摘要
目的 探讨多发性硬化 (Muitiplesclerosis,MS)患者听神经损害的部位及其可能机制。方法 对 5 8例MS患者同期检查的脑干听觉诱发电位 (brainstemauditoryevokedpotential,BAEP)以及头颅MRI资料进行分析。 结果 BAEPⅠ波潜伏期延长 2 7例 ,其中双侧延长 5例。Ⅰ波消失单、双侧各 2例。听阈明显增高双侧 5例 (均以一侧为重 ) ,单侧 3例。单侧主观听力障碍 4例。BAEP显示脑干内损害 13例。MRI显示脑干内病灶 11例 ,3例 (5 17% )可观察到听神经颅骨内段条索状长T1长T2 异常信号 ,听神经肿胀。结论 部分MS患者存在听神经损害 ,主要表现为单侧BAEPⅠ波潜伏期延长 ,部分Ⅰ波消失 ,较少部分听神经损害严重者可出现听觉障碍的表现 ,有时听神经MRI检查还可以发现病灶。听神经远端发生脱髓鞘病变是其可能病理生理机制。
Objective To investigate the lesions within the acoustic nerve of patients with multiple sclerosis(MS) as well as its mechanism.Methods The materials of brainstem auditory evoked potentials(BAEP) and head MRI examined in same time of 58 MS patients were analyzed.Results BAEP showed unilateral or bilateral wave Ⅰ with prolonged latency in 27 patients,and unilateral or bilateral absence of wave Ⅰ in 2 patients respectively.BAEP showed high auditory threshold in 5 patients,unilateral hearing impairment in 3 patients. BAEP and MRI showed impairment of brainstem in 13 and 11 patients,respectively.MRI showed abnormal signs in the distal part of the acoustic nerve in 3 patients.Conclusion The results suggest that some MS have lesions in the distal part of the acoustic nerve.The demyelination lesions in acoustic nerve may be its mechanism.
