摘要
目的 :评价腹腔转移瘤CT模拟定位立体定向适形放疗的剂量分布优势。方法 :选择腹腔转移瘤病人共 38例 ,在CT定位并确定计划靶区 (planningtargetvolume,PTV)后 ,采用三维适形治疗计划系统 (TPS) ,以尽量小的治疗区 (treatedvolume,TV)完全包绕PTV区的原则 ,设计适形野照射计划 ,利用TPS的剂量、体积计算功能评价本组病例适形野立体定向放射治疗的剂量分布特点和优势。结果 :38例患者PTV内平均最大剂量为 ( 115 .30± 5 .2 3) % ,最小剂量为 ( 88.0 0±2 32 ) % ,平均剂量为 ( 99.2 0± 1.2 0 ) % ;PTV区相应水平膀胱、肾脏、脊髓、肝脏等邻近正常重要器官的平均最大剂量分别为 ( 6 5 .4 5± 2 .2 3) %、( 35 .2 2± 1.32 ) %、( 6 0 .32± 1.13) %、( 2 6 .32± 2 .5 7) % ;平均剂量分别为 ( 30 .2 0± 1.6 6 ) %、( 2 6 .12±31 12 ) %、( 2 9.4 2± 1.0 2 ) %、( 15 .32± 2 .0 2 ) % ;(TV PTV) /TV值为 ( 7.2 2± 2 .32 ) %。结论 :采用适形立体定向放射治疗腹腔转移肿瘤 ,TV高度适形PTV 。
Objective:To appraise the dose distribution of stereotactic conformal radiotherapy with CT simulated align for metastasis tumor of abdomen.Methods:38 patients with metastasis tumor of abdomen were collected in this study.After CT localization were finished and the planning target (PTV ) were confirmed,the treatment plans of stereotactic conformal radiotherapy were designed by the rule of that the PTV should be enclosed by the smallest treated volume (TV),and the dose distributon were appraised by 3D TPS.Results:The mean values of the maximal absorbed dose of the bladder,renal,spinal cord and liver was (65.45±2.23)%,(35.22±1.32)%,(60.32±1 13)% and (26.32±2.57)% respectively;and the average absorbed dose was (30.20±1.66)%,(26.12±31.12)%,(29.42±1 02)% and (15.32±2.02)% respectively.The mean value of the maximal dose,the minimal dose and the average dose of the PTVs was (115.30±5.23)%,(88.00±2.32)% and (99.20±1.20)% respectively.Conclusion:The TV can conform the PTV of metastasis tumor of abdomen perfectly when treated by stereotactic conformal radiotherapy with CT simulated align.The PTV can be irradiated with high dose when the organ at risk were protected satisfactorily.
