摘要
目的 研究小梁切除术中辅助应用全氟丙烷 (C3 F8)气体后滤过道的组织病理学变化。方法 对 2 0只新西兰兔实施小梁切除术。随机分为两组 ,每组各 10只兔 (2 0只眼 )。第 1组 :随机选择 1只眼做单纯小梁切除术 ,另 1只眼做小梁切除术并辅助应用C3 F8气体 ;第 2组 :随机选择 1只眼做小梁切除术联合应用丝裂霉素C(MMC) ,另 1只眼做小梁切除术并辅助应用C3 F8气体。采用组织病理学和免疫组化技术 ,对兔眼术后 3d ,1、2、3、4周不同时期的滤过泡组织进行病理检查 ,观察成纤维细胞、新生胶原纤维、新生血管、炎性细胞的改变。结果 应用C3 F8气体组与单纯小梁切除术组术后滤过道新生胶原纤维量、成纤维细胞量差异均有显著意义 (P <0 0 5 ) ;应用MMC组与单纯小梁切除术组术后滤过道新生胶原纤维量、成纤维细胞量差异均有显著意义 (P <0 0 5 ) ;应用C3 F8气体组与MMC组术后滤过道新生胶原纤维量、成纤维细胞量差异无显著意义 (P >0 0 5 )。 3种不同术式兔眼术后滤过道的新生血管量与术后 5个不同时期滤过道的炎性细胞量进行比较 ,差异均无显著意义 (P >0 0 5 )。结论 小梁切除术中辅助应用C3 F8气体可以在术后早期抑制成纤维细胞增殖和新生胶原纤维的合成 ,抑制或减轻术后滤过道的瘢痕化 ,从而提高手术的?
Objective To study the histopathologic changes of filtering blebs by subconjunctival application of perfluoropropane (C 3F 8) after trabeculectomy in rabbit eyes. Methods Twenty rabbits were randomly divided into two groups, each group consisting of 10 rabbits. Trabeculectomy with topical application of C 3F 8 was performed on one eye and trabeculectomy alone on the other eye in first group. Whereas, trabeculectomy with topical application of mitomycine (MMC) was performed on one eye and trabeculectomy with topical application of C 3F 8 on the other eye in second group. Histopathologic changes in filter bleb were investigated by immunostaining at 3 days, and at week 1, 2, 3, and 4 after surgery. Results Fibroblasts: The numbers of fibroblasts in eyes after trabeculectomy with C 3F 8 gas as well as trabeculectomy with MMC were significantly reduced (P<0.05) compared to eyes with trabeculectomy alone. There was no significant statistical difference in the numbers of fibroblasts between trabeculectomy with C 3F 8 gas group and trabeculectomy with MMC group. Newly formed collagen fiber: Immunostaining of collagen fibers showed that the numbers of newly formed collagen fibers in trabeculectomy with C 3F 8 gas group and trabeculectomy with MMC group were significantly decreased compared to trabeculectomy alone group. However, there was no significant statistical difference between numbers of fibers in trabeculectomy with C 3F 8 gas group and trabeculectomy with MMC group. Neovascularization and inflammation of filtering blebs: The differences were not statistically significant among trabeculectomy with C 3F 8 gas, trabeculectomy with MMC group and trabeculectomy alone group. Conclusion Trabeculectomy with subconjunctival retention of C 3F 8 gas can enhance the success rates of surgery by suppressing the growth of fibroblasts and synthesis of newly formed collagen fibers, and by inhibiting scar formation.
