摘要
目的 :为髋臼前柱拉力螺钉内固定提供解剖学基础。方法 :取半骨盆标本 3 0个 ,自髋臼切迹至髋臼上缘作系列截骨面 ,用一斯氏针由最小截面的圆心逆行打入 ,在髂骨后外侧穿出点为P。将该截面下方的系列截面解剖复位 ,再将斯氏针顺行打出 ,测量其在髋臼前柱骨皮质内的长度。作一参考线AB ,其中A为髂前上、下棘间的切迹 ,B为坐骨大切迹 ,并作AB的中垂线CD。结果 :P位于AB的中垂线上 ,P点距AB的距离为 (15 .3± 4.7)mm ,斯氏针与AB的夹角为 (90 .1± 4.7)° ,与CD的夹角为 (2 5 .3± 3 .9)° ,髋臼前柱骨皮质内斯氏针长度为 (82 .0± 7.9)mm。最小截面位于髋臼切迹上方 15 .0mm处 ,该截面平均直径为 :(5 .2± 1.9)mm。结论 :髋臼前柱拉力螺钉的入钉点在髂前上下棘之间切迹与坐骨大切迹连线中点上方 (15 .3± 4.7)mm处 ,其进针方向与该线呈 (90 .1± 4.7)° ,与该线中垂线呈 (2 5 .3± 3 .9)° ,该拉力螺钉直径为 (5 .2± 1.9)mm ,长度为 (82 .0± 7.9)mm。
Objective: To provide anatomical basis for the placement of lag screw in the acetabular anterior column. Methods: Thirty hemipelves were harvested for this study. All specimens were sectioned at 5.0 mm intervals perpendicularly to the anterior surface of the anterior column. Successive cross-sections were made from the inferior bony acetabular margin to the superior bony margin of the acetabulum. A Steinmann pin was put retrogradely through the center point of the smallest cross-section, then the cross sections below were repositioned anatomically, and the Steinmann pin was forced downward with the help of a drill. The functional axis and the length of the lag screw of the anterior column were determined. A line designated AB was drawn to connect the deepest portion of the sciatic notch (B) and the anterior interspinous notch (A) .Line CD was perpendicular to the line AB and passed the midpoint of line AB. The angle formed by the Steinmann pin and the line AB was named angle α. The angle between the Stenmann pin and the line CD was named angle β. Results: P was just on the line CD. The distance between P and line AB was 15.3±4.7 mm, and angle α was 90.1°±4.7°,angle β 25.3°±3.9°. The mean length of the Steinmann pin in the anterior column was 82.0±7.9 mm. The smallest cross-section was 15.0 mm superior to the inferior bony acetabular margin. The diameter of this section averaged 5.2±1.9 mm. Conclusion: The enter point of the anterior column was 15.3±4.7 mm superior to the midpoint of line AB connecting the deepest portion of the sciatic notch and the anterior interspinous notch . The mean length of the lag screws for anterior column fixaion was 82.0±7.9 mm.The mean inclination of the lag screw was 90.1°±4.7° for angle α and 25.3°±3.9° for angle β. The mean diameter of the lag screw was 5.2±1.9 mm.
