摘要
目的探讨腭裂修复术手术方法及技巧对腭裂患者治疗效果的影响。方法对97例先天性腭裂患者进行手术治疗,根据腭裂的不同类型选择不同术式,对手术方法及技巧进行总结,探讨腭裂修复术式的选择和其术后并发症的防治方法。结果97例先天性腭裂患者,92例伤口Ⅰ期愈合,2例软腭穿孔,3例悬雍垂不全裂开。术后复诊的68位患者,语音恢复效果满意者26例,明显改善14例,语音不良28例。结论手术方法以两瓣法为首选,软硬腭交界处鼻腔粘膜均应切断,手术操做应轻柔,不仅要关闭裂隙,还应使软腭达到足够的长度和动度,重建良好"腭咽闭合",术中止血应彻底,术后应预防伤口感染和加强护理。出院后应积极进行语音训练。
Objective To study the effects of the operation type and experience on the result of cleft palate patients' treatment. Methods 97 CP patients accepted different cleft repair operation accoding to their individual deformity.The results of the treatment including complication and speech were assessed,and the methods to avoid application and how to get clear speech was summarized. Results In 5 among the 97 patients, perforation or resplitting were accured. In the 68 call-back patients,26 of them have very good speech,and 14 with significant improvement, while 28 still without articulation progress. Conclusion It is recommended that two-flap method in the repair of cleft palate. Reconstruction of velopharyngeal closure is very important in the speech recovery of the patients, and the postoperative speech therapy is of utmost importance.
