Objectives To characterize fine particulate matter(PM_(2.5))-bound polycyclic aromatic hydrocarbons(PAHs)emitted from different cooking fumes and their exposure routes and assess their health-associated impact to prov...Objectives To characterize fine particulate matter(PM_(2.5))-bound polycyclic aromatic hydrocarbons(PAHs)emitted from different cooking fumes and their exposure routes and assess their health-associated impact to provide a reference for health risk prevention from PAH exposure across different age and sex groups.Methods Sixteen PM_(2.5)-bound PAHs emitted from 11 cooking styles were analyzed using GC-MS/MS.The health hazards of these PAHs in the Handan City population(stratified by age and sex)were predicted using the incremental lifetime cancer risk(ILCR)model.The respiratory deposition doses(RDDs)of the PAHs in children and adults were calculated using the PM_(2.5)deposition rates in the upper airway,tracheobronchial,and alveolar regions.Results The total concentrations of PM_(2.5)-bound PAHs ranged from 61.10 to 403.80 ng/m3.Regardless of cooking styles,the ILCRtotal values for adults(1.23×10^(-6)to 3.70×10^(-6))and older adults(1.28×10^(-6)to 3.88×10^(-6))exceeded the acceptable limit of 1.00×10^(-6).With increasing age,the ILCRtotal value first declined and then increased,varying substantially among the population groups.Cancer risk exhibited particularly high sensitivity to short exposure to barbecue-derived PAHs under equivalent body weights.Furthermore,barbecue,Sichuan and Hunan cuisine,Chinese cuisine,and Chinese fast food were associated with higher RDDs for both adults and children.Conclusion ILCRtotal values exceeded the acceptable limit for both females and males of adults,with all cooking styles showing a potentially high cancer risk.Our findings serve as an important reference for refining regulatory strategies related to catering emissions and mitigating health risks associated with cooking styles.展开更多

