Objectives To characterize fine particulate matter(PM_(2.5))-bound polycyclic aromatic hydrocarbons(PAHs)emitted from different cooking fumes and their exposure routes and assess their health-associated impact to prov...Objectives To characterize fine particulate matter(PM_(2.5))-bound polycyclic aromatic hydrocarbons(PAHs)emitted from different cooking fumes and their exposure routes and assess their health-associated impact to provide a reference for health risk prevention from PAH exposure across different age and sex groups.Methods Sixteen PM_(2.5)-bound PAHs emitted from 11 cooking styles were analyzed using GC-MS/MS.The health hazards of these PAHs in the Handan City population(stratified by age and sex)were predicted using the incremental lifetime cancer risk(ILCR)model.The respiratory deposition doses(RDDs)of the PAHs in children and adults were calculated using the PM_(2.5)deposition rates in the upper airway,tracheobronchial,and alveolar regions.Results The total concentrations of PM_(2.5)-bound PAHs ranged from 61.10 to 403.80 ng/m3.Regardless of cooking styles,the ILCRtotal values for adults(1.23×10^(-6)to 3.70×10^(-6))and older adults(1.28×10^(-6)to 3.88×10^(-6))exceeded the acceptable limit of 1.00×10^(-6).With increasing age,the ILCRtotal value first declined and then increased,varying substantially among the population groups.Cancer risk exhibited particularly high sensitivity to short exposure to barbecue-derived PAHs under equivalent body weights.Furthermore,barbecue,Sichuan and Hunan cuisine,Chinese cuisine,and Chinese fast food were associated with higher RDDs for both adults and children.Conclusion ILCRtotal values exceeded the acceptable limit for both females and males of adults,with all cooking styles showing a potentially high cancer risk.Our findings serve as an important reference for refining regulatory strategies related to catering emissions and mitigating health risks associated with cooking styles.展开更多

BACKGROUND The neurological and psychiatric sequelae of coronavirus disease 2019(COVID-19)have been documented,yet further data are needed to thoroughly evaluate the impact of COVID-19 on brain health years after the ...BACKGROUND The neurological and psychiatric sequelae of coronavirus disease 2019(COVID-19)have been documented,yet further data are needed to thoroughly evaluate the impact of COVID-19 on brain health years after the infection.AIM To examine whether COVID-19 infection is associated with exacerbation,recurrence,or progression of pre-existing neurological or psychiatric disorders-a highrisk population that is underrepresented in COVID-19 outcome research in National Guard Health Affair patients in 4-years following a COVID-19 diagnosis.METHODS For this multicenter retrospective cohort study,we used data from the National Guard hospitals electronic health records network(BestCare)with over 10 million patients.Our cohort comprised patients who had a COVID-19 diagnosis;a matched control cohort included patient did not expose to COVID-19 in same period.Age and sex were matching factors.We estimated the incidence of 14 neurological and psychiatric outcomes in nearly 4 years after a confirmed diagnosis of COVID-19.Using a multiple logistic regression,we compared incidences with those in propensity score-matched cohorts of patients with no exposure to COVID-19.Ramadan M et al.Neurological and psychiatric risks post-COVID-19 RESULTS Our primary cohort comprised 4437 patients diagnosed with COVID-19,and our propensity-score 1:1 matched control cohort comprised 4437 individuals.Nearly two-third of the COVID-19 cohort(71%)were diagnosed in 2020,and 2021.The most prevalent diagnoses for both cohorts were epilepsy 30.68%,mood disorder 23.92%,and nerve plexus disorder 22.13%.Dementia was nearly 4 times higher among COVID-19 cohort(8.27%)compared to the control cohort(2.57%).Five neurological and psychiatric outcomes had odds ratios(OR)that were significantly higher than 1 for people who had COVID-19 compared to people who had never been infected.These outcomes were cognitive deficit OR=1.54,95%confidence interval(CI):1.23-1.91,P=0.0001;nerve plexus disorder OR=1.13,95%CI:1.01-1.25,P=0.02;substance use disorder OR=1.95,95%CI:1.12-3.38,P=0.01;mood disorder OR=1.16,95%CI:1.05-1.29,P=0.003;and anxiety disorder OR=1.39,95%CI:1.07-1.79,P=0.01.CONCLUSION The study highlights the persistent risk of neurological and psychiatric conditions in COVID-19 survivors up to four years post-infection.Although the incidence was lower than in previous large studies,long-term consequences remain significant,emphasizing the need for ongoing monitoring and support in mental health and neurological care.展开更多

