摘要
目的探讨舌三针埋针下电刺激联合通督调神电针对结核性脑膜炎(TBM)伴吞咽障碍患者临床症状及炎性反应的影响。方法选取2023年3月至2025年3月广州市胸科医院收治的52例TBM伴吞咽障碍的患者为研究对象,随机将其分为对照组和观察组,各26例。对照组采用TBM基础治疗联合吞咽功能康复治疗,观察组在对照组的基础上采用舌三针埋针下电刺激联合通督调神电针治疗。对比两组的干预效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的电视荧光功能吞咽检查(VFSS)各维度评分明显高于对照组,舌肌表面肌电信号(sEMG)最大波幅值大于对照组,吞咽时程短于对照组,血清高敏C反应蛋白(hs-CRP)、白细胞介素-18(IL-18)及白细胞介素-13(IL-13)水平低于对照组(P<0.05)。结论舌三针埋针下电刺激联合通督调神电针在TBM伴吞咽障碍中的应用效果显著,可增强患者的吞咽功能及舌肌肌力,减轻机体的炎性反应。
Objective To explore the effects of tongue three needle buried needle electrical stimulation combined with Tongdu Tiaoshen electroacupuncture on clinical symptom and inflammatory response in patients with tuberculous meningitis(TBM)accompanied by swallowing disorders.Methods A total of 52 TBM patients with swallowing disorders admitted in Guangzhou Chest Hospital from March 2023 to March 2025 were selected as the research objects and randomly divided into control group and observation group,with 26 cases in each group.The control group was treated with basic treatment of TBM combined with swallowing function rehabilitation.On the basis of the control group,the observation group was treated with tongue three needle buried needle electrical stimulation combined with Tongdu Tiaoshen electroacupuncture.The intervention effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the scores of each dimension of video fluoroscopic swallowing study(VFSS)of the observation group were significantly higher than those of the control group,the maximum amplitude of the surface electromyography(sEMG)of the tongue muscle was greater than that of the control group,the swallowing time was shorter than that of the control group,and the levels of serum hypersensitive C-reactive protein(hs-CRP),interleukin-18(IL-18)and interleukin-13(IL-13)were lower than those of the control group(P<0.05).Conclusion The tongue three needle buried needle electrical stimulation combined with Tongdu Tiaoshen electroacupuncture in TBM with swallowing disorders has significant application effect,which can enhance the swallowing function and tongue muscle strength of patients,and reduce the inflammatory response of the body.