Chronic obstructive pulmonary disease(COPD)is a complex condition marked by considerable interindividual heterogeneity.Comorbidities exacerbate this variability,worsening disease severity and reducing health-related q...Chronic obstructive pulmonary disease(COPD)is a complex condition marked by considerable interindividual heterogeneity.Comorbidities exacerbate this variability,worsening disease severity and reducing health-related quality of life(HRQoL).Despite the high prevalence of COPD in China,COPD patient clusters remain poorly characterized.This study aimed to identify and validate clusters of Chinese patients with COPD,characterized primarily by comorbidity profiles,using cluster analysis.This cross-sectional,multicenter cohort study used data from the Chinese Enjoying Breathing Program(2020–2023).HRQoL was measured using the EuroQol five dimension(EQ-5D).Dimension reduction was performed via multiple correspondence analysis on 31 variables,including indicators of 27 comorbidities and four sociodemographic or health-related characteristics.Unsupervised machine learning algorithms,K-means++,and hierarchical clustering identified distinct clusters.Robustness was assessed using random forest classification.Logistic regression evaluated the association between cluster membership and EQ-5D outcomes.Among 11145 patients,59.4%had comorbidities.Four clusters emerged:young male smokers,biomass-exposed females,respiratory comorbidity,and elderly multimorbid.The last two clusters had notably lower HRQoL.Cluster analysis identified four clinically meaningful COPD patient clusters based on comorbidities and risk profiles,each with distinct HRQoL outcomes.These findings support targeted public health interventions and integrated care strategies for COPD management.展开更多
Background and objectives:The ongoing mpox outbreaks have garnered significant attention due to their public health implications,particularly the potential mental health impacts.Despite the growing concern,there has b...Background and objectives:The ongoing mpox outbreaks have garnered significant attention due to their public health implications,particularly the potential mental health impacts.Despite the growing concern,there has been limited exploration of the intersection between mpox and mental health within the research literature.This study aims to conduct a comprehensive bibliometric analysis to examine global research trends,regional distribution,and thematic focus areas related to mpox's psychological and psychiatric implications.Methods:We conducted a bibliometric analysis using Scopus and the Web of Science database.The analysis was carried out using the R-bibliometrics package and involved identifying literature on mpox and mental health,focusing on global research trends,regional distribution,and thematic areas of study.The analysis included 416 documents obtained from 295 sources from January 1,2014 to August 27,2024.Results:Our analysis revealed a growing but unevenly distributed literature on mpox and mental health.Most studies concentrated on the relationship between mpox and conditions such as depression and anxiety,while other psychiatric outcomes remain underexplored.The geographic distribution of research was also uneven,with regions like Europe and the Americas receiving more focus than others.Conclusions:The study highlights the need for more targeted research on the mental health sequelae of mpox,particularly for vulnerable populations and regions that are currently underrepresented in the literature.Future research should include longitudinal studies to assess the long-term effects of mpox on mental health and the development of robust methodologies to establish causality.Integrating mental health considerations into public health responses to mpox outbreaks is crucial,with significant implications for research,policy,and clinical practice.展开更多
Objective To examine the mechanistic of organochlorine-associated changes in lung function.Methods This study investigated 76 healthy older adults in Jinan,Shandong Province,over a fivemonth period.Personal exposure t...Objective To examine the mechanistic of organochlorine-associated changes in lung function.Methods This study investigated 76 healthy older adults in Jinan,Shandong Province,over a fivemonth period.Personal exposure to organochlorines was quantified using wearable passive samplers,while inflammatory factors and thyroid hormones were analyzed from blood samples.Participants’lung function was evaluated.After stratifying participants according to their thyroid hormone levels,we analyzed the differential effects of organochlorine exposure on lung function and inflammatory factors across the low and high thyroid hormone groups.Mediation analysis was further conducted to elucidate the relationships among organochlorine exposures,inflammatory factors,and lung function.Results Bis(2-chloro-1-methylethyl)ether(BCIE),was negatively associated with forced vital capacity(FVC,–2.05%,95%CI:–3.11%to–0.97%),and associated with changes in inflammatory factors such as interleukin(IL)-2,IL-7,IL-8,and IL-13 in the low thyroid hormone group.The mediation analysis indicated a mediating effect of IL-2(15.63%,95%CI:0.91%to 44.64%)and IL-13(13.94%,95%CI:0.52%to 41.07%)in the association between BCIE exposure and FVC.Conclusion Lung function and inflammatory factors exhibited an increased sensitivity to organochlorine exposure at lower thyroid hormone levels,with inflammatory factors potentially mediating the adverse effects of organochlorines on lung function.展开更多
Background:Information on the association between physical activity(PA)and the risk of chronic kidney disease(CKD)is limited.We aimed to explore the associations of total,domain-specific,and intensity-specific PA with...Background:Information on the association between physical activity(PA)and the risk of chronic kidney disease(CKD)is limited.We aimed to explore the associations of total,domain-specific,and intensity-specific PA with CKD and its subtypes in China.Methods:The study included 475,376 adults from the China Kadoorie Biobank aged 30-79 years during 2004-2008 at baseline.An interviewer-administered questionnaire was used to collect the information about PA,which was quantified as metabolic equivalent of task hours per day(MET-h/day)and categorized into 4 groups based on quartiles.Cox regression was used to analyze the association between PA and CKD risk.Results:During a median follow-up of 12.1 years,5415 incident CKD cases were documented,including 1159 incident diabetic kidney disease(DKD)cases and 362 incident hypertensive nephropathy(HTN)cases.Total PA was inversely associated with CKD risk,with an adjusted hazard ratio(HR,95%confidence interval(95%CI))of 0.83(0.75-0.92)for incident CKD in the highest quartile of total PA as compared with participants in the lowest quartile.Similar results were observed for risk of DKD and HTN,and the corresponding HRs(95%CIs)were 0.75(0.58-0.97)for DKD risk and 0.56(0.37-0.85)for HTN risk.Increased nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA were significantly associated with a decreased risk of CKD,with HRs(95%CIs)of 0.80(0.73-0.88),0.85(0.77-0.94),and 0.85(0.76-0.95)in the highest quartile,respectively.Conclusion:PA,including nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA,was inversely associated with the risk of CKD,including DKD,HTN,and other CKD,and such associations were dose dependent.展开更多
Objective:Accurate measurement of QT interval,the ventricular action potential from depolarization to repolarization,is important for the early detection of Long QT syndrome.The most effective QT correction(QTc)formul...Objective:Accurate measurement of QT interval,the ventricular action potential from depolarization to repolarization,is important for the early detection of Long QT syndrome.The most effective QT correction(QTc)formula has yet to be determined in the pediatric population,although it has intrinsically greater extremes in heart rate(HR)and is more susceptible to errors in measurement.The authors of this study compare six dif-ferent QTc methods(Bazett,Fridericia,Framingham,Hodges,Rautaharju,and a computer algorithm utilizing the Bazett formula)for consistency against variations in HR and RR interval.Methods:Descriptive Retrospective Study.We included participants from a pediatric cardiology practice of a community hospital who had an ECG performed in 2017.All participants were healthy patients with no past medical history and no regular med-ications.Results:ECGs from 95 participants from one month to 21 years of age(mean 9.7 years)were included with a mean HR of 91 beats per minute(bpm).The two-sample paired t-test or Wilcoxon signed-rank test assessed for any difference between QTc methods.A statistically significant difference was observed between every combination of two QTc formulae.The Spearman’s rank correlation analysis explored the QTc/HR and QTc/RR relationships for each formula.Fridericia method was most independent of HR and RR with the lowest absolute value of correlation coefficients.Bazett and Computer had moderate correlations,while Framingham and Rautaharju exhibited strong correlations.Correlations were positive for Bazett and Computer,reflecting results from prior studies demonstrating an over-correction of Bazett at higher HRs.In the linear QTc/HR regression analysis,Bazett had the slope closest to zero,although Computer,Hodges,and Fridericia had comparable values.Alternatively,Fridericia had the linear QTc/RR regression coefficient closest to zero.The Bland-Altman method assessed for bias and the limits of agreement between correction formulae.Bazett and Computer exhibited good agreement with minimal bias along with Framingham and Rautaharju.To account for a possible skewed distri-bution of QT,all the above analyses were also performed excluding the top and bottom 2%of data as sorted by heart rate ranges(N=90).Results from this data set were consistent with those derived from all participants(N=95).Conclusions:Overall,the Fridericia correction method provided the best rate correction in our pedia-tric study cohort.展开更多
Purpose:It is aimed to assess whether school uniforms are associated with population-level gender inequalities in physical activity,and whether associations differ by school level,country/region income,and assessment ...Purpose:It is aimed to assess whether school uniforms are associated with population-level gender inequalities in physical activity,and whether associations differ by school level,country/region income,and assessment method.Methods:An ecological study design was employed.We collected data about global uniform practices using an online survey.We searched for country/region-level estimates of school-aged youth meeting physical activity guidelines from international surveillance studies.Study selection was conducted in duplicate using a systematic process,and a random sample of all data was checked to ensure extraction and pooling processes were accurate.We calculated absolute and relative gender inequalities in physical activity for each country.Linear regression examined associations between country/region-level uniform practices(binary yes/no exposure variable) and country/region-level gender inequalities in physical activity guideline compliance(absolute and relative inequalities).We investigated moderation by school level,stratified analyses by income group,and repeated primary analyses using device-measured data.Results:Pooling data from 135 countries/regions(n=1,089,852),we found no association between population-level uniform practices and gender inequalities in physical activity across all ages(absolute:β=-0.2;95% confidence interval(95%CI):-1.7 to 1.3,p=0.74;relative:β=0.1;95%CI:-0.1 to 0.2,p=0.51).Subgroup analysis suggested a positive association in primary school settings(absolute:β=4.3;95%CI:-0.0 to 8.6,p=0.05).Among high-income countries,absolute inequalities were significantly greater in countries/regions with uniform practices(N=37) compared to those without(N=48)(9.1(SD=3.6) vs.7.8 percentage points(SD=4.3)).Repeating analyses using device-measured data(n=32,130;N=24) did not alter our primary finding.From initial descriptive statistics,we found that in countries/regions where a maj ority of schools(>50%) reportedly use uniforms,there was lower compliance with physical activity guidelines among all genders(median:16.0%,interquartile range:13.2%-19.9%,N=103) compared to generally non-uniform countries/regions(median:19.5%,interquartile range:16.4%-23.5 %,N=32)(z=3.04,p=0.002).(N=countries,regions and studies repre sented;n=sample size or participants included).Conclusion:School uniforms are associated with greater gender inequalities in physical activity in primary school settings and in high-income countries.Our population-level findings warrant testing using individual-level data across contexts.展开更多
Background Psychiatric comorbidities are common in patients with epilepsy.Reasons for the co-occurrence of psychiatric conditions and epilepsy remain poorly understood.Aim We aimed to triangulate the relationship betw...Background Psychiatric comorbidities are common in patients with epilepsy.Reasons for the co-occurrence of psychiatric conditions and epilepsy remain poorly understood.Aim We aimed to triangulate the relationship between epilepsy and psychiatric conditions to determine the extent and possible origins of these conditions.Methods Using nationwide Swedish health registries,we quantified the lifetime prevalence of psychiatric disorders in patients with epilepsy.We then used summarydata from genome-wide association studies to investigate whether the identified observational associations could be attributed to a shared underlying genetic aetiology using cross-trait linkage disequilibrium score regression.Finally,we assessed the potential bidirectional relationships using two-sample Mendelian randomisation.Results In a cohort of 7628495 individuals,we found that almost half of the 94435 individuals diagnosed with epilepsy were also diagnosed with a psychiatric condition in their lifetime(adjusted lifetime prevalence,44.09%;95%confidence interval(Cl)43.78%to 44.39%).We found evidence for a genetic correlation between epilepsy and some neurodevelopmental and psychiatric conditions.For example,we observed a genetic correlation between epilepsy and attention-deficit/hyperactivity disorder(r,=0.18,95%Cl 0.09 to 0.27,p<0.001)—a correlation that was more pronounced in focal epilepsy(r=0.23,95%CI 0.09 to 0.36,p<0.001).Findings from Mendelian randomisation using common genetic variants did not support bidirectional effects between epilepsy and neurodevelopmental or psychiatric conditions.Conclusions Psychiatric comorbidities are common in patients with epilepsy.Genetic correlations may partially explain some comorbidities;however,there is little evidence of a bidirectional relationship between the genetic liability of epilepsy and psychiatric conditions.These findings highlight the need to understand the role of environmental factors or rare genetic variations in the origins of psychiatric comorbidities in epilepsy.展开更多

