声学造影与多普勒诊断移植肾急性肾小管坏死的实验研究 被引量:3

摘要 目的:探讨应用声学造影时间-强度曲线(TIC)诊断移植肾急性肾小管坏死的可行性,并与常规二维及多普勒超声检查进行对比分析。方法:建立移植肾急性肾小管坏死(ATN)动物模型,采用自身对照法,进行常规二维及多普勒超声检查、经静脉间断触发超谐波声学造影(UHI)及声学定量分析TIC各参数,同时跟踪病理活检。结果:ATN发生前后对比发现:声学造影示TIC曲线峰值前移,TIC参数除PI值外,AUC、HT、MTT、PI与AUC比率均明显降低;二维超声和多普勒检查示:移植肾大小无明显改变,RI值在正常值范围波动。结论:声学造影结合TIC定量分析,可客观反映移植肾ATN血流灌注变化,为诊断移植肾ATN提供了可行的新方法。 Objective: To investigate the feasibility of diagnostic acute tubular necrosis(ATN) in renal allograft using time-intensity curves(TIC) by intravenous contrast enhanced ultrasonography. Methods: The canine models of acute tubular necrosis in renal allograft were established, and examined subsequently with two dimensional and color Doppler flow imaging and intravenous contrast ultrasonography using auto-control, and also analyzed the parameters of time-intensity curves, correlated with renal biopsies simultaneously. Results: Through pre- and post-ATN comparison, we detected that the curve peak was ante-displaced anteriorly and parameters of TIC(except PI), AUC, HT, MTT, ratio of PI to AUC were decreased dramatically, no changes were found on 2D and CDFI. Conclusion: Contrast ultrasonography combined with TIC quantitative analysis could reflect the variation of blood perfusion objectively, this may offer a new method for the diagnosis of ATN in renal allograft.
