摘要
目的探讨多聚环氧化合物(PC)预处理同种异体静脉的实验应用。方法30只实验犬,60侧股动脉移植段,随机分为三组,经PC(n=30)和戊二醛(GA,n=20)交联处理的异体犬静脉以及自体静脉(NS,n=10)分别植入犬股动脉缺损处,以多普勒超声检测或DSA观测血管植入后3、7、14、30、60、90d的通畅率,并取移植血管标本进行扫描和透射电镜观察,图像分析和测定内膜/中膜比值。结果移植早期(14d),PC组、GA组和NS组累积通畅率分别为64.2%、42.86%、90%。PC组在术后30、60、90d的累积通畅率分别为48.15%、21.88%、13.13%,与GA组比较有显著差异(P<0.05)。PC组7d内皮细胞向静脉爬行,90d覆盖静脉表面。GA组90d内膜面有局灶状分布的新生内皮细胞。NS组30d内皮细胞重新覆盖内膜面。术后7d,三组的内膜/中膜比值无显著差异(P>0.05);术后30、90d,三组之间比值有显著差异(P<0.05)。结论PC预处理同种异体静脉植入动脉环境后,内膜再内皮化、内膜增殖和90d内累积通畅率均好于GA组。
Objective To explore the feasibility of a novel polyepoxy compound ( PC ) to pretreat allogenic veins in the experimental study. Methods Thirty mongrel canines including 60 femoral arterial transplanted sites were divided randomly into three groups. Allogenic jugular veins pretreated by PC (n =30) and glutaraldehyde ( GA, n = 20) , respectively, as well as 10 homogenic jugular veins (NS) were implanted into the femoral lesions. The patency of each graft was observed by Duplex and DSA on d3 , d7 , d14, d30, d60, d90 after operation, and at the same time the specimens of each graft were taken to be observed by SEM and TEM. The intima/media (I/M) thickness ratios were calculated and analyzed statistically among groups. Results The cumulative patency of PC group, GA group and NS group were 64. 2% , 42. 86% and 90% , respectively, in the early stage of transplantation (14 d) . PC group reached the level of 48. 15% , 21.88% and 13. 13% on d30, d60, d90, respectively, and the statistic results between PC and GA groups were significantly different (P <0.05). After transplantation the endothelia cells began to migrate from the host artery to the graft veins on d7 in PC group, the similar process occurred in GA group and NS group on d14 and d30, respectively. There were no significant differences of the I/M thickness ratios among the three groups on d7 (P >0. 05) , but there were significant differences between any two of the three groups on d30 and d90 (P <0.05). Conclusion The allogenic veins pretreated by PC had better results in endothelializaion and intimal hyperplasia as well as accumulative patency on d90 than those in GA group.
