非创伤性预处理对心脏复苏的影响 被引量:1

摘要 目的 以心脏为靶器官观察短暂下肢缺血预处理对心肺复苏术 (CPR)的影响。方法 以窒息法复制大鼠死亡模型 ,随机分为对照组、短暂下肢缺血预处理组、复苏前后予去甲肾上腺素处理组及复苏前后予腺苷处理组。观察窒息至死亡时间、复苏成功率、自主循环恢复率、复苏成功后血清心肌酶及心肌超微结构。结果 短暂下肢缺血预处理组窒息至死亡时间明显长于对照组 (P <0 0 1) ;复苏成功率大于对照组 (P <0 0 5 ) ;自主循环恢复率大于对照组 (P <0 0 5 ) ;复苏成功后心肌酶明显低于对照组 (P <0 0 1) ,且心肌超微结构损伤小。结论 短暂下肢缺血预处理对CPR具正面影响 ,并由儿茶酚胺受体及腺苷受体部分介导。 Objective To evaluate the effect of transient thigh ischemic precondition(TTIP)and drug precondition on cardio-pulmonary resuscitation(CPR).Methods Healthy male Wistar rats performed asphyxia cardiac arrest and resuscitation were randomly divided into control group(C),TTIP group,norepinephrine treatment before or after CPR group(NEPCⅠ/NEPCⅡ),adenosine treatment before or after CPR group(AdoⅠ/AdoⅡ). To measure time of death,rate of successful CPR,rate of recovery of autocirculation,CK-MB in blood plasma,ultrastructure of heart tissue.Results Time of death of group TTIP was the longest(P<0.01),the others had no significant difference.Rate of successful CPR was higher in group TTIP than in group C and group Ado(P<0.05),but didn't differ from group NEPC significantly.Rate of recovery of autocirculation was higher in each group than in group C(P<0.05), but didn't differ from group TTIP significantly.CK-MB in blood plasma were lower in group TTIP and NEPC than in group C(P<0.01),but they had no significant difference.Ultrastructure of group TTIP was better than that of group C.Conclusion TTIP had positive effect on CPR,adrenergic or adenosine receptors may partly mediated the protection of precondition.
