摘要
目的 :探讨复方丹参注射液配合脑脊液置换术治疗蛛网膜下腔出血的临床疗效。方法 :观察采用复方丹参注射液合脑脊液置术治疗蛛网膜下腔出血 75例 (治疗组 )与未加用复方丹参注射液合脑脊液置换术治疗蛛网膜下腔出血 5 3例 (对照组 )的治疗效果、头痛消失时间、脑血管痉挛 ( CVS)、脑积水、再出血发生率、心电图异常发生率、心电图异常恢复时间。结果 :治疗组总有效率较对照组高 ( P <0 .0 5 ) ;头痛缓解时间缩短 ( P <0 .0 5 ) ;再出血率 ( P >0 .0 5 )、脑血管痉挛发生率 ( P <0 .0 5 )、脑积水发生率 ( P <0 .0 5 )、心电图异常率 ( P <0 .0 1)降低 ;心电图异常恢复时间缩短 ( P <0 .0 5 )。结论 :复方丹参注射液合脑脊液置换术能有效地改善蛛网膜下腔出血的症状 ,提高治愈率 ,保护心肌 ,预防脑血管痉挛 ,减轻蛛网膜粘连 ,防止脑积水 ,对降低病死率具有重要意义。
Objective:To study the clinical effect of compound Danshen injection and spinal fluid replacement on subarachnoid hemorrhage(SAH).Method:75 cases of SAH were treated with compound Danshen injection and spinal fluid replacement on the basis of routine therapy.And 53 cases treated only with routine therapy were set up as the control.Restoration time of headache, cerebrovascular spasm,recurrence rate of hemorrhage,abnormal rate of electrocardiogram(ECG),restoration time of ECG were detected.Result:Compared with the control,the treatment group has higher effective rate(P<0 05),shorter restoration time of headache(P<0 05) and ECG(P<0 05),lower recurrence rate of hemorrhage(P<0 05),incidence of cerebrovascular spasm(P<0 05)?hydrocephalus(P<0 05)and abnormal rate of ECG(P<0 01).Conclusion:compound Danshen injection and spinal fluid replacement can improve the symptom of SAH,increase the cure rate,protect cardiac muscles,prevent cerebrovascular spasm and hydrocephalus,reduce the adhesion of arachnoid membrane.It has significant effect of decreasing the mortality of SAH.
