OBJECTIVE:To explore the possible mechanism of Tongdu Tiaoshen acupuncture combined with Xiaoxuming decoction(小续命汤,XXMD)in the treatment of Parkinson’s disease(PD).METHODS:C57BL/6 mice were randomly divided into eight groups(n=12),including blank group,model group,medication group,acupuncture group,high-dose XXMD group(XXMD-H),low-dose XXMD group(XXMD-L),acupuncture combined with high-dose XXMD group(A+H),and acupuncture combined with low-dose XXMD group(A+L).After treatment for 6 weeks,dopamine(DA)neurons and the pathological changes of tyrosine hydroxylase(TH)positive cells were observed.The enzyme-linked immunosorbent assay(ELISA)was used to measure the content of DA and the level of interleukin-1β(IL-1β),interleukin-6(IL-6),interleukin-10(IL-10)and tumor necrosis factor alpha(TNF-α).The m RNA level of PINK1 and Parkin and the protein expression of Nix,PINK1 and Parkin in the substantia nigra were also detected.RESULTS:Combination treatment effectively ameliorated the symptoms of PD.Compared with model group,combined treatment significantly up-regulated the protein expression of Nix,Parkin and PINK1 and the m RNA levels of PINK1 and Parkin in the substantia nigra(P<0.0001,P<0.001,P<0.01 or P<0.05).Furthermore,the levels of pro-inflammation cytokines were obviously decreased after combination therapy,while IL-10 content was increased remarkably(P<0.01).CONCLUSION:Compared with each treatment alone,combination therapy improved the pathological damage of DA neurons of PD mice more effectively.The possible mechanism may be attributed to the up-regulated level of mitochondrial autophagy and improved mitochondrial function.These results provide fresh insight into the mechanism of co-treatment with Tongdu Tiaoshen acupuncture and XXMD for PD. 展开更多

Objective:To observe the impacts of the combined treatment of Tongdu Tiaoshen moxibustion(moxibustion for promoting the circulation of the governor vessel and regulating the spirit)and rehabilitation training on the motor function recovery in the patients with apoplectic hemiplegia.Methods:A total of 50 patients with apoplectic hemiplegia and qualified in the trial recruitment criteria were divided into two groups according to random number table,25 cases in each group.In the control group,the simple rehabilitation training was provided.In the observation group,on the base of the treatment as the control group,Tongdu Tiaoshen moxibustion was given.Main acupoints:Baihul(百会GV20),Fengfu(风府GV16)and Dazhui(大椎GV14).The treatment was given once a day,6 treatments a week,4 weeks as one course and two courses of treatment were required.Before and after treatment,the scores of Fugle-Meyer assessment(FMA),the modified Barthel index(MBI)and action research arm test(ARAT)were detected before and after treatment in the two groups separately.Results:After 4 and 8 weeks of treatment,the scores of FMA,MBI and ARAT were all improved obviously as compared with those before treatment respectively in patients of the two groups(all P<0.01).After8 weeks of treatment,the score of each scale in the observation group was more obviously improved as compared with the control group(all P<0.05).The difference in the clinical therapeutic effect was significant statistically between the two groups(P=0.005).Conclusion:The combined treatment of Tongdu Tiaoshen moxibustion and rehabilitation training promotes the recovery of the upper limb motor function and improves the self-ability of living activities in the patients with apoplectic hemiplegia.The long-term effect of this combined therapy is superior to that of simple rehabilitation training. 展开更多

