长期实施“全球哮喘防治创议”干预对哮喘患者肺功能和生活质量的影响 被引量:8

摘要 目的:观察长期实施“全球哮喘防治创议”(GlobalInitiativeforAsthma,GINA方案)干预对哮喘患者肺功能及生活质量的影响。方法:对62例哮喘稳定期患者(其中6例失访)按照GINA方案进行长期干预、管理和治疗,于长期干预前、干预6,12,24个月后分别测定肺功能及进行生活质量的评估;并对其前后1年内人均医疗费用、住院和急诊次数、误工误学天数进行对比分析。结果:长期干预前、干预6,12,24个月后生活质量各项评分指标改善有显著性意义(t=3.98,4.13,4.56;P均<0.01);肺功能第1秒用力呼气量(FEV1)和FEV1/预计值改善有显著性意义(t=3.87,4.64,5.73,P均<0.01),时间越长,改善越显著;年均住院和急诊次数、误工误学天数明显减少,年均医疗费用明显降低(t=12.41,10.12,9.56;P均<0.01)。结论:长期实施GINA方案干预能明显改善哮喘患者肺通气功能,减少发作次数,提高生活质量,降低医疗费用。 AIM: To observe the effect of long term implement intervention of Global Initiative for Asthma(GINA) on the lung function and life quality in patients with asthma. METHODS:Sixty two asthma patients of stationary phase(6 of them lost during the follow up)were under long term intervention, management and treatment according to GINA.They were measured lung function and evaluated life quality at the beginning of the intervention and at the 6,12,24 months after it.The annual mean medical cost, number of times of being in hospital and emergency treatment, number of days of delay and absence from work or study were recorded for contrast analysis. RESULTS:Scores of each item of life quality were significantly improved at the beginning of the intervention and at the 6,12,24 months after it (t=3.98,4.13,4.56,P< 0.01). Forced expiratory volume in first second(FEV1) of pulmonary function and FEV1/ predictive values improved significantly(t=3.87,4.64,5.73,P< 0.01),the longer it lasted, the more significant the improvement was. Number of times of being in hospital and emergency treatment and number of days of delay and absence from work or study decreased significantly, and the mean medical cost reduced significantly(t=12.41,10.12,9.56,P< 0.01).