Methods: We conducted a cross-sectional study at the Yalgado OUEDRAOGO University Hospital from January 31 to May 31, 2022, among chronic hepatitis B carriers aged at least 18 years who were followed for at least six months, treated or not. Results: The study included 135 patients, with an average age of 41.62 ± 10.69 years and a male predominance (64.44%). The average infection duration was 8.11 years ± 6, and the Chronic Liver Disease Questionnaire - Hepatitis B Virus (CLDQ-HBV) global quality of life score was 5.95 ± 0.90. Respondents’ overall quality of life was linked with gender, history of depression, and history of fatigue (ORa = 0.42 (p = 0.009), ORa = 1.57 (p Conclusion: Our study enabled us to assess mean quality of life scores according to the CLDQ-HBV dimensions, and to identify factors associated with the quality of life of chronic HBV carrier patients. In this scoping review,we evaluated the performance of artificial intelligence(AI)in clinical radiology practice and examined health professionals'perspec-tives regarding AI use in radiology.This review followed t...In this scoping review,we evaluated the performance of artificial intelligence(AI)in clinical radiology practice and examined health professionals'perspec-tives regarding AI use in radiology.This review followed the Joanna Briggs Institute(JBI)methodological guidelines.We searched multiple databases and the gray literature from March 15,2016 to December 31,2023.Of 49 articles reviewed,13 assessed the performance of AI in radiology clinical practice,and 36 examined the attitudes of health professionals toward the use of AI in radi-ology.In four separate studies,AI significantly improved the diagnostic sensi-tivity or detection rate.Furthermore,six articles emphasized a significant reduction in case reading times with AI use.Although three studies suggested an increase in specificity with the assistance of AI,these findings did not reach statistical significance.Health professionals expressed the belief that AI would have a significant impact on radiology but would not replace radiologists in the near future.Limited knowledge of AI was observed among health professionals,Abbreviations:AI,artificial intelligence;NPV,negative predictive value;PPV,positive predictive value.Chanchan He and Weiqi Liu are co-first authors.who supported increased education and explicit regulations and guidelines related to AI.Overall,AI can enhance diagnostic efficiency and accuracy in clinical radiology practice.However,knowledge gaps and the concerns of health professionals should be addressed by prioritizing education and reinforcing ethical and legal regulations to facilitate the advancement of AI use in radiology.This scoping review provides evidence toward a comprehensive understanding of AI's potential in clinical radiology practice,promoting its use and stimulating further discussion on related challenges and implications.展开更多
Background:Muscular strength is a powerful marker of current health status and robust predictor of age-related disease and disability.Handgrip strength(HGS)using isometric dynamometry is a convenient,feasible,and wide...Background:Muscular strength is a powerful marker of current health status and robust predictor of age-related disease and disability.Handgrip strength(HGS)using isometric dynamometry is a convenient,feasible,and widely used method of assessing muscular strength among people of all ages.While adult HGS norms have been published for many countries,no study has yet synthesized available data to produce international norms.The objective of this study was to generate international sex-and age-specific norms for absolute and body size-normalized HGS across the adult lifespan.Methods:Systematic searches were conducted in 6 databases/web search engines(MEDLINE,SPORTDiscus,Embase,Web of Science,CINAHL,and Google Scholar)up to December 1,2023.We included full-text peer-reviewed observational studies that reported normative HGS data for adults aged ≥20 years by sex and age.Pseudo data were generated using Monte Carlo simulation following harmonization for methodo-logical variation.Population-weighted Generalized Additive Models for Location,Scale,and Shape were used to develop sex-and age-specific norms for absolute HGS(kg)and HGS normalized by height(Ht,m)squared(i.e.,HGS/Ht^(2)in kg/m^(2)).Norms were tabulated as percentile values(5th to 95th)and visualized as smoothed percentile curves.Results:We included data from 100 unique observational studies representing 2,405,863 adults(51.9%female)aged 20 to 100+years from 69 countries and regions tested from the year 2000 onward.On average,absolute and normalized HGS values negligibly improved throughout early adulthood,peaked from age 3039 years(at 49.7 kg(males)and 29.7 kg(females)for absolute HGS or 16.3 kg/m^(2)(males)and 11.3 kg/m^(2)(females)for HGS/Ht^(2)),and declined afterwards.The age-related decline in HGS accelerated from middle to late adulthood and was slightly larger for males than for females during middle adulthood.Conclusion:This study provides the world’s largest and most geographically comprehensive international norms for adult HGS by sex and age.These norms have utility for global peer-comparisons,health screening,and surveillance.展开更多

