摘要
目的 探讨成熟期和过熟期白内障超声乳化手术中连续环形撕囊的方法并评价。方法 对 484例 513眼核硬度Ⅲ级以上的白内障 ,分别行黏弹剂下常规连续环形撕囊和水下连续环形撕囊 ,并常规行晶状体乳化吸出和后房人工晶状体植入。结果 黏弹剂下连续环形撕囊 2 3 7眼 ,成功 2 0 6眼 ,占 86 96% ;水下连续环形撕囊 2 76眼 ,成功 2 72眼 ,占 98 55%。二者差异具有非常显著性意义 (χ2 =14 0 7,P <0 0 0 1)。所有病例均顺利进行了后房人工晶状体植入 ,术后未发生严重并发症。
ObjectiveTo evaluate the methods of capsulorh ex is in phacoemulsification of mature or hypermature cataracts and its effects.Methods513 eyes of cataracts in 484 cases with grade Ⅲ nuclear h ardness or more than it were performed by standard continuous curvilinear capsul orhexis (CCC) under viscoelastic agent or subaqueous-CCC respectively,phacoemuls ification and posterior chamber intraocular lens implantation were also carried out.Results237 eyes were performed by standard-CCC under visco elastic materials and 206 eyes were successful (86.96%);276 eyes were taken suba queous-CCC and 272 eyes completely perfect (98.55%),the difference was significa nt between the two methods(χ 2=14 07,P<0 001).The success rates of s tandard-CCC under viscoelastic agent among cataracts which in grade Ⅲ,Ⅳ and Ⅴ nuclear hardness were 91.20%,87.95% and 79.37% respectively,whereas the rates of subaqueous-CCC were 100%,98.94% and 96.15% respectively.The differences betw een the two methods in the three categories hard nuclei were all statistically s ignificant (χ 2=7.35,7.44 or P<0.01 respectively).Intraocular lens w ere performed uneventfully for all cases.No severe postoperative complication oc curred.ConclusionSubaqueous-CCC has been shown to be a safely practicable approach for insufficient red fundus reflex and liquid cortex of mat ure or hypermature cataracts.
