摘要
目的 :探讨原发性高血压患者主动脉根部顺应性对冠状动脉血流储备功能的影响。方法 :选择冠状动脉造影正常的原发性高血压患者 5 5例 (原发性高血压组 ) ,其中主动脉根部内径 >3 5cm者 2 7例 ,主动脉根部内径≤ 3 5cm者 2 8例 ;血压正常患者 2 3例 (血压正常组 )。用食道超声检测主动脉根部扩张度 (D值 )和僵硬度 (β值 ) ,同时检测冠状动脉血流储备。结果 :原发性高血压组D值为 (2 87± 0 76) 10 -6cm2 /dyn ,β值为 5 3 4± 2 84;血压正常组D值为 (3 47± 0 95 ) 10 -6cm2 /dyn ,β值为 3 63± 1 67,两者比较均有显著性差异 (P <0 0 0 1和 <0 0 5 )。主动脉根部内径 >3 5cm者D值为(2 3 9± 0 84) 10 -6cm2 /dyn ,β值为 6 5 3± 2 62 ;主动脉根部内径≤ 3 5cm者D值为 (2 94± 0 71) 10 -6cm2 /dyn ,β值为5 12± 2 46,两者比较均有显著性差异 (P <0 0 5 )。原发性高血压组患者D值与冠状动脉血流储备呈显著正相关 (r =0 3 72 ,P <0 0 0 5 ) ;β值与冠状动脉血流储备呈显著负相关 (r =-0 3 95 ,P <0 0 0 5 ) ;原发性高血压组患者主动脉根部内径与 β值呈显著正相关 (r =0 3 3 0 ,P <0 0 5 )。结论 :原发性高血压患者存在不同程度的主动脉根部顺应性减退 。
Objective:To study the effect of aortic root compliance on the coronary blood flow reserve(CFR)in patients with essential hypertension. Methods:Twenty-three patients without hypertension (group B)and 55 patients with hypertension (group A)were divided in to group A_1(>3.5 cm in inner diameter of aortic root)and group A_2(≤3.5 cm in inner diameter of aortic root). The dilatation (D),stiffness(β)and CFR were measured by transesophageal echocardiography(TEE). Results:There was significant difference between hypertension and control groups in D and β(patients with hypertension: D= 2.87±0.76(10 -6 cm 2/dyn),β=5.34±2.84;group B:D=3.47±0.95(10 -6 cm 2/dyn),β=3.63±1.67, p <0.001, p <0.05). There was significant difference between group A_1 and group A_2 in D and β(group A_1:D=2.39±0.84(10 -6 cm 2/dyn),β=6.53±2.62;group A_2:D=2.94±0.71[10 -6 cm 2/dyn],β=5.12±2.46,p <0.05).CFR was significantly related to D ( r = 0.372,p <0.005)and was negatively related to β( r =-0.395,p <0.005)in patients with hypertension.The inner diameter of aortic root was significantly related to β( r=0.330,p <0.05). Conclusions:The compliance of aortic root was significantly decreased in the patients with essential hypertension.It also reduced CFR.