Objective To investigate the association between birth weight and dementia risk and the mediating roles of chronic diseases,and to assess potential biological pathways underlying the birth weight-associated dementia r...Objective To investigate the association between birth weight and dementia risk and the mediating roles of chronic diseases,and to assess potential biological pathways underlying the birth weight-associated dementia risk based on large-scale proteomics.Methods We used data from 279743 participants aged 40 to 69 years enrolled in the UK Biobank.Birth weight was categorized into low birth weight(≤2500 g),normal birth weight(2500-3999 g),and macrosomia(≥4000 g).Multivariable Cox proportional hazards regression models were used to assess the associations between birth weight categories and all-cause dementia and its subtypes(Alzheimer's disease and vascular dementia).Proteomics analyses were conducted to identify proteins and the potential pathways involved.Results Low birth weight was associated with higher risks for all-cause dementia and its subtypes.The hazard ratios were 1.18(95%CI,1.08-1.30)for all-cause dementia,1.14(95%CI,1.00-1.31)for Alzheimer's disease,and 1.22(95%CI,1.01-1.48)for vascular dementia.A non-linear relationship was observed between birth weight and dementia risk(P for nonlinearity<0.001).Certain cardiometabolic diseases in middle-aged adults,such as diabetes,stroke,hypertension,and dyslipidemia,played a significant mediating role in the relationship between low birth weight and dementia risk,with the mediation proportion being 6.3%to 15.8%.Proteomic analyses identified 21 proteins linked to both low birth weight and all-cause dementia risk,which were significantly enriched in the pathways for viral protein interaction with cytokines and cytokine receptors,adipocytokine signaling,and cytokine-cytokine receptor interaction.Conclusion Low birth weight is positively associated with dementia risk.Cardiometabolic diseases in middle-aged adults may mediate the relationship between low birth weight and dementia risk.A number of proteins and the associated pathways underscore the relationship between low birth weight and dementia risk.展开更多

【Objective】Medical imaging data has great value,but it contains a significant amount of sensitive information about patients.At present,laws and regulations regarding to the de-identification of medical imaging data...【Objective】Medical imaging data has great value,but it contains a significant amount of sensitive information about patients.At present,laws and regulations regarding to the de-identification of medical imaging data are not clearly defined around the world.This study aims to develop a tool that meets compliance-driven desensitization requirements tailored to diverse research needs.【Methods】To enhance the security of medical image data,we designed and implemented a DICOM format medical image de-identification system on the Windows operating system.【Results】Our custom de-identification system is adaptable to the legal standards of different countries and can accommodate specific research demands.The system offers both web-based online and desktop offline de-identification capabilities,enabling customization of de-identification rules and facilitating batch processing to improve efficiency.【Conclusions】This medical image de-identification system robustly strengthens the stewardship of sensitive medical data,aligning with data security protection requirements while facilitating the sharing and utilization of medical image data.This approach unlocks the intrinsic value inherent in such datasets.展开更多

Personal care products(PCPs)are a class of emerging pollutants that have attracted public concern owing to their harmful effects on humans and the environment.Biomonitoring data is valuable for insight the levels of P...Personal care products(PCPs)are a class of emerging pollutants that have attracted public concern owing to their harmful effects on humans and the environment.Biomonitoring data is valuable for insight the levels of PCPs in the human body and can be crucial for identifying potential health hazards.To gain a better understanding of timely exposure profiles and health risk of reproductive-age population to PCPs,we determined six parabens,six benzophenone-type ultraviolet filters,and three disinfectants in 256 urine samples collected from young adults aged 18-44 years in Beijing,China.The urinary levels of benzophenone-3(BP-3)and 4-hydroxybenzophenone(4-OHBP)were significantly higher in summer compared to winter,suggesting these compounds have different seasonal usage patterns.Moreover,the total concentration of 15 PCPs in female was 430 ng/mL,approximately two times higher than that in male.P-chloro-m-xylenol(PCMX),as a new type of antibacterial agent,has the greatest level among all target analytes,indicating the increasingly use of this antibacterial alternative recently.Five potential influencing factors that lead to the elevated exposure level of PCPs were identified.Over 19%of the target population had a high hazard index value(greater than 1)which was attributed to exposure to propyl paraben(PrP),benzophenone-1(BP-1),BP-3 and PCMX,indicating that PCPs may pose a relatively high exposure risk at environmental levels that should be a cause for concern.展开更多

