脊柱椎体单纯性和病理性压缩骨折的磁共振成像 被引量:5

摘要 椎体压缩性骨折在临床上并不少见,病因有骨质疏松、外伤所致的单纯性椎体压缩与病理性压缩骨折。对不同病因引起的椎体压缩性骨折,治疗原则和方法有极大的区别,因此病因的鉴别对临床诊断和治疗有非常重要的意义。本文比较了单纯性和病理性脊柱压缩骨折的不同影像诊断方法及常规磁共振上两者的不同表现与鉴别点,同时详细介绍了磁共振的新技术——弥散技术在鉴别单纯性和病理性脊柱压缩骨折的应用。 Vertebral compression is a common clinical problem and the accurate diagnosis of the causes is important for appropriate treatment and prognosis, so the distinction between benign and malignant cause is critical. Those traditional imaging methods such as plain film, computed tomography and radionuclide scintigraphy are obviously insufficient to access the cause with either low sensitivity or specificity. Although conventional MR imaging has been verified useful in most cases, it is quite difficult to make the final diagnosis because of the similar signal intensity characteristics. As a new imaging method diffusion - weighted MR can reflect the various signal intensity of water molecules under different gradient impulse. In this study, we are to evaluate the effectiveness of diffusion - weighted magnetic resonance imaging for differentiating between benign and pathologic vertebral compression fracture.
