螺旋CT在胸膜孤立性纤维瘤中的诊断价值探讨 被引量:3

摘要 目的:分析探讨胸膜孤立性纤维瘤(solitary fibrous tumor of the pleura,SFTP)的螺旋CT(multi slice computedtomography,MSCT)表现特点,以提高对本病的认识及诊断率。方法:12例SFTP患者均经病理证实,术前均接受MSCT平扫+增强检查,其中10例患者术前行X线检查,分析总结SFTP的影像学表现特点。结果:本组12例SFTP患者MSCT均表现为胸腔单发实性肿块;其中91.7%(11例)良性,8.3%(1例)恶性;66.7%(8例)为胸部单发巨大肿块占位性病变,边缘清晰光整;33.3%(4例)为胸壁结节状肿块占位性病变。肿块直径为:2.0~12.5 cm,平均(8.6±4.5)cm;其中58.3%(7例)的肿块密度均匀,41.7%(5例)的肿块密度不均匀。25%(3例)出现少量胸腔积液。MSCT增强扫描:25%(3例)表现为特征性的"地图样"强化。镜下观察见肿瘤细胞呈梭形、编织状排列,细胞核呈长梭形,少数可见核分裂象,间质大量胶原纤维沉积,肿瘤内可见坏死,部分肿块可见丰富的血管。vimentin、CD34均为阳性。结论:MSCT平扫+增强检查可清晰显示SFTP病灶的大小、形态及与周围组织的关系,MSCT表现有一定的特征性,结合病史及实验室检查术前多可作出正确的诊断。