Chronic obstructive pulmonary disease(COPD)is a complex condition marked by considerable interindividual heterogeneity.Comorbidities exacerbate this variability,worsening disease severity and reducing health-related q...Chronic obstructive pulmonary disease(COPD)is a complex condition marked by considerable interindividual heterogeneity.Comorbidities exacerbate this variability,worsening disease severity and reducing health-related quality of life(HRQoL).Despite the high prevalence of COPD in China,COPD patient clusters remain poorly characterized.This study aimed to identify and validate clusters of Chinese patients with COPD,characterized primarily by comorbidity profiles,using cluster analysis.This cross-sectional,multicenter cohort study used data from the Chinese Enjoying Breathing Program(2020–2023).HRQoL was measured using the EuroQol five dimension(EQ-5D).Dimension reduction was performed via multiple correspondence analysis on 31 variables,including indicators of 27 comorbidities and four sociodemographic or health-related characteristics.Unsupervised machine learning algorithms,K-means++,and hierarchical clustering identified distinct clusters.Robustness was assessed using random forest classification.Logistic regression evaluated the association between cluster membership and EQ-5D outcomes.Among 11145 patients,59.4%had comorbidities.Four clusters emerged:young male smokers,biomass-exposed females,respiratory comorbidity,and elderly multimorbid.The last two clusters had notably lower HRQoL.Cluster analysis identified four clinically meaningful COPD patient clusters based on comorbidities and risk profiles,each with distinct HRQoL outcomes.These findings support targeted public health interventions and integrated care strategies for COPD management.展开更多
Chemical exposure during prenatal development has significant implications for both maternal and child health.Compared to blood,saliva is a non-invasive and less resource-intensive,alternative.Given the temporal varia...Chemical exposure during prenatal development has significant implications for both maternal and child health.Compared to blood,saliva is a non-invasive and less resource-intensive,alternative.Given the temporal variability of xenobiotic metabolites(XM),repeated sampling is essential.Therefore,saliva offers a valuable tool for the longitudinal assessment of prenatal exposomes.Despite its potential,no studies have explored saliva for XM measurement.This study pioneered using saliva to assess XM detectability and investigate the associations between prenatal XM and endogenous metabolomes in pregnant women.Saliva samples were analysed using mass spectrometry from 80 pregnant women at 24–34 weeks gestation.Metabolomes and exposomes were annotated using the Human Metabolome and U.S.Environmental Protection Agency databases.Metabolome-XM associations were clustered using Glay community clustering.Linear regression models,adjusted for age,estimated associations between catecholamines and XMs.XM levels were validated in a cohort of women(n=14)with and without preeclampsia.Our study identified 582 metabolomes and 125 XM in saliva,demonstrating its potential as a matrix for exposure measurement.After false discovery rate correction,18109 significant metabolome-XM associations were identified.Community clustering revealed 37 connected clusters,with the largest cluster(238 nodes)enriched in tyrosine and catecholamine metabolism.Food-contactchemicals and food-additives were significantly associated with higher catecholamine and their metabolite levels.Subgroup analyses revealed higher concentrations of these chemicals in women with preeclampsia compared to healthy controls.This study demonstrates that saliva contains valuable molecular data for measuring exposomes.Food-related chemicals were associated with higher catecholamine levels,which may be relevant to the prevalence of hypertensive crises in pregnancy.展开更多
Background and objectives:The ongoing mpox outbreaks have garnered significant attention due to their public health implications,particularly the potential mental health impacts.Despite the growing concern,there has b...Background and objectives:The ongoing mpox outbreaks have garnered significant attention due to their public health implications,particularly the potential mental health impacts.Despite the growing concern,there has been limited exploration of the intersection between mpox and mental health within the research literature.This study aims to conduct a comprehensive bibliometric analysis to examine global research trends,regional distribution,and thematic focus areas related to mpox's psychological and psychiatric implications.Methods:We conducted a bibliometric analysis using Scopus and the Web of Science database.The analysis was carried out using the R-bibliometrics package and involved identifying literature on mpox and mental health,focusing on global research trends,regional distribution,and thematic areas of study.The analysis included 416 documents obtained from 295 sources from January 1,2014 to August 27,2024.Results:Our analysis revealed a growing but unevenly distributed literature on mpox and mental health.Most studies concentrated on the relationship between mpox and conditions such as depression and anxiety,while other psychiatric outcomes remain underexplored.The geographic distribution of research was also uneven,with regions like Europe and the Americas receiving more focus than others.Conclusions:The study highlights the need for more targeted research on the mental health sequelae of mpox,particularly for vulnerable populations and regions that are currently underrepresented in the literature.Future research should include longitudinal studies to assess the long-term effects of mpox on mental health and the development of robust methodologies to establish causality.Integrating mental health considerations into public health responses to mpox outbreaks is crucial,with significant implications for research,policy,and clinical practice.展开更多
Objective To examine the mechanistic of organochlorine-associated changes in lung function.Methods This study investigated 76 healthy older adults in Jinan,Shandong Province,over a fivemonth period.Personal exposure t...Objective To examine the mechanistic of organochlorine-associated changes in lung function.Methods This study investigated 76 healthy older adults in Jinan,Shandong Province,over a fivemonth period.Personal exposure to organochlorines was quantified using wearable passive samplers,while inflammatory factors and thyroid hormones were analyzed from blood samples.Participants’lung function was evaluated.After stratifying participants according to their thyroid hormone levels,we analyzed the differential effects of organochlorine exposure on lung function and inflammatory factors across the low and high thyroid hormone groups.Mediation analysis was further conducted to elucidate the relationships among organochlorine exposures,inflammatory factors,and lung function.Results Bis(2-chloro-1-methylethyl)ether(BCIE),was negatively associated with forced vital capacity(FVC,–2.05%,95%CI:–3.11%to–0.97%),and associated with changes in inflammatory factors such as interleukin(IL)-2,IL-7,IL-8,and IL-13 in the low thyroid hormone group.The mediation analysis indicated a mediating effect of IL-2(15.63%,95%CI:0.91%to 44.64%)and IL-13(13.94%,95%CI:0.52%to 41.07%)in the association between BCIE exposure and FVC.Conclusion Lung function and inflammatory factors exhibited an increased sensitivity to organochlorine exposure at lower thyroid hormone levels,with inflammatory factors potentially mediating the adverse effects of organochlorines on lung function.展开更多
Objective:Ozone(O_(3))is recognized as a predominant and increasingly prevalent gaseous pollutant contributing to the Global Burden of Disease.However,its effect on the development of lung cancer has not been adequate...Objective:Ozone(O_(3))is recognized as a predominant and increasingly prevalent gaseous pollutant contributing to the Global Burden of Disease.However,its effect on the development of lung cancer has not been adequately acknowledged.Methods:Based on Health Evaluation And risk Reduction through nationwide Teamwork(ChinaHEART),a nationwide,population-based cohort study,2,006,878 participants living in 20,159 communities or villages were passively followed for the incidence of lung cancer through a linkage of data with the National Central Cancer Registry.The average O_(3) levels over ten years from the year of enrolment in their respective localities were determined based on geographic coordinates.We conducted Cox proportional-hazards regression models to assess the independent hazard ratios(HRs)associated with O_(3) exposure and the development of lung cancer.Results:During the follow-up of 4.93 million person-years,4555 new cases of lung cancer were identified.After adjusting for participant characteristics and other environmental factors,we observed a significant positive association between ambient O_(3) exposure and lung cancer.Compared with participants in the quartile 1 of O_(3),HRs and 95%confidence intervals(CI)for the other three quartiles were 1.09(95%CI:1.00-1.17),1.17(95%CI:1.06-1.29)and 1.42(95%CI:1.26-1.59),respectively.Conclusions:Long-term exposure to ambient O_(3) is associated with a substantially higher risk of lung cancer.Further studies are needed to explore its pathogenic mechanisms,as well as to evaluate measures for exposure protection or harm mitigation at the individual or population level.展开更多

