摘要
目的分析中年良性前列腺增生(BPH)患者性激素水平与胰岛素抵抗的相关性。方法回顾性分析某三甲医院体检中心2020-01至2022-12参加健康体检的中年BPH男性患者135例临床资料,根据胰岛素抵抗指数(HOMA-IR)水平分别纳入BPH胰岛素抵抗(IR)组55例(HOMA-IR>2.69)和BPH非IR组80例(HOMA-IR≤2.69)。比较两组一般资料、性激素指标和糖代谢指标。分析性激素水平与糖代谢指标的相关性。结果IR组FBG、INS、HOMA-IR水平高于非IR组,血清T、FSH水平低于非IR组,E_(2)和E_(2)/T水平高于非IR组。血清睾酮(T)与糖化血红蛋白(HbA1c)(r=-0.27,P=0.002)、HOMA-IR(r=-0.23,P=0.007)呈显著的负相关,血清T与空腹胰岛素(FINS)呈负相关(r=-0.20,P=0.021),血清雌二醇(E_(2))与FINS(r=0.19,P=0.023)、HOMA-IR(r=0.18,P=0.041)呈正相关,E_(2)/T与FINS(r=0.30,P<0.001)、HOMA-IR(r=0.30,P<0.001)呈显著正相关,血清卵泡刺激素与FINS(r=-0.25,P=0.004)、HOMA-IR(r=-0.25,P=0.004)呈显著负相关,血清泌乳素与空腹血糖呈负相关(r=-0.21,P=0.013)。结论胰岛素抵抗可能与中年BPH患者多种性激素水平相关,对BPH预防有一定临床价值。
Objective To analyze the correlation between sex hormone levels and insulin resistance in middle-aged patients with benign prostatic hyperplasia(BPH).Methods The clinical data of 135 middle-aged male patients with BPH who participated in physical examination in a top three hospital from January 2020 to December 2022 were retrospectively analyzed,and according to the level of the insulin resistance index(HOMA-IR),55 cases in the BPH insulin resistance(IR)group(HOMA-IR>2.69)and 80 cases in the BPH non-IR group(HOMA-IR≤2.69)were included,respectively.General data,sex hormone indexes and glucose metabolism indexes were compared between the two groups.The correlation between sex hormone levels and glucose metabolism indexes was analyzed.Results The levels of FBG,INS and HOMA-IR in IR group were higher than those in non-IR group,serum T and FSH levels were lower than those in non-IR group,E_(2)and E_(2)/T levels were higher than those in non-IR group.Serum testosterone(T)showed a significant negative correlation with HbA1c(r=-0.27,P=0.002),HOMA-IR(r=-0.23,P=0.007),and serum T was negatively correlated with fasting insulin(FINS)(r=-0.20,P=0.021).Serum estradiol(E_(2))was positively correlated with FINS(r=0.19,P=0.023)and HOMA-IR(r=0.18,P=0.041),and E_(2)/T was significantly positively correlated with FINS(r=0.30,P<0.001)and HOMA-IR(r=0.30,P<0.001).Serum follicle-stimulating hormone(FSH)was negatively correlated with FINS(r=-0.25,P=0.004)and HOMA-IR(r=-0.25,P=0.004);serum prolactin was negatively correlated with fasting blood glucose(r=-0.21,P=0.013).Conclusions Insulin resistance may be related with the levels of various sex hormones in middle-aged BPH patients,which has some clinical value for BPH prevention.
