注射光敏剂与激光照射间隔时间的光动力学疗法实验研究 被引量:3

Laser Irradiation Time in Malignant Tumor Photodynamic Therapy after Photofrin Infusion:an Experimental Study

摘要 目的 探索恶性肿瘤光动力学疗法注射光敏剂后的最佳照光时间.方法 制作昆明小鼠S180肿瘤实验动物模型155只,随机分为2组,对照组为单纯肿瘤组,不给以光敏剂和激光照射.实验组尾静脉注射光敏剂Photofrin 7.5 mg/ml并以能量密度为120 J/cm2的630 nm激光照射肿瘤,注射光敏剂后激光照射时间间隔分别为1、5、15、30和60min,6、12、24和48 h相应的分为T1~T9组.在光照后即刻、光照后48 h每组各处死1只小鼠获取肿瘤标本,另有12只小鼠作长期疗效观察.光照后每天观察肿瘤及皮肤变化情况并计算小鼠生存率.光照后40 d处死所有小鼠获取肿瘤标本进行组织病理学检查和电子显微镜检查.结果 光照后小鼠皮肤反应以T1~T5组为重;T6~T9组光照后患侧肢体瘫痪的发生率较低;光照后40 d T6~T9组部分小鼠修复皮肤易形成瘢痕;用光敏剂后1~12 h肿瘤复发率低;从皮肤组织学修复方面看PDT的光照时间应当在应用光敏剂后6 h以内;电镜观察细胞结构显示T5、T6、T7组光照后即刻线粒体肿胀最为明显.讨论PDT的最佳光照时间在注射光敏剂后6~12 h. Objective To explore the best time to irradiate the tumor after the Photofrin injection to get the best photodynamic therapy (PDT) efficacy. Methods The control group was not given Photofrin and laser irradiation. The test groups were given Photofrin and laser irradiation. According to the different irradiation interval time after Photofrin injection, the groups were divided into 9 groups. We got the specimens from the sacrified mouse at once and 48h after irradiation in every test group. It was assured that 12 mice in every group were observed for long survival study. All of the mice were sacrificed to get the specimens on the 40 days after irradiation. The Photofrin of 7.5mg/kg was injected into the mouse body. The implanted tumor of the mouse was irradiated with microlens fiber at energy fluence of 120J/cm2. The tumor specimens were also took to do routine pathological examination and electron microscopic examination. Results Skin reaction was gently from the appearance of the skin after irradiation and the paralysis incidence was lower in T_6-T_9. But some of the cured T_6-T_9 mice had skin scar formation. The best time to reduce tumor relapse was irradiated at 1-12h after photosensitizer injection. Under electron microscope, the cytochondriomes tumescence was severe in T_5,T_6,T_7. And there was no difference among the test groups in tumor cells necrosis under electron microscope and optic microscope. Conclusions It indicated the best irradiating time is at 12h after Photofrin injection.
