摘要
目的:对比生长抑素类似物奥曲肽联合质子泵抑制剂奥美拉唑与单用奥美拉唑对非静脉曲张性上消化道出血的治疗作用。方法:84例非静脉曲张性上消化道出血患者,随机分成两组,第1组42例为联合用药组,给予善宁0.1mg首剂静注,然后0.3mg静脉滴注,维持12h,25μg/h;奥美拉唑40mg加入生理盐水100ml静脉滴注。第2组为对照组给予奥美拉唑40mg加入生理盐水100ml静脉滴注,两组均给予去甲肾上腺素冰盐水口服、止血药,并行补液等处理。观察止血时间、再出血率及不良反应。结果:联合用药组疗效明显好于对照组,差异有显著性(P<0.05),联合用药组再出血率明显低于对照组,差异有显著性(P<0.05)。结论:奥曲肽联用奥美拉唑治疗非静脉曲张性上消化道出血效果好,再出血率低,无明显不良反应。
Objective:To compare the efficacy of somatostatin analogue Octreotide combined porton pump inhibitors Omeprazole vs only Omeprazole in controlling acute non-variceal gastrointestinal bleeding.Methods:A total of 84 patients with acute upper gastrointestinal bleeding due to duodenal orgastric ulcer were randomly divided into 2 groups.Group Ⅰ,the joints,consisted of 42 patients were treated with Octreotide 0.1mg i.v.Followed by continuous i.v.infusion at a rate of 0.6mg/d for 72h.Group Ⅱ,the controls,consisted of 42 patients were treated with Omeprozole 40mg/d by continuous i.v.infusion for 72h.Results:The time of bleeding stop was shorter in the joint groups than in the controls( P <0.05).The rebleeding rate was lower on the treatment groups in Group Ⅰ than that in Group Ⅱ ( P <0.05).Conclusion:Octreotide combined Omeprazole shows more effective in controlling acute non-variceal gastrointestinal bleeding.No adverse effects are found in the two groups.