文摘Objectives To characterize fine particulate matter(PM_(2.5))-bound polycyclic aromatic hydrocarbons(PAHs)emitted from different cooking fumes and their exposure routes and assess their health-associated impact to provide a reference for health risk prevention from PAH exposure across different age and sex groups.Methods Sixteen PM_(2.5)-bound PAHs emitted from 11 cooking styles were analyzed using GC-MS/MS.The health hazards of these PAHs in the Handan City population(stratified by age and sex)were predicted using the incremental lifetime cancer risk(ILCR)model.The respiratory deposition doses(RDDs)of the PAHs in children and adults were calculated using the PM_(2.5)deposition rates in the upper airway,tracheobronchial,and alveolar regions.Results The total concentrations of PM_(2.5)-bound PAHs ranged from 61.10 to 403.80 ng/m3.Regardless of cooking styles,the ILCRtotal values for adults(1.23×10^(-6)to 3.70×10^(-6))and older adults(1.28×10^(-6)to 3.88×10^(-6))exceeded the acceptable limit of 1.00×10^(-6).With increasing age,the ILCRtotal value first declined and then increased,varying substantially among the population groups.Cancer risk exhibited particularly high sensitivity to short exposure to barbecue-derived PAHs under equivalent body weights.Furthermore,barbecue,Sichuan and Hunan cuisine,Chinese cuisine,and Chinese fast food were associated with higher RDDs for both adults and children.Conclusion ILCRtotal values exceeded the acceptable limit for both females and males of adults,with all cooking styles showing a potentially high cancer risk.Our findings serve as an important reference for refining regulatory strategies related to catering emissions and mitigating health risks associated with cooking styles.

文摘Chronic obstructive pulmonary disease(COPD)is a complex condition marked by considerable interindividual heterogeneity.Comorbidities exacerbate this variability,worsening disease severity and reducing health-related quality of life(HRQoL).Despite the high prevalence of COPD in China,COPD patient clusters remain poorly characterized.This study aimed to identify and validate clusters of Chinese patients with COPD,characterized primarily by comorbidity profiles,using cluster analysis.This cross-sectional,multicenter cohort study used data from the Chinese Enjoying Breathing Program(2020–2023).HRQoL was measured using the EuroQol five dimension(EQ-5D).Dimension reduction was performed via multiple correspondence analysis on 31 variables,including indicators of 27 comorbidities and four sociodemographic or health-related characteristics.Unsupervised machine learning algorithms,K-means++,and hierarchical clustering identified distinct clusters.Robustness was assessed using random forest classification.Logistic regression evaluated the association between cluster membership and EQ-5D outcomes.Among 11145 patients,59.4%had comorbidities.Four clusters emerged:young male smokers,biomass-exposed females,respiratory comorbidity,and elderly multimorbid.The last two clusters had notably lower HRQoL.Cluster analysis identified four clinically meaningful COPD patient clusters based on comorbidities and risk profiles,each with distinct HRQoL outcomes.These findings support targeted public health interventions and integrated care strategies for COPD management.
文摘Background and objectives:The ongoing mpox outbreaks have garnered significant attention due to their public health implications,particularly the potential mental health impacts.Despite the growing concern,there has been limited exploration of the intersection between mpox and mental health within the research literature.This study aims to conduct a comprehensive bibliometric analysis to examine global research trends,regional distribution,and thematic focus areas related to mpox's psychological and psychiatric implications.Methods:We conducted a bibliometric analysis using Scopus and the Web of Science database.The analysis was carried out using the R-bibliometrics package and involved identifying literature on mpox and mental health,focusing on global research trends,regional distribution,and thematic areas of study.The analysis included 416 documents obtained from 295 sources from January 1,2014 to August 27,2024.Results:Our analysis revealed a growing but unevenly distributed literature on mpox and mental health.Most studies concentrated on the relationship between mpox and conditions such as depression and anxiety,while other psychiatric outcomes remain underexplored.The geographic distribution of research was also uneven,with regions like Europe and the Americas receiving more focus than others.Conclusions:The study highlights the need for more targeted research on the mental health sequelae of mpox,particularly for vulnerable populations and regions that are currently underrepresented in the literature.Future research should include longitudinal studies to assess the long-term effects of mpox on mental health and the development of robust methodologies to establish causality.Integrating mental health considerations into public health responses to mpox outbreaks is crucial,with significant implications for research,policy,and clinical practice.

文摘Objective To examine the mechanistic of organochlorine-associated changes in lung function.Methods This study investigated 76 healthy older adults in Jinan,Shandong Province,over a fivemonth period.Personal exposure to organochlorines was quantified using wearable passive samplers,while inflammatory factors and thyroid hormones were analyzed from blood samples.Participants’lung function was evaluated.After stratifying participants according to their thyroid hormone levels,we analyzed the differential effects of organochlorine exposure on lung function and inflammatory factors across the low and high thyroid hormone groups.Mediation analysis was further conducted to elucidate the relationships among organochlorine exposures,inflammatory factors,and lung function.Results Bis(2-chloro-1-methylethyl)ether(BCIE),was negatively associated with forced vital capacity(FVC,–2.05%,95%CI:–3.11%to–0.97%),and associated with changes in inflammatory factors such as interleukin(IL)-2,IL-7,IL-8,and IL-13 in the low thyroid hormone group.The mediation analysis indicated a mediating effect of IL-2(15.63%,95%CI:0.91%to 44.64%)and IL-13(13.94%,95%CI:0.52%to 41.07%)in the association between BCIE exposure and FVC.Conclusion Lung function and inflammatory factors exhibited an increased sensitivity to organochlorine exposure at lower thyroid hormone levels,with inflammatory factors potentially mediating the adverse effects of organochlorines on lung function.