BACKGROUND The use of induction immunosuppression agents has improved kidney transplant outcomes,but selecting the optimal agent remains a point of debate.AIM To compare the long-term outcomes of kidney transplant rec...BACKGROUND The use of induction immunosuppression agents has improved kidney transplant outcomes,but selecting the optimal agent remains a point of debate.AIM To compare the long-term outcomes of kidney transplant recipients receiving alemtuzumab vs basiliximab induction,focusing on graft function,acute rejection,infection,malignancy,post-transplant glomerulonephritis,and survival,using a propensity score matched cohort design.METHODS Kidney transplant recipients who received alemtuzumab or basiliximab induction from 2014 to 2019 across two nephrology centres in Northwest England were evaluated.Propensity score matching at a 1:1.5 ratio ensured comparability between cohorts.Baseline characteristics,immunosuppression regimens,and outcomes were analyzed.Linear,binary logistic and Cox proportional hazard regression models.RESULTS A total of 436 recipients were included,with a median follow-up of 5.2 years.The matched cohort(n=262)had a mean age of 51.1±13.5 years;39%were female and 92%were white.There was no significant difference in the cumulative incidence of acute rejection[odds ratio(OR)=2.10;95%CI:0.9-4.9;P=0.110].Compared with basiliximab,alemtuzumab was associated with lower estimated glomerular filtration rate at 12 months(-6.6 mL/minute/1.73 m2;95%CI:-10.5 to-2.7;P<0.001)and higher risks of cytomegalovirus viremia(OR=3.2;95%CI:1.6-6.5;P<0.001),BK viremia(OR=2.4;95%CI:1.1-5.5;P=0.02),post-transplant malignancy(OR=6.2;95%CI:1.6-29.9;P=0.013),and death-censored graft loss(hazard ratio=3.6;95%CI:1.2-11.4;P=0.03).No significant differences were observed in post-transplant glomerulonephritis or recipient mortality.CONCLUSION In this propensity score-matched analysis,alemtuzumab induction was associated with lower graft function at 12 months and higher risks of viral infection,post-transplant malignancy,and graft loss compared with basiliximab.These findings highlight the need for further studies to confirm the long-term safety and effectiveness of alemtuzumab in kidney transplantation.展开更多

