摘要
目的 :探讨新辅助化疗对胃癌患者细胞免疫功能及胃癌细胞凋亡与增殖的影响。方法 :进展期胃癌患者 6 0例 ,随机分为 3组 (每组 2 0例 ) ,A组为单纯手术 ,B组为LF +手术 ,C组为LOF +手术。分别应用免疫荧光法检测T细胞亚群 ,核素释放法测定NK细胞活性 ,TUNEL法和免疫组织化学染色法检测胃癌细胞的凋亡 (AI)与增殖 (PI)及二者之比 (AI/PI)。结果 :B、C组与A组AI、PI差异具有极显著意义 ,P <0 0 1 ;AI/PI差异有显著意义 ,P <0 0 5。化疗前、后及术后 3组T细胞亚群的百分率差异无显著意义 ,P >0 0 5。化疗后及术后 ,B、C组较A组NK细胞活性明显降低 ,P <0 0 5 ,P <0 0 1。结论 :新辅助化疗可明显促进胃癌细胞凋亡并抑制其增殖 ,对手术期患者的T细胞亚群免疫功能无明显影响 。
Objectives To investigate whether neoadjuvant chemotherapy could induce tumor cell apoptosis in gastric carcinoma patients, and its effect on cellular immunity and cell proliferation.Methods Sixty cases of advanced gastric cancer were randomly divided into three groups (A,B and C),20 cases each group:Group A,operation;Group B,LF+operation;Group C,LOF+operation.The percentages of T cell subset in perpheral venous blood and NK cell activity were detected with the immuno flurescence assay and nuclein release methods.Proliferative index(PI) of gastric cancer cells and ratio of AI and PI were detected by using the TUNEL and immuno histochemical staining.Results There were very significant differences of AI and PI between the Group B,C and A, P <0 01,and there was significant difference of AI/PI between them also, P <0 05.Before and after treatment and after operation,there was no significant difference of the percentages of T cell subset ratio among the 3 groups, P >0 05.There were significant differences of NK cell activities after treatment and after operation between the Group B,C and A, P <0 05, P <0 01.The NK cell activities in the Group B and C were much lower than that in Group A.Conclusion Neoadjunvant chemotherapy can markedly enhance apoptosis of gastric cancer cells and inhibites their proliferation,and it has no significant affection on the immunological function of T cell subset but can markedly inhibites the NK cell activity.