文摘Background:Information on the association between physical activity(PA)and the risk of chronic kidney disease(CKD)is limited.We aimed to explore the associations of total,domain-specific,and intensity-specific PA with CKD and its subtypes in China.Methods:The study included 475,376 adults from the China Kadoorie Biobank aged 30-79 years during 2004-2008 at baseline.An interviewer-administered questionnaire was used to collect the information about PA,which was quantified as metabolic equivalent of task hours per day(MET-h/day)and categorized into 4 groups based on quartiles.Cox regression was used to analyze the association between PA and CKD risk.Results:During a median follow-up of 12.1 years,5415 incident CKD cases were documented,including 1159 incident diabetic kidney disease(DKD)cases and 362 incident hypertensive nephropathy(HTN)cases.Total PA was inversely associated with CKD risk,with an adjusted hazard ratio(HR,95%confidence interval(95%CI))of 0.83(0.75-0.92)for incident CKD in the highest quartile of total PA as compared with participants in the lowest quartile.Similar results were observed for risk of DKD and HTN,and the corresponding HRs(95%CIs)were 0.75(0.58-0.97)for DKD risk and 0.56(0.37-0.85)for HTN risk.Increased nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA were significantly associated with a decreased risk of CKD,with HRs(95%CIs)of 0.80(0.73-0.88),0.85(0.77-0.94),and 0.85(0.76-0.95)in the highest quartile,respectively.Conclusion:PA,including nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA,was inversely associated with the risk of CKD,including DKD,HTN,and other CKD,and such associations were dose dependent.

文摘Objective:Accurate measurement of QT interval,the ventricular action potential from depolarization to repolarization,is important for the early detection of Long QT syndrome.The most effective QT correction(QTc)formula has yet to be determined in the pediatric population,although it has intrinsically greater extremes in heart rate(HR)and is more susceptible to errors in measurement.The authors of this study compare six dif-ferent QTc methods(Bazett,Fridericia,Framingham,Hodges,Rautaharju,and a computer algorithm utilizing the Bazett formula)for consistency against variations in HR and RR interval.Methods:Descriptive Retrospective Study.We included participants from a pediatric cardiology practice of a community hospital who had an ECG performed in 2017.All participants were healthy patients with no past medical history and no regular med-ications.Results:ECGs from 95 participants from one month to 21 years of age(mean 9.7 years)were included with a mean HR of 91 beats per minute(bpm).The two-sample paired t-test or Wilcoxon signed-rank test assessed for any difference between QTc methods.A statistically significant difference was observed between every combination of two QTc formulae.The Spearman’s rank correlation analysis explored the QTc/HR and QTc/RR relationships for each formula.Fridericia method was most independent of HR and RR with the lowest absolute value of correlation coefficients.Bazett and Computer had moderate correlations,while Framingham and Rautaharju exhibited strong correlations.Correlations were positive for Bazett and Computer,reflecting results from prior studies demonstrating an over-correction of Bazett at higher HRs.In the linear QTc/HR regression analysis,Bazett had the slope closest to zero,although Computer,Hodges,and Fridericia had comparable values.Alternatively,Fridericia had the linear QTc/RR regression coefficient closest to zero.The Bland-Altman method assessed for bias and the limits of agreement between correction formulae.Bazett and Computer exhibited good agreement with minimal bias along with Framingham and Rautaharju.To account for a possible skewed distri-bution of QT,all the above analyses were also performed excluding the top and bottom 2%of data as sorted by heart rate ranges(N=90).Results from this data set were consistent with those derived from all participants(N=95).Conclusions:Overall,the Fridericia correction method provided the best rate correction in our pedia-tric study cohort.

文摘Purpose:It is aimed to assess whether school uniforms are associated with population-level gender inequalities in physical activity,and whether associations differ by school level,country/region income,and assessment method.Methods:An ecological study design was employed.We collected data about global uniform practices using an online survey.We searched for country/region-level estimates of school-aged youth meeting physical activity guidelines from international surveillance studies.Study selection was conducted in duplicate using a systematic process,and a random sample of all data was checked to ensure extraction and pooling processes were accurate.We calculated absolute and relative gender inequalities in physical activity for each country.Linear regression examined associations between country/region-level uniform practices(binary yes/no exposure variable) and country/region-level gender inequalities in physical activity guideline compliance(absolute and relative inequalities).We investigated moderation by school level,stratified analyses by income group,and repeated primary analyses using device-measured data.Results:Pooling data from 135 countries/regions(n=1,089,852),we found no association between population-level uniform practices and gender inequalities in physical activity across all ages(absolute:β=-0.2;95% confidence interval(95%CI):-1.7 to 1.3,p=0.74;relative:β=0.1;95%CI:-0.1 to 0.2,p=0.51).Subgroup analysis suggested a positive association in primary school settings(absolute:β=4.3;95%CI:-0.0 to 8.6,p=0.05).Among high-income countries,absolute inequalities were significantly greater in countries/regions with uniform practices(N=37) compared to those without(N=48)(9.1(SD=3.6) vs.7.8 percentage points(SD=4.3)).Repeating analyses using device-measured data(n=32,130;N=24) did not alter our primary finding.From initial descriptive statistics,we found that in countries/regions where a maj ority of schools(>50%) reportedly use uniforms,there was lower compliance with physical activity guidelines among all genders(median:16.0%,interquartile range:13.2%-19.9%,N=103) compared to generally non-uniform countries/regions(median:19.5%,interquartile range:16.4%-23.5 %,N=32)(z=3.04,p=0.002).(N=countries,regions and studies repre sented;n=sample size or participants included).Conclusion:School uniforms are associated with greater gender inequalities in physical activity in primary school settings and in high-income countries.Our population-level findings warrant testing using individual-level data across contexts.

