摘要
目的:了解影响预后良好与预后不良首发精神分裂症住院患者预后相关因素。方法:应用一般资料调查表(记录患者的社会人口学资料及某些观察指标(如发病年龄、起病形式、家族史、难产史、病期、诊断分型、病中攻击行为,是否觉得社会歧视等)、阴性症状和阳性症状(SAPS)评定量表、艾森克个性问卷、社会支持评定量表、美国精神疾病诊断标准(DSM-III)轴-V病前1年最佳社会功能评定表及大体功能评定量表对205例连续就诊的首发精神分裂症住院患者进行为期1年的随访评估,分析出院后1年时预后良好者与预后不良者有关因素的差异。结果:预后良好组(n=57)与预后不良组(n=58)患者在受教育水平、职业、病期、病前社会功能、病前个性特征、精神症状特点、起病缓急、感受社会歧视、社会支持、是否维持服药等方面存在显著差异(t=1.99～6.39,χ2=8.28～21.31,P<0.05～0.01)。影响随访时预后的主要因素作用大小的顺序依次是:出院服药情况、社会歧视、注意障碍分量表总评分、思维贫乏分量表总评分及病中有无攻击自杀行为。结论:了解首发精神分裂症患者预后的影响因素,有利于改善患者的预后。
AIM:To explore the the relevant factors of favorable and unfavorable prognosis on first episode schizophrenic. METHODS:Common data inventory,which recorded the social demography data and some observing indices,such as age and form of episode, family history,dystocia history,ill duration, diagnostic typing,aggressive behavior,feeling of being discriminated, scale for the assessment of negative symptoms(SANS)and scale for the assessment of positive symptoms(SAPS),Eysenck personality questionnaire (EPQ),social support rating scale(SSRS),DSM III V,the best scocial functional evaluation and gross functional evaluation,one year follow up was made to 205 inpatients of first episode schizophrenia with successive treatment by analyzed the differences of relevant factors between favorable and unfavorable prognosis patients one year after discharge. RESULTS:There were significant differences in some variables between patients in favorable(n=57)and unfavorable(n=58)prognosis groups,such as educational level, job, ill duration,premorbid level of social functions, premorid characteristics,psychiatric symptom, urgent degree of onset,social discrimination, social supports, maintenance of taking medicine(t=1.99-6.39,χ2=8.28-21.31,P< 0.05-0.01).Main factors,which affected the prognosis of follow-up,were put in order:medicine taking state of discharge, social discrimination, total score of disturbance of attention and poverty of thought,aggressive and suicidal behavior.
