摘要 目的 研究双表型急性白血病 (BAL)生物学特征和临床特征。方法 对 5 39例成人急性白血病 (AL)进行MIC分型实验研究并对其中的 7例进行临床治疗结果观察研究。结果 ①BAL有 36例占总AL的 6 .7%。② 36例BAL中B 髓系占6 3.9%、T 髓系 2 2 .2 %和T B 髓系 13.9%。③ 36例BAL中CD34阳性率 75 .0 % .④ 36例BAL中正常核型 5 2 .8%、异常核型 4 7.2 % ,Ph染色体占总BAL的 2 2 .1%、异常染色体BAL的 4 7.2 %。⑤治疗结果 :7例BAL中CR 4例、NR 3例 ,CR 4例中 3例系t(8;2 1)、并且系CD19阳性的B 髓系BAL。结论 BAL的发生率很低 ,以B 髓系最常见 ,高表达CD34。预后与染色体核型有关。 Objective To study the biological and clinical features of biphenotypic acute leukemia(BAL). Methods 539 cases of acute leukemia(AL) were studied with morphological/cytochemical,immunological and cytogenetic classification(MIC) and 7 cases of 539 AL were treated with DA/DOAP. Results ①BAL was 6.7% in AL. ②In BAL,the B-myeloid lineage BAL was 63.9%,T-myeloid lineage BAL was 22.2% and T-B-myeloid lineage was 13.9%. ③In BAL,the positive rate of CD34was 75.0%. ④In BAL,normal karyotype was 52.8% and abnormal karyotype was 47.2%,Ph chromosome was 22.1% in all BAL and 47.2% in abnormal karyotype BAL. ⑤The treat results:4 cases of 7 BAL were complete remission(CR).Conclusion The incidences of BAL was very lower. In BAL,B-myeloid lineage BAL was the commonest subtype. The expression of CD34 was higher.The prognosis was correlated with chromosomal karyotype.
