Non-islet cell tumor-induced hypoglycemia due to overproduction of insulin-like growth factor-Ⅱ——two case reports and literature review

摘要 2例非胰岛细胞肿瘤性低血糖 (NICTH)患者血清中胰岛素样生长因子 (IGF) Ⅱ水平增高 ,IGF Ⅰ、IGF结合蛋白 (BP) 3、GH水平受到抑制 ,IGF Ⅱ /IGF Ⅰ异常增高 ,其中 1例间质来源的肿瘤患者术后循环GH IGF轴恢复至正常范围 ,表明循环GH IGF轴改变在NICTH的定性诊断中具有重要价值 ,IGF Ⅱ /IGF The relationship between abnormal circulating GH-insulin-like growth factor (IGF) axis and hypoglycaemia was explored in 2 cases of non-islet cell tumor associated hypoglycaemia (NICTH). Serum level of IGF-Ⅱ was increased, the levels of GH, IGF-Ⅰ, IGFBP-3 were decreased to some extent, and IGF-Ⅱ/IGF-Ⅰ (molar ratio) was significantly increased in these two cases. In one case, circulating GH-IGF axis returned to normal after complete removal of the tumor. In the diagnosis of NICTH, the abnormal circulating GH-IGF axis is quite valuable and IGF-Ⅱ/IGF-Ⅰ appears to be a more sensitive parameter.
