摘要
目的 探讨窒息新生儿血清心肌酶水平的变化及临床意义。方法 将 6 9例窒息新生儿根据窒息程度分为轻度窒息组 (31例 )和重度窒息组 (38例 ) ,分别于入院时和住院第 10d测定血清心肌酶水平和进行心电图 (ECG)检查 ,并与 30例正常儿童 (对照组 )进行比较分析。结果 入院时轻度窒息组和重度窒息组患儿与对照组正常儿的血清肌酸激酶 (CK)、肌酸激酶心型同工酶 (CK -MB)、乳酸脱氢酶 (LDH)、α-羟丁酸脱氢酶 (α -HBDH)和谷丙转氨酶 (AST)的均值间差别均有显著性意义 (P <0 0 1) ,重度窒息组与轻度窒息组患儿的CK -MB和α -HBDH的均值间差别亦有显著性意义 (P均 <0 0 1) ;ECG异常 4 1例 ,其中轻度窒息组 14例 ,重度窒息组 2 7例。 10d后复查 ,重度窒息组 (2 9例 )CK、CK -MB和AST降至正常水平 ,但LDH和α -HBDH与对照组均值间差别有显著性意义(P <0 0 1) ,重度窒息组与轻度窒息组LDH均值间差别有显著性意义 (P <0 0 5 ) ;轻度窒息组CK、CK -MB、α -HBDH和AST降至正常水平 ,LDH与对照组均值间差别有显著性意义 (P <0 0 5 ) ;ECG复查异常 5例 ,均为重度窒息组患儿。结论 窒息新生儿早期即有心肌损害 ,心肌酶水平测定与ECG一并检查 ,对判断心肌损害及程度有重要价值。
Objective To explore the change of serum myocardial enzyme level and its clinical significance in asphyxia newborn.Methods 69 cases of asphyxia newborn were classified by the degree of asphyxia into mild asphyxia group (31 cases ) and severe asphyxia group (38 cases) whose serum myocardial enzyme and electrocardiography (ECG) were tested when hospitalized and at the tenth day after hospitalization.Contrasted with those of 30 healthy newborn,the results were analyzed.Results The mean values of Serum creatine kinase (CK),creatine kinase and myocardial myoglobin isoenzyme (CK-MB),lactate dehydrogenase (LDH),α-hydro-oxybutryrate dehydrogenase (α-HBDH ) and aspirate transminase (AST) in the mild group,the severe group as well as the controlled group tested when hospitalized had significantly different mean values (P<0 01).The mean values of CK-MB and α-HB-DH in the severe group and the mild group were significantly different (P<0 01).There were 41 cases with ECG abnormality,14 in the mild group and 27 in the severe group.Retest at the tenth day showed that CK,CK-MB and AST in the severe group(29 cases) went down to the normal level,while the mean values of LDH and α-HBDH were markedly different from those in the controlled group (P<0 01).LDH in the severe and mild group showed marked difference (P<0 05).CK,CK-MB,α-HBDH and AST in the mild group declined to normal while LDH was still highly different from that in the controlled group (P<0 05 ).5 cases were abnormal with ECG found in the severe group.Conclusion Newborn with early asphyxia would have myocardial injury so that myocardial enzyme level test together with ECG examination could have great values in diagnosis of myocardial injury as well as its degree.