The survey was carried out in urban and semi...AIM: To determine the parental transmission of diabetes mellitus (DM) and evaluate its influence on the clinical characteristics. METHODS: This was a cross sectional study. The survey was carried out in urban and semi-urban primary health care centers. Of the 2400 registered with diagnosed diabetes, 1980 agreed and gave their consent to take part in this study, thus giving a response rate of 82.5%. Face to face interviews were conducted using a structured questionnaire followed by laboratory tests. DM was defined according to the World Health Organization expert group. A trained nurse performedphysical examinations and measurements. RESULTS: Of the study population, 72.9% reported a family history of DM. Family history of DM was significantly higher in females (54.2%; P = 0.04) and in the age group below 30 years (24%; P < 0.001). The prevalence of diabetes was higher among patients with a diabetic mother (25.4% vs 22.1%) and maternal aunts/uncles (31.2% vs 22.2%) compared to patients with a diabetic father and paternal aunts/ uncles. Family history of DM was higher in patients of consanguineous parents (38.5%) than those of non-consanguineous parents (30.2%). The development of type 2 diabetes mellitus (T2DM) complications was higher in patients with either a paternal or maternal history of DM than in those without. No significant difference was observed in the metabolic characteristics of patients with/without family history of DM except for hypertension. Complications were higher in diabetic patients with a family history of DM. CONCLUSION: The present study found a significant maternal effect in transmission of T2DM. Family history is associated with the increased incidence of diabetes.展开更多
Selenium is an essential trace mineral important for the maintenance of homeostasis in animals and humans.It evinces a strong antioxidant,anti-inflammatory and potential antimicrobial capacity.Selenium biological func...Selenium is an essential trace mineral important for the maintenance of homeostasis in animals and humans.It evinces a strong antioxidant,anti-inflammatory and potential antimicrobial capacity.Selenium biological function is primarily achieved by its presence in selenoproteins as a form of selenocysteine.Selenium deficiency may result in an array of health disorders,affecting many organs and systems;to prevent this,dietary supplementation,mainly in the forms of organic(i.e.,selenomethionine and selenocysteine)inorganic(i.e.,selenate and selenite)sources is used.In pigs as well as other food animals,dietary selenium supplementation has been used for improving growth performance,immune function,and meat quality.A substantial body of knowledge demonstrates that dietary selenium supplementation is positively associated with overall animal health especially due to its immunomodulatory activity and protection from oxidative damage.Selenium also possesses potential antiviral activity and this is achieved by protecting immune cells against oxidative damage and decreasing viral replication.In this review we endeavor to combine established and novel knowledge on the beneficial effects of dietary selenium supplementation,its antioxidant and immunomodulatory actions,and the putative antimicrobial effect thereof.Furthermore,our review demonstrates the gaps in knowledge pertaining to the use of selenium as an antiviral,underscoring the need for further in vivo and in vitro studies,particularly in pigs.展开更多
Background: Creep feeding is used to stimulate piglet post-weaning feed consumption.L-Glutamine(GLN) is an important source of fuel for intestinal epithelial cells.The objective of this study was to determine the i...Background: Creep feeding is used to stimulate piglet post-weaning feed consumption.L-Glutamine(GLN) is an important source of fuel for intestinal epithelial cells.The objective of this study was to determine the impact of creep feeding and adding GLN or AminoGut(AG;containing glutamine + glutamate) to pre-and post-weaning diets on pig performance and intestinal health.Litters(N = 120) were allotted to four treatments during 14–21 d of lactation: 1) No creep feed(NC,n = 45);2) creep fed control diet(CFCD,n = 45);3) creep fed 1% GLN(CFGLN,n = 15);4) creep fed.88% AG(CFAG,n = 15).After weaning,the NC and CFCD groups were sub-divided into three groups(n = 15 each),receiving either a control nursery diet(NC-CD,CFCD-CD) or a diet supplemented with either GLN(NC-GLN,CFCD-GLN) or with AG(NC-AG,CFCD-AG).Litters that were creep fed with diets containing GLN or AG also were supplemented with those amino acids in the nursery diets(CFGLN-GLN,CFAG-AG).Glutamine was added at 1% in all three post-weaning diet phases and AG was added at.88% in phase 1 and 2 and at.66% in phase 3.Results: Feed conversion(feed/gain) showed means among treatment means close to significance(P = 0.056) and Tukey's test for pairwise mean comparisons showed that Pigs in the CFGLN-GLN group had the best feed conversion(feed/gain) in the first three-week period post-weaning,exceeding(P = 0.044) controls(CFCD-CD) by 34%.The NC-AG group had(P = 0.02) the greatest feed intake in the last three week of the study,exceeding controls(CFCD-CD) by 12%.CFGLN-GLN,CFCD-GLN and sow reared(SR) pigs had the greatest(P = 0.049) villi height exceeding the CFCD-AG group by 18%,20% and 19% respectively.The CFAG-AG group had the deepest(P = 0.001) crypts among all treatments.CFGLN-GLN,CFCD-GLN and SR groups had the greatest(P = 0.001) number of cells proliferating(PCNA) exceeding those in the NC-CD group by 43%,54% and 63% respectively.Sow reared pigs showed the greatest(P = 0.001) intestinal absorption capacity for xylose and mannitol.Conclusion: Supplementation of creep feed and nursery diets with GLN and/or AminoGut in the first three week improved feed conversion possibly due to improved intestinal health.展开更多
The incidence of infertility has recently risen.Semen quality is an important male fertility indicator,and dietary factors can affect semen quality.We conducted this systematic review and meta-analysis to determine th...The incidence of infertility has recently risen.Semen quality is an important male fertility indicator,and dietary factors can affect semen quality.We conducted this systematic review and meta-analysis to determine the effects of healthy dietary patterns on semen quality.A literature search was conducted in 3 databases(Embase,Web of Science and PubMed)on August 21,2021.The included cross-sectional studies examined the influence of the Mediterranean,Dietary Approaches to Stop Hypertension(DASH),and prudent diet patterns on semen quality parameters;six studies(1244 subjects)were included.By comparing high consumption with low consumption of healthy dietary patterns,the results of the meta-analysis showed significantly higher sperm concentrations(mean difference[MD]=6.88×10^(6) ml^(−1),95%confidence interval[CI]:1.26×10^(6) ml^(−1)–12.49×10^(6) ml^(−1);P<0.05),a significant increase in total sperm count(MD=16.70×10^(6),95%CI:2.37×10^(6)–31.03×10^(6);P<0.05),and a significant increase in progressive sperm motility(MD=5.85%,95%CI:2.59%–9.12%;P<0.01).The sperm concentration,progressive sperm motility,and total sperm count were significantly higher in men with higher versus lower consumption of healthy dietary patterns.However,the results must be interpreted with caution.展开更多