Lumbar spinal stenosis is caused by the compression of the nerve root or cauda equina nerve by stenosis of the lumbar spinal canal or intervertebral foramen,and is manifested as chronic low back and leg pain.Danlu Tongdu(DLTD)tablets can relieve chronic pain caused by lumbar spinal stenosis,but the molecular mechanism remains largely unknown.In this study,the potential molecular mechanism of DLTD tablets in the treatment of lumbar spinal stenosis was first predicted by the network pharmacology method.Results showed that DLTD functions in regulating anti-oxidative,apoptosis,and inflammation signaling pathways.Furthermore,the flow cytometry results showed that DLTD tablets efficiently reduced reactive oxygen species content and inhibited rat neural stem cell apoptosis induced by hydrogen peroxide.DLTD also inhibited the mitochondrial membrane potential damage induced by hydrogen peroxide.Elisa analysis showed that DLTD induced cell cycle-related protein,CDK2 and CDK4,and reduced CDKN1A protein expression level.Taken together,our study provided new insights of DLTD in treating lumbar spinal stenosis through reducing reactive oxygen species content,decreasing apoptosis by inhibiting CDKN1A and promoting CDK2 and CDK4 expression levels. 展开更多

[Objectives]To evaluate the effects of Tongdu Tiaoshen Acupuncture method on improving learning and memory function and hippocampal nerve regeneration and repair after stroke.[Methods]Animal models of cognitive impairment in the recovery phase of cerebral infarction in rats were prepared,and rats were randomly divided into control group,model group,donepezil group,and acupuncture group.Morris water maze experiment was carried out to evaluate learning and memory ability.Nissl staining,BrdU and DCX immunofluorescence staining were used to observe the number and morphological structure of neuronal pyramidal and granular cells in the dentate gyrus(DG)area of the hippocampus,as well as the proliferation and differentiation of neural progenitor cells.TUNEL method was used to detect cell apoptosis.[Results]Before intervention,the scores and ELP of the model group,donepezil group and acupuncture group were significantly higher than those of the blank group(P<0.05);after the intervention,the neurological scores and ELP of the blank group and the model group were not significantly different from those before the intervention(P<0.05);the donepezil group and acupuncture group were significantly reduced,there was no difference between the donepezil group and acupuncture group(P>0.05),but it was still significantly higher than the blank group(P<0.05).The results of BrdU and DCX immunofluorescence staining showed that there were only a few BrdU positive cells and DCX protein in the hippocampal DG area of mice in the blank group,and the positive cells in the model group were significantly increased(P<0.05);donepezil group and acupuncture group showed significant increase compared with the model group(P<0.05);compared with the blank group,the number of neurons in the hippocampal DG area of each group was significantly reduced(P<0.01),and the neuronal apoptosis index was significantly increased(P<0.01);compared with the blank group,the number of neurons in the hippocampal DG area of each group was significantly reduced(P<0.01),and the neuronal apoptosis index was significantly increased(P<0.01).The number of neurons in the hippocampal DG area of rats in the acupuncture group and donepezil group was significantly increased(P<0.01),and the neuronal apoptosis index was significantly decreased(P<0.01).[Conclusions]Tongdu Tiaoshen Acupuncture method can improve the learning and memory function of stroke model rats by promoting the regeneration and repair of hippocampal nerve in rats with cerebral infarction. 展开更多

Spine-related diseases include scoliosis, intervertebral disc herniation, intervertebral stenosis, spinal fracture, ankylosing spondylitis, spinal cord injury, spinal tumour, etc., which seriously affect people’s daily life. This paper summarises the relevant studies on the treatment of spinal diseases by Tongzhu therapy. The mechanism of action of Tongzhu therapy in treating spinal diseases is closely related to the control of inflammatory factors, the improvement of local microcirculation, analgesia, and the promotion of nerve repair. This paper describes the application and modern mechanism of Tongzhu therapy in the treatment of spinal diseases, which will provide a direction for future research on Tongzhu therapy. 展开更多

