摘要
目的 探讨长时间的腹腔镜手术对机体的影响 ,可为今后开展腹腔镜下高难度手术积累经验及提供理论依据。方法 回顾性分析 1999年 6月~ 2 0 0 2年 12月手术时间在 2 10min以上的 39例作为研究对象 ,其中分为良性组 19例、恶性组 2 0例。随机抽取同期手术时间少于 2 10min者 35例作为对照组。结果 在研究组中 ,恶性组比良性组具有更长的手术时间、较多的术中出血量、较高的输血率、较多的CO2 使用量、体温较晚恢复正常、尿管拔除时间晚、术后住院时间较长 ,但在术后排气时间、WBC数及可能出院时间方面两组间无显著差异 ;良性组仅在CO2 使用量、术后WBC数值方面明显高于对照组 ,其他方面均与对照组无显著性差异。结论 长时间的腹腔镜手术对患者的机体及术后恢复有一定影响 ,其中恶性组的影响大于良性组 ,尽快提高腹腔镜手术技巧 ,缩短手术时间 ,可减少患者的创伤 ,促进早日康复。
Objective:To investigate the influnce of long-time laparoscopy on organism ,accumulating experience and providing scientific basis for carrying out the more difficult laparoscopy from now on.Methods: A prospective study was conducted from June 1999 to Dec. 2002,39 cases of laparoscopy operative time over 210 min were used as the study group,in which nonmalignancy 19 cases and malignancy 20 cases;radom sampling 35 cases laparoscopy operating time lower 210 min were used as the control group. Results:The malignancy group had a significantly longer operative time( P <0.05)more estimated blood loss( P <0.05) highter blood transfusion rate( P <0.05) more CO 2 using volum( P <0.001) slower urinate recovery time ( P <0.05) longer temperature recovery normal time( P < 0.05)longer hospital stay( P < 0.001) than nonmalignancy group. But postoperative gastrointestinal recovery time and probable hospital discharge were found no differences between malignancy and nonmalignancy. The nonmalignancy group had a significantly more CO 2 using volume( P <0.001)and higher the number of postoperative WBC( P <0.001) than the control group. On the other hand were found no differences between nonmalignancy and the control group. Conclusions:The long-time laparoscopy made to a certain extent effect on the organism and postoperative recovery,in which the malignancy group had more serious than the nonmalignancy. If we improve laparoscopic technique as soon as possible to shorten operative time ,we should reduce damage of organism and promote recovery earlier. 收稿日期:2004-02-18