文摘Objectives To characterize fine particulate matter(PM_(2.5))-bound polycyclic aromatic hydrocarbons(PAHs)emitted from different cooking fumes and their exposure routes and assess their health-associated impact to provide a reference for health risk prevention from PAH exposure across different age and sex groups.Methods Sixteen PM_(2.5)-bound PAHs emitted from 11 cooking styles were analyzed using GC-MS/MS.The health hazards of these PAHs in the Handan City population(stratified by age and sex)were predicted using the incremental lifetime cancer risk(ILCR)model.The respiratory deposition doses(RDDs)of the PAHs in children and adults were calculated using the PM_(2.5)deposition rates in the upper airway,tracheobronchial,and alveolar regions.Results The total concentrations of PM_(2.5)-bound PAHs ranged from 61.10 to 403.80 ng/m3.Regardless of cooking styles,the ILCRtotal values for adults(1.23×10^(-6)to 3.70×10^(-6))and older adults(1.28×10^(-6)to 3.88×10^(-6))exceeded the acceptable limit of 1.00×10^(-6).With increasing age,the ILCRtotal value first declined and then increased,varying substantially among the population groups.Cancer risk exhibited particularly high sensitivity to short exposure to barbecue-derived PAHs under equivalent body weights.Furthermore,barbecue,Sichuan and Hunan cuisine,Chinese cuisine,and Chinese fast food were associated with higher RDDs for both adults and children.Conclusion ILCRtotal values exceeded the acceptable limit for both females and males of adults,with all cooking styles showing a potentially high cancer risk.Our findings serve as an important reference for refining regulatory strategies related to catering emissions and mitigating health risks associated with cooking styles.
文摘There have been an increasing number of studies on climate change and population health over the past 20 years,with most focusing on health risk assessment,targeting different locations and populations with various diseases[1−2].While these studies have provided the necessary epidemiological evidence for health authorities in policymaking,it is time to develop and implement tailored health interventions to protect the health and well-being of communities,and particularly that of vulnerable groups.

文摘Standardized datasets are foundational to healthcare informatization by enhancing data quality and unleashing the value of data elements.Using bibliometrics and content analysis,this study examines China's healthcare dataset standards from 2011 to 2025.It analyzes their evolution across types,applications,institutions,and themes,highlighting key achievements including substantial growth in quantity,optimized typology,expansion into innovative application scenarios such as health decision support,and broadened institutional involvement.The study also identifies critical challenges,including imbalanced development,insufficient quality control,and a lack of essential metadata—such as authoritative data element mappings and privacy annotations—which hampers the delivery of intelligent services.To address these challenges,the study proposes a multi-faceted strategy focused on optimizing the standard system's architecture,enhancing quality and implementation,and advancing both data governance—through authoritative tracing and privacy protection—and intelligent service provision.These strategies aim to promote the application of dataset standards,thereby fostering and securing the development of new productive forces in healthcare.
文摘BACKGROUND The neurological and psychiatric sequelae of coronavirus disease 2019(COVID-19)have been documented,yet further data are needed to thoroughly evaluate the impact of COVID-19 on brain health years after the infection.AIM To examine whether COVID-19 infection is associated with exacerbation,recurrence,or progression of pre-existing neurological or psychiatric disorders-a highrisk population that is underrepresented in COVID-19 outcome research in National Guard Health Affair patients in 4-years following a COVID-19 diagnosis.METHODS For this multicenter retrospective cohort study,we used data from the National Guard hospitals electronic health records network(BestCare)with over 10 million patients.Our cohort comprised patients who had a COVID-19 diagnosis;a matched control cohort included patient did not expose to COVID-19 in same period.Age and sex were matching factors.We estimated the incidence of 14 neurological and psychiatric outcomes in nearly 4 years after a confirmed diagnosis of COVID-19.Using a multiple logistic regression,we compared incidences with those in propensity score-matched cohorts of patients with no exposure to COVID-19.Ramadan M et al.Neurological and psychiatric risks post-COVID-19 RESULTS Our primary cohort comprised 4437 patients diagnosed with COVID-19,and our propensity-score 1:1 matched control cohort comprised 4437 individuals.Nearly two-third of the COVID-19 cohort(71%)were diagnosed in 2020,and 2021.The most prevalent diagnoses for both cohorts were epilepsy 30.68%,mood disorder 23.92%,and nerve plexus disorder 22.13%.Dementia was nearly 4 times higher among COVID-19 cohort(8.27%)compared to the control cohort(2.57%).Five neurological and psychiatric outcomes had odds ratios(OR)that were significantly higher than 1 for people who had COVID-19 compared to people who had never been infected.These outcomes were cognitive deficit OR=1.54,95%confidence interval(CI):1.23-1.91,P=0.0001;nerve plexus disorder OR=1.13,95%CI:1.01-1.25,P=0.02;substance use disorder OR=1.95,95%CI:1.12-3.38,P=0.01;mood disorder OR=1.16,95%CI:1.05-1.29,P=0.003;and anxiety disorder OR=1.39,95%CI:1.07-1.79,P=0.01.CONCLUSION The study highlights the persistent risk of neurological and psychiatric conditions in COVID-19 survivors up to four years post-infection.Although the incidence was lower than in previous large studies,long-term consequences remain significant,emphasizing the need for ongoing monitoring and support in mental health and neurological care.