Objective To study the action mechanism of Wenshen Tongdu Recipe(WSTDR)inpromoting microvascular endothelial cells(MECs)phagocytosis and clearing myelin debris by reducing iron overload phenomenon after spinal cord injury,thereby improving the infiammatory microenvironment after spinal cord injury.Methods Fifty C57BL/6 mice were divided into 5 groups by random number table method:sham operation group,model group,deferoxamine(DFO)group,WSTDR group,WSTDR+DFO group,10 mice in each group.A mouse model of spinal cord injury was prepared by modified Allen's method.The pathological changes of the spinal cord were observed by HE staining and Nissl staining.The iron ion content in serum was determined by the iron ion colorimetric assay kit.The expression levels of monocyte chemoattractant protein-1(MCP-1),interleukin(IL)-6,IL-1β,and tumor necrosis factor-α(TNF-α)in the homogenate and serum of the spinal cord were detected by ELISA.The phagocytosis and clearance of myelin debris by MECs in the spinal cord were observed by 3D laser confocal.Results One mouse died in the model group,DFO group,and WSTDR group,respectively.The results of HE staining showed that compared with the model group,spinal cord cavities were reduced with nerve fiber disorder in DFO group and WSTDR group.Nissl staining analysis showed that compared with the model group,Nissl morphology and neuronal pyknosis were improved in DFO group and WSTDR group.Ferric ion colorimetric assay showed that compared with the model group,serum iron ion concentration were significantly reduced in DFO group and WSTDR group(P<0.01).ELISA results showed that compared with the sham-operation group,the levels of MCP-1,IL-6,IL-1β,and TNF-αin serum and spinal cord tissues significantly increased in the model group(P<0.01).Compared with the model group,serum levels of MCP-1,IL-6,IL-1β,and TNF-αwere significantly lowered in DFO group and WSTDR group(P<0.05,P<0.01).Serum concentrations of MCP-1 and IL-1βwere significantly lowered in WSTDR+DFO group(P<0.01).Compared with WSTDR group,serum concentrations of MCP-1,IL-6,anddIL-1βwere significantly improved in WSTDR+DFO group(P<0.01).Compared with the model group,the levels of MCP-1,IL-6,IL-1β(except WSTDR group),and TNF-αin spinal cord tissues were significantly reduced in DFO group,WSTDR group,and WSTDR+DFO group(P<0.05,P<0.01).3D laser confocal results showed MECs phagocytosis and clearance of myelin debris were promoted in DFO group and WSTDR group.Conclusion WSTDR promotes MECs to engulf myelin debris by reducing iron overload,improving the infiammatory microenvironment,and promoting spinal cord injury repairment. 展开更多

目的探讨舌三针埋针下电刺激联合通督调神电针对结核性脑膜炎(TBM)伴吞咽障碍患者临床症状及炎性反应的影响。方法选取2023年3月至2025年3月广州市胸科医院收治的52例TBM伴吞咽障碍的患者为研究对象,随机将其分为对照组和观察组,各26例... 目的探讨舌三针埋针下电刺激联合通督调神电针对结核性脑膜炎(TBM)伴吞咽障碍患者临床症状及炎性反应的影响。方法选取2023年3月至2025年3月广州市胸科医院收治的52例TBM伴吞咽障碍的患者为研究对象,随机将其分为对照组和观察组,各26例。对照组采用TBM基础治疗联合吞咽功能康复治疗,观察组在对照组的基础上采用舌三针埋针下电刺激联合通督调神电针治疗。对比两组的干预效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的电视荧光功能吞咽检查(VFSS)各维度评分明显高于对照组,舌肌表面肌电信号(sEMG)最大波幅值大于对照组,吞咽时程短于对照组,血清高敏C反应蛋白(hs-CRP)、白细胞介素-18(IL-18)及白细胞介素-13(IL-13)水平低于对照组(P<0.05)。结论舌三针埋针下电刺激联合通督调神电针在TBM伴吞咽障碍中的应用效果显著,可增强患者的吞咽功能及舌肌肌力,减轻机体的炎性反应。 展开更多

