摘要
目的 探讨X刀治疗继发性癫痫 (SEp)和原发性颞叶癫痫 (ITLEp)的方法。方法 使用 2 4hEEG、CT、MRI对继发性癫痫灶定位 ,应用X刀损毁致痫灶治疗SEp 4 0例 ;毁损剂量 2 0～ 30Gy。应用X刀毁损杏仁核治疗ITLEp病人 30例 ;毁损剂量 14 0Gy ;准直仪 7～ 10mm ;采用 14 0Gy一次和分两次毁损治疗 (每次间隔 2 4h)。结果 4 0例SEp病人和 30例ITLEp病人X刀治疗后 4～ 2 4个月随访 :5 0例病人癫痫发作完全停止 (SEp 32例 ,ITLEp 18例 ) ;2 0例病人发作减少 5 0 %以上 ;无并发症发生。结论 应用X刀毁损SEp致痫灶及杏仁核 ,可安全有效地治疗SEp和ITLEp。
Objective To investigate the curative effect of X knife treatment method for secondary epilepsy (SEp) and idiopathic temporal lobe epilepsy (ITLEp). Methods The 24-hour EEG,CT,MRI were used for locating the epileptogenic lesion of SEp.Forty patients of SEp were treated to destroy epileptogenic lesion by X knife with dose,20-30 Gy.Thirty patients of ITLEp were treated with X knife for amygdalectomy at 140 Gy and collimator 7-10 mm.The 140 Gy were given either by single full dose or 2-time fractionated treatment (Q.24.h). Results By 4-24 month follow-up of all these patients it was showed that seizures were alleviated completly in 50 patients (SEp 32 cases,ITLEp 18 cases),alleviated over 50% in 20 patients (SEp 8 cases,ITLEp 12 cases).No complications were observed. Conclusion X knife treatment of SEp is a useful and safe method to destroy epileptogenic lesion in patients with SEp and to perform amygdalectomy in patients with ITLEp.This methods is thus proved to be effective for such patients. ;
