摘要
目的 :探讨健胃愈疡颗粒 (JYG)抗消化性溃疡 (PU)复发的机制。方法 :以白细胞介素 lβ(IL 1β)所致胃溃疡复发大鼠为模型 ,以JYG、胃苏颗粒治疗干预 ,取胃组织检测胃溃疡指数 (GUI)、bcl 2、bax的表达。临床观察随机分为JYG组和洛赛克组 ,服药 4周 ,取胃粘膜组织检测fas/fasL、雌激素结合蛋白 (hTFF1)、表皮生长因子受体 (EGFR)的表达。结果 :JYG可使胃GUI、溃疡复发率明显降低 ,临床疗效和溃疡愈合率与西药组相当 ,但一年随访复发率明显低于对照组。JYG增加胃粘膜凋亡相关蛋白bcl 2、hTFF1、EGFR蛋白表达 ,提高bcl 2 /bax比值 ,使胃粘膜上皮细胞凋亡减少 ;改善溃疡愈合后再生粘膜组织成熟度。与对照组比较差异有统计学意义。能显著降低肝郁脾虚或肝胃不和的幽门螺杆菌 (Hp)阳性PU患者胃粘膜组织fas/fasL蛋白的表达。 结论 :JYG可通过调节凋亡相关蛋白表达 ,改善溃疡愈合质量 。
Objective: To investigate the molecular biological mechanism of Jianwei Yuyang granule(JYG) in preventing peptic ulcer relapse in both animal experiment and clinic patients.Methods:Rat model of peptic ulcer relapse was induced by interleukin 1β(IL 1β).After treated with JYG and Weishu granule, gastric tissue was obtained to examine gastric ulcer index(GUI), expression of bcl 2 and bax protein. In clinical research, the patients were treated with JYG and Losec respectively. After 4 weeks of treatment, fas/fasL, hTFF 1, EGFR protein expression of the gastric mucosal tissue were examined.Results:JYG increased the expression of bcl 2 protein and the ratio of bcl 2/bax in gastric mucosa, reduced epithelial cell apoptosis. JYG ameliorated histological and ultrastructural features of the gastric mucosa in areas of healed ulcers. The expression of hTFF 1 and EGFR increased in JYG andcontrol group.JYG obviously reduced hyperapoptosis and hyperproliferation of gastric mucosal epithelial cell of 'stagnation of liver qi and deficiency of spleen' or 'incoordination between liver and stomach' PU patients with Hp infection.Conclusion: JYG can prevent peptic ulcer and its relapse through regulating expression of apoptosis related protein and improving the heal quality of gastric ulcer in both experimental animals and clinic patients.