文摘Background Psychiatric comorbidities are common in patients with epilepsy.Reasons for the co-occurrence of psychiatric conditions and epilepsy remain poorly understood.Aim We aimed to triangulate the relationship between epilepsy and psychiatric conditions to determine the extent and possible origins of these conditions.Methods Using nationwide Swedish health registries,we quantified the lifetime prevalence of psychiatric disorders in patients with epilepsy.We then used summarydata from genome-wide association studies to investigate whether the identified observational associations could be attributed to a shared underlying genetic aetiology using cross-trait linkage disequilibrium score regression.Finally,we assessed the potential bidirectional relationships using two-sample Mendelian randomisation.Results In a cohort of 7628495 individuals,we found that almost half of the 94435 individuals diagnosed with epilepsy were also diagnosed with a psychiatric condition in their lifetime(adjusted lifetime prevalence,44.09%;95%confidence interval(Cl)43.78%to 44.39%).We found evidence for a genetic correlation between epilepsy and some neurodevelopmental and psychiatric conditions.For example,we observed a genetic correlation between epilepsy and attention-deficit/hyperactivity disorder(r,=0.18,95%Cl 0.09 to 0.27,p<0.001)—a correlation that was more pronounced in focal epilepsy(r=0.23,95%CI 0.09 to 0.36,p<0.001).Findings from Mendelian randomisation using common genetic variants did not support bidirectional effects between epilepsy and neurodevelopmental or psychiatric conditions.Conclusions Psychiatric comorbidities are common in patients with epilepsy.Genetic correlations may partially explain some comorbidities;however,there is little evidence of a bidirectional relationship between the genetic liability of epilepsy and psychiatric conditions.These findings highlight the need to understand the role of environmental factors or rare genetic variations in the origins of psychiatric comorbidities in epilepsy.
文摘Introduction: According to the World Health Organization, chronic hepatitis B is the leading cause of mortality from liver disease and constitutes a public health problem. The aim of this study was to assess the quality of life of chronic hepatitis B virus carriers at the Yalgado OUEDRAOGO University hospital, and to identify the factors associated with this quality of life. Methods: We conducted a cross-sectional study at the Yalgado OUEDRAOGO University Hospital from January 31 to May 31, 2022, among chronic hepatitis B carriers aged at least 18 years who were followed for at least six months, treated or not. Results: The study included 135 patients, with an average age of 41.62 ± 10.69 years and a male predominance (64.44%). The average infection duration was 8.11 years ± 6, and the Chronic Liver Disease Questionnaire - Hepatitis B Virus (CLDQ-HBV) global quality of life score was 5.95 ± 0.90. Respondents’ overall quality of life was linked with gender, history of depression, and history of fatigue (ORa = 0.42 (p = 0.009), ORa = 1.57 (p Conclusion: Our study enabled us to assess mean quality of life scores according to the CLDQ-HBV dimensions, and to identify factors associated with the quality of life of chronic HBV carrier patients. Public health measures such as early diagnosis and management, and free care related to this infection would improve the quality of life of these patients.
文摘In this scoping review,we evaluated the performance of artificial intelligence(AI)in clinical radiology practice and examined health professionals'perspec-tives regarding AI use in radiology.This review followed the Joanna Briggs Institute(JBI)methodological guidelines.We searched multiple databases and the gray literature from March 15,2016 to December 31,2023.Of 49 articles reviewed,13 assessed the performance of AI in radiology clinical practice,and 36 examined the attitudes of health professionals toward the use of AI in radi-ology.In four separate studies,AI significantly improved the diagnostic sensi-tivity or detection rate.Furthermore,six articles emphasized a significant reduction in case reading times with AI use.Although three studies suggested an increase in specificity with the assistance of AI,these findings did not reach statistical significance.Health professionals expressed the belief that AI would have a significant impact on radiology but would not replace radiologists in the near future.Limited knowledge of AI was observed among health professionals,Abbreviations:AI,artificial intelligence;NPV,negative predictive value;PPV,positive predictive value.Chanchan He and Weiqi Liu are co-first authors.who supported increased education and explicit regulations and guidelines related to AI.Overall,AI can enhance diagnostic efficiency and accuracy in clinical radiology practice.However,knowledge gaps and the concerns of health professionals should be addressed by prioritizing education and reinforcing ethical and legal regulations to facilitate the advancement of AI use in radiology.This scoping review provides evidence toward a comprehensive understanding of AI's potential in clinical radiology practice,promoting its use and stimulating further discussion on related challenges and implications.