文摘Chronic obstructive pulmonary disease(COPD)is a complex condition marked by considerable interindividual heterogeneity.Comorbidities exacerbate this variability,worsening disease severity and reducing health-related quality of life(HRQoL).Despite the high prevalence of COPD in China,COPD patient clusters remain poorly characterized.This study aimed to identify and validate clusters of Chinese patients with COPD,characterized primarily by comorbidity profiles,using cluster analysis.This cross-sectional,multicenter cohort study used data from the Chinese Enjoying Breathing Program(2020–2023).HRQoL was measured using the EuroQol five dimension(EQ-5D).Dimension reduction was performed via multiple correspondence analysis on 31 variables,including indicators of 27 comorbidities and four sociodemographic or health-related characteristics.Unsupervised machine learning algorithms,K-means++,and hierarchical clustering identified distinct clusters.Robustness was assessed using random forest classification.Logistic regression evaluated the association between cluster membership and EQ-5D outcomes.Among 11145 patients,59.4%had comorbidities.Four clusters emerged:young male smokers,biomass-exposed females,respiratory comorbidity,and elderly multimorbid.The last two clusters had notably lower HRQoL.Cluster analysis identified four clinically meaningful COPD patient clusters based on comorbidities and risk profiles,each with distinct HRQoL outcomes.These findings support targeted public health interventions and integrated care strategies for COPD management.

文摘Chemical exposure during prenatal development has significant implications for both maternal and child health.Compared to blood,saliva is a non-invasive and less resource-intensive,alternative.Given the temporal variability of xenobiotic metabolites(XM),repeated sampling is essential.Therefore,saliva offers a valuable tool for the longitudinal assessment of prenatal exposomes.Despite its potential,no studies have explored saliva for XM measurement.This study pioneered using saliva to assess XM detectability and investigate the associations between prenatal XM and endogenous metabolomes in pregnant women.Saliva samples were analysed using mass spectrometry from 80 pregnant women at 24–34 weeks gestation.Metabolomes and exposomes were annotated using the Human Metabolome and U.S.Environmental Protection Agency databases.Metabolome-XM associations were clustered using Glay community clustering.Linear regression models,adjusted for age,estimated associations between catecholamines and XMs.XM levels were validated in a cohort of women(n=14)with and without preeclampsia.Our study identified 582 metabolomes and 125 XM in saliva,demonstrating its potential as a matrix for exposure measurement.After false discovery rate correction,18109 significant metabolome-XM associations were identified.Community clustering revealed 37 connected clusters,with the largest cluster(238 nodes)enriched in tyrosine and catecholamine metabolism.Food-contactchemicals and food-additives were significantly associated with higher catecholamine and their metabolite levels.Subgroup analyses revealed higher concentrations of these chemicals in women with preeclampsia compared to healthy controls.This study demonstrates that saliva contains valuable molecular data for measuring exposomes.Food-related chemicals were associated with higher catecholamine levels,which may be relevant to the prevalence of hypertensive crises in pregnancy.
文摘Background and objectives:The ongoing mpox outbreaks have garnered significant attention due to their public health implications,particularly the potential mental health impacts.Despite the growing concern,there has been limited exploration of the intersection between mpox and mental health within the research literature.This study aims to conduct a comprehensive bibliometric analysis to examine global research trends,regional distribution,and thematic focus areas related to mpox's psychological and psychiatric implications.Methods:We conducted a bibliometric analysis using Scopus and the Web of Science database.The analysis was carried out using the R-bibliometrics package and involved identifying literature on mpox and mental health,focusing on global research trends,regional distribution,and thematic areas of study.The analysis included 416 documents obtained from 295 sources from January 1,2014 to August 27,2024.Results:Our analysis revealed a growing but unevenly distributed literature on mpox and mental health.Most studies concentrated on the relationship between mpox and conditions such as depression and anxiety,while other psychiatric outcomes remain underexplored.The geographic distribution of research was also uneven,with regions like Europe and the Americas receiving more focus than others.Conclusions:The study highlights the need for more targeted research on the mental health sequelae of mpox,particularly for vulnerable populations and regions that are currently underrepresented in the literature.Future research should include longitudinal studies to assess the long-term effects of mpox on mental health and the development of robust methodologies to establish causality.Integrating mental health considerations into public health responses to mpox outbreaks is crucial,with significant implications for research,policy,and clinical practice.

文摘Objective To examine the mechanistic of organochlorine-associated changes in lung function.Methods This study investigated 76 healthy older adults in Jinan,Shandong Province,over a fivemonth period.Personal exposure to organochlorines was quantified using wearable passive samplers,while inflammatory factors and thyroid hormones were analyzed from blood samples.Participants’lung function was evaluated.After stratifying participants according to their thyroid hormone levels,we analyzed the differential effects of organochlorine exposure on lung function and inflammatory factors across the low and high thyroid hormone groups.Mediation analysis was further conducted to elucidate the relationships among organochlorine exposures,inflammatory factors,and lung function.Results Bis(2-chloro-1-methylethyl)ether(BCIE),was negatively associated with forced vital capacity(FVC,–2.05%,95%CI:–3.11%to–0.97%),and associated with changes in inflammatory factors such as interleukin(IL)-2,IL-7,IL-8,and IL-13 in the low thyroid hormone group.The mediation analysis indicated a mediating effect of IL-2(15.63%,95%CI:0.91%to 44.64%)and IL-13(13.94%,95%CI:0.52%to 41.07%)in the association between BCIE exposure and FVC.Conclusion Lung function and inflammatory factors exhibited an increased sensitivity to organochlorine exposure at lower thyroid hormone levels,with inflammatory factors potentially mediating the adverse effects of organochlorines on lung function.