目的:探究针药结合治疗脑梗死恢复期气虚血瘀证患者的疗效及对血脂代谢、血清同型半胱氨酸(Hcy)的影响。方法:将60例脑梗死恢复期气虚血瘀证患者按随机数字表法分为对照组和观察组,每组各30例。对照组接受通督调神针刺结合抗血小板聚集... 目的:探究针药结合治疗脑梗死恢复期气虚血瘀证患者的疗效及对血脂代谢、血清同型半胱氨酸(Hcy)的影响。方法:将60例脑梗死恢复期气虚血瘀证患者按随机数字表法分为对照组和观察组,每组各30例。对照组接受通督调神针刺结合抗血小板聚集、稳定斑块、活血化瘀和营养脑神经等对症处理及二级预防治疗;观察组在对照组基础上加服小续命汤加减。两组均连续治疗2周。观察比较两组临床疗效、美国国立卫生研究院卒中量表(NIHSS)评分、Barthel指数评分、中医证候评分和Hcy、血脂[低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)]水平。结果:治疗后观察组总有效率90.00%,对照组80.00%,两组疗效比较,差异有统计学意义(P<0.05);治疗后两组NIHSS评分、中医证候评分均较治疗前降低,且观察组低于对照组(P<0.05);两组Barthel指数评分均较治疗前升高,且观察组高于对照组(P<0.05);两组LDL-C、TG、TC、Hcy水平均较治疗前降低,且观察组低于对照组(P<0.05);两组HDL-C水平均较治疗前升高,且观察组高于对照组(P<0.05)。结论:针药结合能改善脑梗死恢复期气虚血瘀证患者肢体功能障碍,提高患者生活质量,降低血脂和Hcy水平。 展开更多

目的探究通督调神针法联合舌三针治疗结核性脑膜炎(tuberculous meningitis,TBM)吞咽障碍的效果,为“通督调神”针法联合舌三针在结核性脑膜炎吞咽障碍诊治中的应用提供临床数据。方法选取广州市胸科医院2023年3月至2024年5月的TBM吞咽... 目的探究通督调神针法联合舌三针治疗结核性脑膜炎(tuberculous meningitis,TBM)吞咽障碍的效果,为“通督调神”针法联合舌三针在结核性脑膜炎吞咽障碍诊治中的应用提供临床数据。方法选取广州市胸科医院2023年3月至2024年5月的TBM吞咽障碍患者进行前瞻性研究,100例患者随机分为观察组和对照组,每组50例,分别予TBM治疗+常规吞咽功能康复治疗+针刺治疗和TBM治疗+常规吞咽功能康复治疗,比较两组治疗前后洼田饮水试验结果、标准吞咽功能评定量表(SSA)评分评价临床疗效,比较治疗前后神经生长因子(NGF)、脑源性神经营养因子(BDNF)、脑脊液压力(CSF)和舌骨下肌群、颏下肌群的表面肌电图平均振幅和持续时间探讨其作用机制。结果以洼田饮水试验结果等级Ⅱ以下为“治愈”标准,观察组和对照组的治愈率分别为90%(45/50)和68%(34/50),观察组远高于对照组(χ^(2)=12.8525,P=0.000337);SSA评分治疗前两组间差异无统计学意义(Z=-0.575,P=0.565),但治疗后SSA评分观察组远低于对照组(Z=-3.330,P=0.001),治疗前后SSA评分差值观察组远大于对照组(Z=-3.339,P=0.001);治疗后BDNF、NGF、CSF压力与治疗前相比,两组间BDNF比值差异无统计学意义(Z=-1.820,P=0.068),NGF比值观察组显著高于对照组(Z=-6.922,P=0.000),CSF压力则反之(Z=-2.735,P=0.006);两种治疗前后,舌骨下肌群、颏下肌群的持续时间均有缩短、平均振幅均有减小,但变化幅度观察组远大于对照组(均P<0.05)。结论通督调神针法联合舌三针治疗TBM吞咽障碍疗效显著,改善颅脑神经作用微环境和恢复吞咽相关肌肉功能可能为其作用机制。 展开更多

