目的评估新辅助化疗(NACT)治疗中晚期子宫内膜癌(EC)的疗效及比较NACT和初始肿瘤细胞减灭术(PCS)治疗结果。方法回顾性分析2010年1月至2018年12月在天津医科大学总医院妇科诊治的100例EC患者的临床资料,其中NACT组21例,PCS组79例。结果N...目的评估新辅助化疗(NACT)治疗中晚期子宫内膜癌(EC)的疗效及比较NACT和初始肿瘤细胞减灭术(PCS)治疗结果。方法回顾性分析2010年1月至2018年12月在天津医科大学总医院妇科诊治的100例EC患者的临床资料,其中NACT组21例,PCS组79例。结果NACT组总化疗有效率为61.9%;与PCS组相比,NACT组患者的术中失血量少(214.29 ml vs392.78 ml,P=0.027)、术后并发症低(19.0%vs45.5%,P=0.026)、平均住院时间更短(中位时间17.53 d vs24.55 d,P=0.001)、胃肠道毒副反应发生率低(52.38%vs93.44%,P<0.001),而平均手术时间、腹水量、是否输血、最佳减瘤手术率以及术后住院时间差异均无统计学意义(P>0.05)。此外,Kaplan-Meier显示两组PFS及OS差异均无统计学意义(49.01个月vs 75.02个月;P=0.601)及(67.86个月vs72.40个月;P=0.351)。结论对于术前评估不能达到满意的肿瘤细胞减灭术或者合并症多,体能状态较差的中晚期EC患者,NACT可作为一种有价值的替代治疗。展开更多
目的:探讨窄带成像(narrow band imaging,NBI)辅助白光经尿道膀胱肿瘤切除术(transurethral resection of bladder tumor,TURBT)治疗膀胱尿路上皮癌的效果,总结NBI手术经验。方法:选择2018年4月至2020年1月北京大学第三医院泌尿外科就...目的:探讨窄带成像(narrow band imaging,NBI)辅助白光经尿道膀胱肿瘤切除术(transurethral resection of bladder tumor,TURBT)治疗膀胱尿路上皮癌的效果,总结NBI手术经验。方法:选择2018年4月至2020年1月北京大学第三医院泌尿外科就诊膀胱尿路上皮癌的患者,麻醉后行TURBT。术中先在白光下寻找膀胱肿瘤并切除,完整取出病理标本。换成NBI,寻找可疑病灶并切除,将病理标本完整取出。两组术后标本区分开分别送病理科,记录白光下病灶的数目、位置及病理诊断结果,同时记录NBI下白光残留病灶的情况。计算白光下电切后膀胱肿瘤残留的比例,评估NBI的效果。后期病例按时间顺序平均分为3组,统计各组的临床数据,观察NBI辅助白光下TURBT的学习曲线。结果:参加研究的患者共45例,其中男32例,女13例,年龄23~89岁,平均65.2岁。45例手术均顺利完成,术后未见明显并发症及发生。术中发现9例为单发,36例为多发,肿瘤最大径0.5~4.0 cm,平均2.2 cm。白光下切除标本病理诊断均为尿路上皮癌,另有19例白光切除后NBI辅助下再次切除标本病理诊断阳性(42.2%)。按时间顺序将45例分为3组,每组15例,白光下切除后3组NBI的真阳性率分别为33.3%、46.7%、46.7%,假阳性率分别为60.0%、46.7%、26.7%。结论:TURBT是治疗膀胱尿路上皮癌的有效方式,NBI是白光的有效补充,能增加膀胱肿瘤的检出率,减少术后复发。NBI光源有一定的学习曲线,随着病例的增加,NBI下假阳性率逐渐降低,而术者NBI经验丰富后,在白光下对扁平样肿瘤的辨识度也逐渐提高,白光下肿瘤切除后NBI辅助治疗肿瘤残留率降低。展开更多

Aiming at the leakage problem in the compact sandstone drilling of the Keziluoyi Formation in South-west Tarim,Nano-core-emulsion was prepared by coating modified nano-SiO_(2) with nano-emulsion,its particle size D50 ...Aiming at the leakage problem in the compact sandstone drilling of the Keziluoyi Formation in South-west Tarim,Nano-core-emulsion was prepared by coating modified nano-SiO_(2) with nano-emulsion,its particle size D50 is about 100 nm,with good dispersion stability.When 0.8%Nano-core-emulsion is added to 5%bentonite slurry,the fluid loss can be reduced by 40%,and the filter cake thickness can be reduced by 84%.Using a Nano-core-emulsion to optimize the plugging performance of potassium pol-ysulfonate drilling fluid can reduce the fluid loss of the drilling fluid by 52%,the resulting filter cake is dense and tough,and the thickness is reduced by 40%.Using the pressure conduction method to evaluate the plugging rate,the plugging rate of the drilling fluid of the Nano-core-emulsion on the core of the Keziluoyi Formation is 63.4%,which is 20.9%higher than that of the field drilling fluid.According to microscopic examination and CT scanning analysis,the material has the plugging characteristics of"inner rigid support+outer soft deformation"and has demonstrated good field application results.展开更多