文摘Objective:Ozone(O_(3))is recognized as a predominant and increasingly prevalent gaseous pollutant contributing to the Global Burden of Disease.However,its effect on the development of lung cancer has not been adequately acknowledged.Methods:Based on Health Evaluation And risk Reduction through nationwide Teamwork(ChinaHEART),a nationwide,population-based cohort study,2,006,878 participants living in 20,159 communities or villages were passively followed for the incidence of lung cancer through a linkage of data with the National Central Cancer Registry.The average O_(3) levels over ten years from the year of enrolment in their respective localities were determined based on geographic coordinates.We conducted Cox proportional-hazards regression models to assess the independent hazard ratios(HRs)associated with O_(3) exposure and the development of lung cancer.Results:During the follow-up of 4.93 million person-years,4555 new cases of lung cancer were identified.After adjusting for participant characteristics and other environmental factors,we observed a significant positive association between ambient O_(3) exposure and lung cancer.Compared with participants in the quartile 1 of O_(3),HRs and 95%confidence intervals(CI)for the other three quartiles were 1.09(95%CI:1.00-1.17),1.17(95%CI:1.06-1.29)and 1.42(95%CI:1.26-1.59),respectively.Conclusions:Long-term exposure to ambient O_(3) is associated with a substantially higher risk of lung cancer.Further studies are needed to explore its pathogenic mechanisms,as well as to evaluate measures for exposure protection or harm mitigation at the individual or population level.

文摘Alzheimer’s disease(AD)is the most common form of dementia characterized pathologically by the deposition of amyloid plaques and hyperphosphorylated tau containing neurofibrillary tangles.The disease presents clinically with progressive memory loss and disruption of cognitive function.Currently,there is no cure for AD;recent advances in the therapeutics aimed at clearing the amyloid protein from the brain have led to potential disease stabilization,however,this does not prevent eventual disease progression(Cummings et al.,2024).

文摘Vitamin D deficiency(VDD)represents a significant nutritional concern among children and adolescents.The estimated prevalence of VDD in China is 46.8%in this population^([1]).VDD during childhood and adolescence has been associated with the onset of various conditions,including acute respiratory infections,asthma,atopic dermatitis,and food allergies^([2]).Multiple factors,including age,sun exposure,adiposity,and genetics,influence vitamin D levels^([2,3]).Increasing attention has been directed toward understanding the environmental determinants that may influence vitamin D status.Given the potential of metallic pollutants to disrupt endocrine function and their ubiquity in the environment,investigating the effects of metal exposure on human vitamin D status,particularly in vulnerable populations,is imperative.

文摘BACKGROUND The use of induction immunosuppression agents has improved kidney transplant outcomes,but selecting the optimal agent remains a point of debate.AIM To compare the long-term outcomes of kidney transplant recipients receiving alemtuzumab vs basiliximab induction,focusing on graft function,acute rejection,infection,malignancy,post-transplant glomerulonephritis,and survival,using a propensity score matched cohort design.METHODS Kidney transplant recipients who received alemtuzumab or basiliximab induction from 2014 to 2019 across two nephrology centres in Northwest England were evaluated.Propensity score matching at a 1:1.5 ratio ensured comparability between cohorts.Baseline characteristics,immunosuppression regimens,and outcomes were analyzed.Linear,binary logistic and Cox proportional hazard regression models.RESULTS A total of 436 recipients were included,with a median follow-up of 5.2 years.The matched cohort(n=262)had a mean age of 51.1±13.5 years;39%were female and 92%were white.There was no significant difference in the cumulative incidence of acute rejection[odds ratio(OR)=2.10;95%CI:0.9-4.9;P=0.110].Compared with basiliximab,alemtuzumab was associated with lower estimated glomerular filtration rate at 12 months(-6.6 mL/minute/1.73 m2;95%CI:-10.5 to-2.7;P<0.001)and higher risks of cytomegalovirus viremia(OR=3.2;95%CI:1.6-6.5;P<0.001),BK viremia(OR=2.4;95%CI:1.1-5.5;P=0.02),post-transplant malignancy(OR=6.2;95%CI:1.6-29.9;P=0.013),and death-censored graft loss(hazard ratio=3.6;95%CI:1.2-11.4;P=0.03).No significant differences were observed in post-transplant glomerulonephritis or recipient mortality.CONCLUSION In this propensity score-matched analysis,alemtuzumab induction was associated with lower graft function at 12 months and higher risks of viral infection,post-transplant malignancy,and graft loss compared with basiliximab.These findings highlight the need for further studies to confirm the long-term safety and effectiveness of alemtuzumab in kidney transplantation.

文摘AIM: To determine the parental transmission of diabetes mellitus (DM) and evaluate its influence on the clinical characteristics. METHODS: This was a cross sectional study. The survey was carried out in urban and semi-urban primary health care centers. Of the 2400 registered with diagnosed diabetes, 1980 agreed and gave their consent to take part in this study, thus giving a response rate of 82.5%. Face to face interviews were conducted using a structured questionnaire followed by laboratory tests. DM was defined according to the World Health Organization expert group. A trained nurse performedphysical examinations and measurements. RESULTS: Of the study population, 72.9% reported a family history of DM. Family history of DM was significantly higher in females (54.2%; P = 0.04) and in the age group below 30 years (24%; P < 0.001). The prevalence of diabetes was higher among patients with a diabetic mother (25.4% vs 22.1%) and maternal aunts/uncles (31.2% vs 22.2%) compared to patients with a diabetic father and paternal aunts/ uncles. Family history of DM was higher in patients of consanguineous parents (38.5%) than those of non-consanguineous parents (30.2%). The development of type 2 diabetes mellitus (T2DM) complications was higher in patients with either a paternal or maternal history of DM than in those without. No significant difference was observed in the metabolic characteristics of patients with/without family history of DM except for hypertension. Complications were higher in diabetic patients with a family history of DM. CONCLUSION: The present study found a significant maternal effect in transmission of T2DM. Family history is associated with the increased incidence of diabetes.

