摘要
目的:探讨X线三维适形放疗对晚期胰腺癌的治疗可能性和治疗效果。方法:1999年9月至2003年7月,采用X线三维适形放疗治疗晚期胰腺癌28例。根据包绕计划靶区的等剂量线(80%的等剂量曲线)确定治疗剂量,5～10Gy/次,隔日1次,使计划靶区边缘剂量达35～50Gy。结果:治疗结束8～10周后评价近期疗效,完全缓解(CR)率为32.1%,部分缓解(PR)率为39.3%,无变化(NC)率为28.6%,总有效(CR+PR)率达71.4%。1年累计局部控制率为75%,1年累计生存率为57.1%。不良反应主要为恶心、呕吐,患者均能耐受。结论:X线三维适形放射治疗胰腺癌近期疗效满意,不良反应轻,为不能手术的晚期胰腺癌患者提供了一种有效治疗手段。
Objective To explore the possibility of X-ray Three Dimensional Conformal Radiotherapy for advanced pancreatic cancer. Method From september 1999 to July 2003,28 patients with advanced pancreatic cancer(9 with Stage Ⅱ,13 with Stage Ⅲ and 6 with Stage Ⅳ)were treated with X-ray Three Dimensional Conformal Radiotherapy. The total radiation dose was 35-50Gy,5-10Gy per fraction(80% isodose line),qod. Results The complete response(CR) rate,partial response(PR) rate and no change(NC) rate was 32.1%,39.3% and 28.6%,respectively. The overall response(CR+PR) rate was 71.4%.The 1-year local control rate and 1-year survival rate was 75% and 57.1%,respectively. The acute radiation reactions mostly were nausea,and were tolerable. Conclusion X-ray Three Dimensional Conformal Radiotherapy is a safe and effective treatment for patients with advanced pancreatic cancer.The better local control rate and long-term survival rate can be expected.