目的探讨通督灸联合穴位贴敷干预对多发性骨髓瘤(MM)癌性疼痛患者痛感与生活质量的影响。方法选取2022年6月—2024年6月于医院接受治疗的80例MM癌性疼痛患者为研究对象,按照随机数字表法分为对照组和观察组,各40例。两组患者均给予常规... 目的探讨通督灸联合穴位贴敷干预对多发性骨髓瘤(MM)癌性疼痛患者痛感与生活质量的影响。方法选取2022年6月—2024年6月于医院接受治疗的80例MM癌性疼痛患者为研究对象,按照随机数字表法分为对照组和观察组,各40例。两组患者均给予常规护理干预治疗,对照组给予WHO三阶梯止痛法,观察组在对照组基础上给予通督灸联合穴位贴敷治疗,两组均连续治疗4周。比较两组患者治疗有效率、中医证候积分、疼痛数字评分法(NRS)评分、卡氏评分(KPS)、血清疼痛介质[前列腺素E2(PGE2)、P物质(SP)、5-羟色胺(5-HT)]水平、癌症患者生活质量核心问卷(EORTC QLQ-C30)评分以及不良反应发生率。结果干预后,观察组患者治疗有效率高于对照组,差异有统计学意义(P<0.05)。干预后,观察组患者的中医证候积分、NRS评分、EORTC QLQ-C30的症状评分、SP、5-HT、PGE2水平均低于对照组,差异有统计学意义(P<0.05),KPS评分、EORTC QLQ-C30的功能评分高于对照组,差异有统计学意义(P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论通督灸与穴位贴敷干预对MM癌性疼痛患者的疗效显著,能够有效缓解其中医证候状况,减轻痛感,改善血清疼痛介质水平和提升生活质量水平,且治疗安全性较好。 展开更多

目的 探讨通督调神法针刺结合石冰醒脑颗粒对血管性痴呆(vascular dementia,VaD)大鼠肠道菌群的影响。方法 从48只SD大鼠中随机选取8只作为假手术组,其余大鼠采用改良双侧颈总动脉永久结扎法复制VaD模型,将模型复制成功的32只大鼠采用... 目的 探讨通督调神法针刺结合石冰醒脑颗粒对血管性痴呆(vascular dementia,VaD)大鼠肠道菌群的影响。方法 从48只SD大鼠中随机选取8只作为假手术组,其余大鼠采用改良双侧颈总动脉永久结扎法复制VaD模型,将模型复制成功的32只大鼠采用随机数字表法分为模型组、针刺组、中药组、针药组,每组8只。针刺组大鼠选取“神庭”“百会”“大椎”“至阳”“腰阳关”进行针刺,中药组大鼠予以石冰醒脑颗粒灌胃持续治疗4周,针药组大鼠予以针刺结合石冰醒脑颗粒治疗,假手术组、模型组大鼠不予任何干预与治疗。采用Morris水迷宫实验对大鼠的认知能力进行评估,苏木精—伊红染色检测大鼠海马组织病理形态变化,16S rDNA高通量测序法分析大鼠肠道菌群。结果 针刺组、中药组及针药组大鼠较假手术组逃避潜伏期显著缩短(P<0.05),穿越平台次数显著增加,学习和记忆能力显著改善(P<0.05),海马神经元损伤减少;针刺组、中药组及针药组大鼠肠道菌群丰度异常及群落多样性减少的情况改善;各组大鼠肠道菌群在门、科、属水平上的物种丰度不同,差异菌不同。结论 通督调神法针刺结合石冰醒脑颗粒可调节VaD大鼠肠道微生物群的多样性和组成,从而改善VaD大鼠的认知功能。 展开更多