文摘Selenium is an essential trace mineral important for the maintenance of homeostasis in animals and humans.It evinces a strong antioxidant,anti-inflammatory and potential antimicrobial capacity.Selenium biological function is primarily achieved by its presence in selenoproteins as a form of selenocysteine.Selenium deficiency may result in an array of health disorders,affecting many organs and systems;to prevent this,dietary supplementation,mainly in the forms of organic(i.e.,selenomethionine and selenocysteine)inorganic(i.e.,selenate and selenite)sources is used.In pigs as well as other food animals,dietary selenium supplementation has been used for improving growth performance,immune function,and meat quality.A substantial body of knowledge demonstrates that dietary selenium supplementation is positively associated with overall animal health especially due to its immunomodulatory activity and protection from oxidative damage.Selenium also possesses potential antiviral activity and this is achieved by protecting immune cells against oxidative damage and decreasing viral replication.In this review we endeavor to combine established and novel knowledge on the beneficial effects of dietary selenium supplementation,its antioxidant and immunomodulatory actions,and the putative antimicrobial effect thereof.Furthermore,our review demonstrates the gaps in knowledge pertaining to the use of selenium as an antiviral,underscoring the need for further in vivo and in vitro studies,particularly in pigs.
文摘Background: Creep feeding is used to stimulate piglet post-weaning feed consumption.L-Glutamine(GLN) is an important source of fuel for intestinal epithelial cells.The objective of this study was to determine the impact of creep feeding and adding GLN or AminoGut(AG;containing glutamine + glutamate) to pre-and post-weaning diets on pig performance and intestinal health.Litters(N = 120) were allotted to four treatments during 14–21 d of lactation: 1) No creep feed(NC,n = 45);2) creep fed control diet(CFCD,n = 45);3) creep fed 1% GLN(CFGLN,n = 15);4) creep fed.88% AG(CFAG,n = 15).After weaning,the NC and CFCD groups were sub-divided into three groups(n = 15 each),receiving either a control nursery diet(NC-CD,CFCD-CD) or a diet supplemented with either GLN(NC-GLN,CFCD-GLN) or with AG(NC-AG,CFCD-AG).Litters that were creep fed with diets containing GLN or AG also were supplemented with those amino acids in the nursery diets(CFGLN-GLN,CFAG-AG).Glutamine was added at 1% in all three post-weaning diet phases and AG was added at.88% in phase 1 and 2 and at.66% in phase 3.Results: Feed conversion(feed/gain) showed means among treatment means close to significance(P = 0.056) and Tukey's test for pairwise mean comparisons showed that Pigs in the CFGLN-GLN group had the best feed conversion(feed/gain) in the first three-week period post-weaning,exceeding(P = 0.044) controls(CFCD-CD) by 34%.The NC-AG group had(P = 0.02) the greatest feed intake in the last three week of the study,exceeding controls(CFCD-CD) by 12%.CFGLN-GLN,CFCD-GLN and sow reared(SR) pigs had the greatest(P = 0.049) villi height exceeding the CFCD-AG group by 18%,20% and 19% respectively.The CFAG-AG group had the deepest(P = 0.001) crypts among all treatments.CFGLN-GLN,CFCD-GLN and SR groups had the greatest(P = 0.001) number of cells proliferating(PCNA) exceeding those in the NC-CD group by 43%,54% and 63% respectively.Sow reared pigs showed the greatest(P = 0.001) intestinal absorption capacity for xylose and mannitol.Conclusion: Supplementation of creep feed and nursery diets with GLN and/or AminoGut in the first three week improved feed conversion possibly due to improved intestinal health.

文摘The incidence of infertility has recently risen.Semen quality is an important male fertility indicator,and dietary factors can affect semen quality.We conducted this systematic review and meta-analysis to determine the effects of healthy dietary patterns on semen quality.A literature search was conducted in 3 databases(Embase,Web of Science and PubMed)on August 21,2021.The included cross-sectional studies examined the influence of the Mediterranean,Dietary Approaches to Stop Hypertension(DASH),and prudent diet patterns on semen quality parameters;six studies(1244 subjects)were included.By comparing high consumption with low consumption of healthy dietary patterns,the results of the meta-analysis showed significantly higher sperm concentrations(mean difference[MD]=6.88×10^(6) ml^(−1),95%confidence interval[CI]:1.26×10^(6) ml^(−1)–12.49×10^(6) ml^(−1);P<0.05),a significant increase in total sperm count(MD=16.70×10^(6),95%CI:2.37×10^(6)–31.03×10^(6);P<0.05),and a significant increase in progressive sperm motility(MD=5.85%,95%CI:2.59%–9.12%;P<0.01).The sperm concentration,progressive sperm motility,and total sperm count were significantly higher in men with higher versus lower consumption of healthy dietary patterns.However,the results must be interpreted with caution.
