大黄对大鼠急性出血性胰腺炎的影响 被引量:54

Effect of rhubarb on acute hemorrhagic pancreatitis in rats

摘要 目的 观察中药大黄对大鼠急性出血性胰腺炎的作用以及对胰腺外分泌功能的影响。方法 采用铃蟾肽加水浸束缚应激的方法诱导大鼠急性出血性胰腺炎 ,经口灌服大黄醇抽提物 75~ 150mg/kg 2次。观察血清淀粉酶、胰腺湿重、胰腺血流量及胰腺组织病理改变 ;并观察大黄与奥曲肽对胰液外分泌功能的影响。结果 大黄治疗组的胰腺坏死和中性粒细胞浸润明显减轻 ;胰腺湿重、血清淀粉酶活性和胰腺血流量均有剂量依赖性改善。胰腺炎组大鼠胰液流量、胰液蛋白含量均明显下降 ,胰液碳酸氢根含量没有明显改变 ,大黄治疗组胰液淀粉酶活性明显降低 ,与奥曲肽治疗组比较无统计学差异 ,其胰液碳酸氢根含量明显升高 ,奥曲肽则无此作用。结论 大黄可有效改善大鼠急性出血性胰腺炎的严重程度 ,与抑制胰腺炎性反应、改善胰腺血流量和抑制胰酶分泌。 Objective To investigate the effect of rhu barb on acute hemorrhagic pancreatitis (AHP) in rats. Methods AHP was induced by 5 h restraint water-immersion stress plus two intraperitoneal injections of cerulein (Cn, 40 μg/kg) at 1 h interval. Rhubarb (Rb, 75-150 mg/kg) was orally fed twice at 2 h and 15 h before the fir st cerulein injection. The pancreas wet weight (PWW), serum amylase (sAmy) level s, pancreatic blood flow(PBF) and histological alteration were evaluated. The ef fect of Rb and somatostatin on pancreatic exocrine secretion in the same models were also eval uated. Results In the controls, PWW was ( 3.29 ±0.12) g /kg, sAmy w as (8136±220) U/dl. Rb alone did not induce any significant changes comparing wi th the normal rats. In Cn+stress group, PWW (11.60 g/kg±0.61 g/kg) and s Amy (45 849 U/dl±4310 U/dl) increased significantly ( P <0.01 vs. control) . sAmy was sign ificantly reduced in the Cn+stress+Rb group (29 865 U/dl±3645 U/dl). The PBF d ropped to 38%(93 ml·min -1 ·100 g -1 ± 5 ml·mim -1 ·100 g - 1 ) of the control value at 5 h, an d partly recovered in the Cn+stress+Rb group (135 ml·min -1 ·100 g -1 ± 12 ml·mim -1 ·100 g -1 ). Histologic observation showed that necrosis and polymorphonuclear neutrophlil s infiltration of pancreas were signif icantly reduced in the rats treated with Rb. Rb and somatostatin parti ally affected the pancreatic exocrine function which was impaired in rats with A HP. Conclusions It is concluded that Rb was effective in the treat m ent of hemorrhagic pancreatitis under our experimental conditions, probably thro ugh inhibition of pancreatic inflammation and improvement of the pancreatic micr o-circulation and exocrine secretion.