目的比较调脊通督针灸法联合Bobath技术训练与单纯Bobath技术治疗脑卒中后下肢运动障碍的临床疗效。方法将60例脑卒中后下肢运动功能障碍患者随机分为观察组和对照组,每组30例,两组均给予基础治疗和Bobath技术训练,观察组联合调脊通督... 目的比较调脊通督针灸法联合Bobath技术训练与单纯Bobath技术治疗脑卒中后下肢运动障碍的临床疗效。方法将60例脑卒中后下肢运动功能障碍患者随机分为观察组和对照组,每组30例,两组均给予基础治疗和Bobath技术训练,观察组联合调脊通督针灸法,共治疗4周。采用Fugl-Meyer量表(Fugl-Meyer assessment,FMA)下肢部分、Berg平衡量表(Berg balance scale,BBS)、改良Barthel指数(modified Barthel index,MBI)、6分钟步行试验(6-minute walk test,6MWT)评估患者的运动和平衡能力,并基于FMA下肢部分评分判定临床疗效。结果治疗4周后,两组患者FMA下肢部分评分、BBS评分、MBI、6MWT距离均较治疗前显著升高(P<0.05),且观察组上述指标升高程度显著大于对照组(P<0.05);观察组基于FMA下肢部分评分的临床疗效显著优于对照组(P<0.05)。结论在Bobath技术训练基础上联合调脊通督针灸法可以明显提高脑卒中后下肢运动障碍患者的下肢运动能力及生活自理能力。 展开更多

目的:观察通督调神针法联合银杏二萜内酯葡胺注射液治疗脑梗死的临床疗效及对血清NOD样受体热蛋白结构域相关蛋白3(NLRP3)、白细胞介素-1β(IL-1β)的影响。方法:选取安徽中医药大学第二附属医院2021年1月—2023年1月收治的96例脑梗死病... 目的:观察通督调神针法联合银杏二萜内酯葡胺注射液治疗脑梗死的临床疗效及对血清NOD样受体热蛋白结构域相关蛋白3(NLRP3)、白细胞介素-1β(IL-1β)的影响。方法:选取安徽中医药大学第二附属医院2021年1月—2023年1月收治的96例脑梗死病人,采用随机数字表法将其分为观察组和对照组,各48例。对照组采用银杏二萜内酯葡胺注射液治疗,观察组在对照组基础上联合通都调神针法,两组连续治疗2周后评估疗效。观察两组治疗前后中医证候积分、脑血流动力学指标[患侧大脑中动脉平均血流量(Qm)、平均血流速度(Vm)、脑血管阻力指数(RI)]、血清炎性因子及不良反应情况。结果:观察组治疗总有效率高于对照组(97.92%与85.42%,P<0.05)。治疗后,观察组半身不遂、口眼歪斜、言语不利、头晕呕吐、神疲乏力中医证候积分均低于对照组(P<0.05);治疗后,观察组Qm、Vm均高于对照组(P<0.05),RI值低于对照组(P<0.05);治疗后,观察组血清NLRP3及IL-1β水平低于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(12.5%与8.3%,P>0.05)。结论:通督调神针法联合银杏二萜内酯葡胺注射液治疗脑梗死病人,可促进神经功能恢复,减轻炎症反应,提高临床疗效,且安全性较好。 展开更多