文摘Background:Muscular strength is a powerful marker of current health status and robust predictor of age-related disease and disability.Handgrip strength(HGS)using isometric dynamometry is a convenient,feasible,and widely used method of assessing muscular strength among people of all ages.While adult HGS norms have been published for many countries,no study has yet synthesized available data to produce international norms.The objective of this study was to generate international sex-and age-specific norms for absolute and body size-normalized HGS across the adult lifespan.Methods:Systematic searches were conducted in 6 databases/web search engines(MEDLINE,SPORTDiscus,Embase,Web of Science,CINAHL,and Google Scholar)up to December 1,2023.We included full-text peer-reviewed observational studies that reported normative HGS data for adults aged ≥20 years by sex and age.Pseudo data were generated using Monte Carlo simulation following harmonization for methodo-logical variation.Population-weighted Generalized Additive Models for Location,Scale,and Shape were used to develop sex-and age-specific norms for absolute HGS(kg)and HGS normalized by height(Ht,m)squared(i.e.,HGS/Ht^(2)in kg/m^(2)).Norms were tabulated as percentile values(5th to 95th)and visualized as smoothed percentile curves.Results:We included data from 100 unique observational studies representing 2,405,863 adults(51.9%female)aged 20 to 100+years from 69 countries and regions tested from the year 2000 onward.On average,absolute and normalized HGS values negligibly improved throughout early adulthood,peaked from age 3039 years(at 49.7 kg(males)and 29.7 kg(females)for absolute HGS or 16.3 kg/m^(2)(males)and 11.3 kg/m^(2)(females)for HGS/Ht^(2)),and declined afterwards.The age-related decline in HGS accelerated from middle to late adulthood and was slightly larger for males than for females during middle adulthood.Conclusion:This study provides the world’s largest and most geographically comprehensive international norms for adult HGS by sex and age.These norms have utility for global peer-comparisons,health screening,and surveillance.
文摘Type 2 diabetes mellitus(T2DM)significantly elevates the risk of colorectal cancer(CRC)and complicates its treatment by promoting chemoresistance.Poor glycemic control has been linked to exacerbated CRC progression and diminished chemotherapy efficacy,impacting patient outcomes through various mechanisms such as oxidative stress,activation of metabolic pathways,and altered protein modifications that hinder apoptosis and enhance tumor survival.Clinical evidence shows that T2DM patients experience higher rates of chemoresistance and reduced disease-free survival and overall survival compared to non-diabetic patients.Specifically,those with poor glycemic control exhibit increased chemo-resistance and poorer survival metrics.Antidiabetic treatments,including metformin,acarbose,and gliclazide,show promise in improving chemotherapy response and glycemic management,potentially enhancing patient outcomes.Addressing this challenge requires a comprehensive,multidisciplinary approach involving oncologists,endocrino-logists,and surgeons to optimize patient care.Integrated strategies that prioritize glycemic control are essential for reducing chemoresistance and improving survival in CRC patients with T2DM.
文摘Objective To investigate the association between birth weight and dementia risk and the mediating roles of chronic diseases,and to assess potential biological pathways underlying the birth weight-associated dementia risk based on large-scale proteomics.Methods We used data from 279743 participants aged 40 to 69 years enrolled in the UK Biobank.Birth weight was categorized into low birth weight(≤2500 g),normal birth weight(2500-3999 g),and macrosomia(≥4000 g).Multivariable Cox proportional hazards regression models were used to assess the associations between birth weight categories and all-cause dementia and its subtypes(Alzheimer's disease and vascular dementia).Proteomics analyses were conducted to identify proteins and the potential pathways involved.Results Low birth weight was associated with higher risks for all-cause dementia and its subtypes.The hazard ratios were 1.18(95%CI,1.08-1.30)for all-cause dementia,1.14(95%CI,1.00-1.31)for Alzheimer's disease,and 1.22(95%CI,1.01-1.48)for vascular dementia.A non-linear relationship was observed between birth weight and dementia risk(P for nonlinearity<0.001).Certain cardiometabolic diseases in middle-aged adults,such as diabetes,stroke,hypertension,and dyslipidemia,played a significant mediating role in the relationship between low birth weight and dementia risk,with the mediation proportion being 6.3%to 15.8%.Proteomic analyses identified 21 proteins linked to both low birth weight and all-cause dementia risk,which were significantly enriched in the pathways for viral protein interaction with cytokines and cytokine receptors,adipocytokine signaling,and cytokine-cytokine receptor interaction.Conclusion Low birth weight is positively associated with dementia risk.Cardiometabolic diseases in middle-aged adults may mediate the relationship between low birth weight and dementia risk.A number of proteins and the associated pathways underscore the relationship between low birth weight and dementia risk.

文摘Alzheimer’s disease(AD)is the most common form of dementia characterized pathologically by the deposition of amyloid plaques and hyperphosphorylated tau containing neurofibrillary tangles.The disease presents clinically with progressive memory loss and disruption of cognitive function.Currently,there is no cure for AD;recent advances in the therapeutics aimed at clearing the amyloid protein from the brain have led to potential disease stabilization,however,this does not prevent eventual disease progression(Cummings et al.,2024).

文摘【Objective】Medical imaging data has great value,but it contains a significant amount of sensitive information about patients.At present,laws and regulations regarding to the de-identification of medical imaging data are not clearly defined around the world.This study aims to develop a tool that meets compliance-driven desensitization requirements tailored to diverse research needs.【Methods】To enhance the security of medical image data,we designed and implemented a DICOM format medical image de-identification system on the Windows operating system.【Results】Our custom de-identification system is adaptable to the legal standards of different countries and can accommodate specific research demands.The system offers both web-based online and desktop offline de-identification capabilities,enabling customization of de-identification rules and facilitating batch processing to improve efficiency.【Conclusions】This medical image de-identification system robustly strengthens the stewardship of sensitive medical data,aligning with data security protection requirements while facilitating the sharing and utilization of medical image data.This approach unlocks the intrinsic value inherent in such datasets.
文摘In the words of the late Sir Colin Blakemore,neurologists have historically sought to infer brain functions in a manner akin to to king a hammer to a computeranalyzing localized anatomical lesions caused by trauma,tumors,or strokes,noting deficits,and inferring what functions certain brain regions may be responsible for.This approach exemplifies a deletion heuristic,where the absence of a specific function reveals insights about the underlying structures or mechanisms responsible for it.By observing what is lost when a particular brain region is damaged,throughout the history of the field,neurologists have pieced together the intricate relationship between anatomy and function.

文摘Personal care products(PCPs)are a class of emerging pollutants that have attracted public concern owing to their harmful effects on humans and the environment.Biomonitoring data is valuable for insight the levels of PCPs in the human body and can be crucial for identifying potential health hazards.To gain a better understanding of timely exposure profiles and health risk of reproductive-age population to PCPs,we determined six parabens,six benzophenone-type ultraviolet filters,and three disinfectants in 256 urine samples collected from young adults aged 18-44 years in Beijing,China.The urinary levels of benzophenone-3(BP-3)and 4-hydroxybenzophenone(4-OHBP)were significantly higher in summer compared to winter,suggesting these compounds have different seasonal usage patterns.Moreover,the total concentration of 15 PCPs in female was 430 ng/mL,approximately two times higher than that in male.P-chloro-m-xylenol(PCMX),as a new type of antibacterial agent,has the greatest level among all target analytes,indicating the increasingly use of this antibacterial alternative recently.Five potential influencing factors that lead to the elevated exposure level of PCPs were identified.Over 19%of the target population had a high hazard index value(greater than 1)which was attributed to exposure to propyl paraben(PrP),benzophenone-1(BP-1),BP-3 and PCMX,indicating that PCPs may pose a relatively high exposure risk at environmental levels that should be a cause for concern.
