography was more effective in detecting the neck lymph nodes deeply in muscles in comparison with palpation examination. Conclusion The Color Doppler Ultrasonography is able to reduce the false-negative rate of neck lymph nodes for patients with nasopharyngeal carcinoma treated by radiotheapy and is helpful in the differentiation of benign and malignant lesions of lymph nodes.
