摘要
目的 探讨L -精氨酸对犬离体胰腺低温灌注及保存中缺血再灌注损伤的影响。方法 将犬胰节段移植模型动物随机分为 3组 :(1)L -精氨酸灌注保存组 (L -arg组 ) ,在EuroCollin(EC )液中加用L -精氨酸 ;(2 )硝基精氨酸组 ,在EC液中加入N -硝基 -L -精氨酸甲酯 (L NAME) ;(3 )对照组 ,在EC液中加入生理盐水 ;分别对犬离体胰腺节段进行低温灌注保存。测定移植后犬血清中脂肪酶和淀粉酶含量 ,并测定移植胰腺组织中髓过氧化酶 (MPO )活性、一氧化氮 (NO )含量、一氧化氮合酶 (NOS)及iNOSmRNA的表达 ;同时进行组织学观察。结果 血清脂肪酶含量 :L arg组 <对照组 <L NAME组 ,差异均具显著性 (均P <0 .0 5 )。移植胰腺组织中NO含量 :L arg组 >对照组 >L NAME组。NOS活性 :L arg组 >对照组 >L NAME组。iNOSmRNA的表达 :L arg组 >对照组 >L NAME组。MPO活性 :L arg组 <对照组 <L NAME组 ,差异均具显著性 (均P <0 .0 5 )。L arg组胰小叶间质水肿和胰小叶内中性粒细胞浸润较L NAME组和对照组轻。结论 EC保存液中加入L arg可提高移植胰腺组织中NO含量、NOS活性及iNOSmRNA的表达 ,抑制中性粒细胞浸润 ,提示L arg可通过L arg -NO途径可对移植胰腺再灌注损伤起保护作用。
Objective To study the effects of L-arginine on the reperfusion injury of hypothermic perfusion and on preservation of isolated pancreas(ISP) in canines . Methods After segmental pancreatic transplantation(SPTS) model were set up,the animals were randomly divided into 3 groups:(1)ISP irregulated and preserved with Euro Colln Solution(ECS)plus L-arginine(L-arg group);(2)IPS irregulated and preserved with ECS plus N-nitro-L-arginine methy ester(L-NAME group);and(3)IPS irregulated and preserved with ECS plus normal saline(Control group).The IPS were irregulated and preserved at 0-4℃ for 24h.Serum concentration of amylase and lipase were detected. NO,NOS and myeloperoxidase (MPO) and expression of iNOS mRNA in the pancreas graft were measured. Histological examination was performed. Results The level of NO in the L-arg group was significantly higher than that in the control group(P< 0.05),and in L-NAME group was lower than that in control group(P< 0.05). The activity of NOS in L-arg group was significantly higher than that in the control group(P< 0.05),while in the L-NAME group was significantly lower than that in control group(P<0.05). The expression of iNOS mRNA in L-arg group was significantly higher than that in the control group,and in the L-NAME group was significantly lower than that in control group(P<0.05). The activity of MPO in the L-arg group was obviously lower than the in the control group,while the L-NAME group was significantly higher than that in control group(P<0.05). The pancrease injury in L-arg group was much milder that that in L-NAME group.Conclusions L-arg can increase the NO level, NOS activity and iNOS mRNA expression,and inhibit the neutrophil infiltration in the transplanted pancreas.The results suggest that L-arg has protective effects on ISP against the ischemia/reperfusion injury through L-arg-NO pathway.