中医针刺联合疗法对脊髓损伤后神经源性膀胱的疗效及尿流动力学的影响

[目的]分析温肾通督方联合针刺对脊髓损伤(SCI)后神经源性膀胱(NB)的疗效及对尿流动力学的影响。[方法]回顾性选取2021年1月—2023年11月在四川省康复医院脊髓损伤康复科收治的95例不完全性脊髓损伤(ISCI)后NB患者作为研究对象,根据治疗方案分为对照组(n=45)和针药组(n=50)。对照组给予常规的西医对症支持治疗,针药组在对照组的基础上使用温肾通督方联合针刺进行治疗。对比两组患者的尿流动力学指标、中医证候积分、排尿情况、膀胱功能积分、临床疗效和生活质量。[结果]针药组治疗12周的总有效率(92.00%)显著高于对照组的总有效率(75.56%)(P<0.05)。两组膀胱顺应性(BC)、最大尿流速率(Qmax)、最大尿流速率时逼尿肌压力(Pdet Qmax)在不同时间点之间存在差异(F_(时间)=505.288、54.107、221.903,P_(时间)<0.001),在不同组别之间存在差异(F_(组间)=47.726、12.082、40.533,P_(组间)<0.001),变化趋势存在差异(F_(交互)=27.398、3.644、13.266,P_(交互)<0.05)。中医证候积分在不同时间点之间存在差异(F_(时间)=326.605,P_(时间)<0.001),在不同组别之间存在差异(F_(组间)=59.029,P_(组间)<0.001),变化趋势存在差异(F_(交互)=17.842,P_(交互)<0.001)。两组日均单次排尿量、日均排尿次数、日均导尿次数在不同时间点之间存在差异(F_(时间)=376.083、1619.355、215.564,P_(时间)<0.001),在不同组别之间存在差异(F_(组间)=52.573、252.387、51.996,P_(组间)<0.001),变化趋势存在差异(F_(交互)=81.412、137.974、24.434,P_(交互)<0.001)。两组膀胱功能积分、I-QOL评分在不同时间点之间存在差异(F_(时间)=2672.823、202.110,均P_(时间)<0.001),在不同组别之间存在差异(F_(组间)=141.178、22.778,P_(组间)<0.001),变化趋势存在差异(F_(交互)=46.809、6.840,P_(交互)<0.001)。[结论]温肾通督方联合针刺辅助治疗ISCI后NB患者具有良好的临床疗效,增强膀胱的顺应性,改善排尿功能,提高预后生活质量水平。 展开更多

目的探究通督醒神针刺对缺血性脑卒中(Ischemic stroke,IS)后认知障碍患者的临床疗效、全身炎症反应、血脑屏障及肠道菌群的影响及其作用机制。方法30例(脱落3例)IS后认知障碍患者为疾病组,其中治疗前的患者作为疾病组,通督醒神针刺治... 目的探究通督醒神针刺对缺血性脑卒中(Ischemic stroke,IS)后认知障碍患者的临床疗效、全身炎症反应、血脑屏障及肠道菌群的影响及其作用机制。方法30例(脱落3例)IS后认知障碍患者为疾病组,其中治疗前的患者作为疾病组,通督醒神针刺治疗后的患者作为电针组,同时30例(脱落3例)健康对照者为健康组。电针组为在基础治疗基础上,行通督醒神针刺干预,每天干预1次,每次30 min,共干预14天。分别观察三组MMSE、MoCA和MBI评分;收集所有研究对象的粪便样本和血清样本,使用16S rDNA测序技术和ELISA法检测粪便中的肠道菌群变化及血清中促炎因子IL-6、IL-1β、TNF-α和血脑屏障功能破坏的相关因子S100β的含量变化。结果与健康组比较,疾病组患者的MMSE、MoCA、MBI评分显著降低(P<0.05),血清中促炎因子及S100β蛋白含量显著升高(P<0.05),Shannon指数(P<0.01)和Simpson指数(P<0.001)显著升高;而与疾病组比较,电针组患者的MMSE、MoCA、MBI评分显著升高(P<0.05),血清中促炎因子及S100β含量显著降低(P<0.05),Shannon指数和Simpson指数均有所下降(P>0.05)。健康组中的优势菌群主要包括拟杆菌门、双歧杆菌属、拟杆菌属、粪杆菌属、双歧杆菌科、瘤胃球菌科、拟杆菌科等有益菌(P<0.05),疾病组患者中的优势菌群包括变形菌门、肠杆菌科、埃希氏杆菌属、克雷伯杆菌属等条件致病菌(P<0.05),而电针组患者体内优势菌群与健康组一致,有益菌相对丰度水平显著升高(P<0.05),条件致病菌相对丰度显著降低(P<0.05)。Spearman相关性分析发现有益菌相对丰度水平与临床疗效相关指标呈正相关,而与血清中促炎因子及S100β含量呈负相关。结论通督醒神针刺可通过调节肠道菌群多样性提升拟杆菌属、双歧杆菌属、粪杆菌属等有益菌丰度水平,调控肠道微生态平衡,从而调控全身炎症及血脑屏障功能,进而发挥改善认知功能的作用。 展开更多
