摘要
目的:评定医学院校大学生遭遇具体生活事件对抑郁发生的影响,并探讨两者之间的关系。方法:于2004-03对北京大学医学部医学本专科学生957人进行问卷调查。采用青少年生活事件量表(共27个项目,每项事件的刺激强度根据经历生活事件时的心理感受,分别量化为1为无影响、2为轻度、3为中度、4为重度、5为极重度)评估生活事件对抑郁状态的影响,采用青少年抑郁量表(由20个项目,5级评分标准构成,1为无、2为轻度、3为中度、4为偏重、5为重度,抑郁评估的指标采用20个项目的总平均分),以抑郁状态为因变量,以与之显著相关的27个生活事件项目为自变量进行多元因归分析。结果:纳入数据957人,生活事件量表有效数据中第13项亲友死亡、21项被罚款、其他(2)项目参与者951人,第15项当众丢面子参与者950人,其余各项目参与者均为952人。抑郁量表有效数据参与者882人。①生活事件各项目得分:各个项目的平均分在1.070±0.405至2.120±1.025之间,其中得分项目最高的10个项目分别是考试失败或不理想(2.120±1.025)、学习负担重(2.090±1.047)、被人误会或错怪(1.940±0.987)、与同学或好友发生纠纷(1.940±1.047)、长期远离家人不能团聚(1.790±1.060)、生活习惯(饮食,休息等)明显变化(1.770±0.979)、受人歧视冷遇(1.740±1.006)、
AIM:To assess the influence of specific life events on the occurrence of depre ssion in students of medical college,and investigate the association between the m. METHODS:Totally 957 students of medical collage in Peking University received the questionnaire survey in March 2004.The influence of life events on the depre ssive status was assessed with adolescence life event scale,which consisted of 2 7 items,according to the psychological feeling during the experience of life eve nts,the stimulative intensity of each life event was quantified to 1 as no influ ence,2 as mild,3 as moderate,4 as severe and 5 as very severe.The adolescence de pression scale included 20 items,and the 5 degree scoring standard was used:1 a s none, 2 as mild, 3 as moderate, 4 as a little severe and 5 as severe,and the t otal average score of the 20 items was used as the index of depression,it entere d the multiple regression analysis taking depressive status as dependent variabl e and the significantly correlated 27 life events as independent variables. RESULTS:The data of 957 cases were involved; For the effective data of life ev ents scale, except that 951 cases participated in the items of the 13th item 'd eath of relative or friend' and 21st item 'being fined' and the 2nd subitem in 'others', 950 cases participated in the item of the 15th item 'losing face pub licly',all the other items involved 952 cases;For the depression scale,882 cases had effective data.①The score of each item in life events scale:The average sc ore of each item ranged from 1.070±0.405 to 2.120±1.025;The 10 items with the highest scores in order were 'failed in or unsatisfactory to the exam'(2.120±1 .025),'heavy learning load' (2.090±1.047),'being misunderstood or wrongly bla med' (1.940±0.987),'disputed with classmates or friends'(1.940±1.047),'bein g far away from family and unable to be together the them for a long time' (1.79 0±1.060),'obvious changes of living habits (diet,rest,etc.)' (1.770±0.979),' being discriminated or prejudiced by others' (1.740±1.006),'death of relative or friend' (1.740±1.265),'dislike to go to school' (1.720±1.005) and 'being in serious illness of family member or friend' (1.700±1.135);②Scores of depres sive status evluation:The average score was 1.854±0.67,it was less than 2.00 in 549 cases (62.2%),2.00 to 2.99 in 269 cases (30.5%),3.00 to 3.99 in 56 cases (6.4%),4.00 to 4.99 in 7 cases (0.8%),and 5.00 in 1 cases (0.1%);③Correlatio n analysis between each item of life events and depressive status:All the items in the adolescence life event scale had significant positive correlation with de pression in students of medical college(P< 0.01);④Multiple regression analysis between each item of life events and depressive status: It was indicated that 7 kinds of life events, such as 'dislike to go to school','heavy learning load', 'financial difficulties of family','failed in or unsatisfactory to the exam', 'pressure in entering a higher school','being discriminated or prejudiced by o thers' and 'losing face publicly',entered the regression equation of depression ,and they played a greater role in predicting depression. CONCLUSION:Ten life events related to study pressure, interpersonal relationsh ip,health adaptation and loss have greats stress influence on students of medica l college, whose general depressive levels are mild and below.The life events of college students have extensive positive correlation with depression.Life event s were correlated to depression of collage students of medicine,and there were s even events may be utilized in the prediction of depression.The 7 kinds of life events,such as 'dislike to go to school','heavy learning load','financial dif ficulties of family','failed in or unsatisfactory to the exam','pressure in en tering a higher school','being discriminated or prejudiced by others' and 'los ing face publicly',play a greater role in predicting the occurrence of depression.
